การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาคุณภาพชีวิต ประสบการณ์การมีอาการ และกลวิธีการจัดการกับอาการของผู้ป่วยที่มีภาวะอุดกั้นทางเดินปัสสาวะที่ได้รับการใส่สายระบายปัสสาวะที่กรวยไตผ่านผิวหนัง และศึกษาความสัมพันธ์ระหว่างประสบการณ์การมีอาการ แรงสนับสนุนทางสังคม และระดับความรุนแรงของผิวหนังถูกทำลายรอบสายระบายปัสสาวะ กับคุณภาพชีวิตของผู้ป่วยที่มีภาวะอุดกั้นทางเดินปัสสาวะที่ได้รับการใส่สายระบายปัสสาวะที่กรวยไตผ่านผิวหนัง กลุ่มตัวอย่าง จำนวน 95 คน ได้รับการคัดเลือกโดยวิธีการสุ่มอย่างง่าย ที่ได้รับการวินิจฉัยว่ามีภาวะอุดกั้นทางเดินปัสสาวะจากการลุกลามของมะเร็งในอุ้งเชิงกรานและได้รับการใส่สายระบายปัสสาวะที่กรวยไตผ่านผิวหนังตั้งแต่ 3 เดือนขึ้นไป กลับมารับการติดตามอาการและเปลี่ยนสายระบายปัสสาวะที่กรวยไตผ่านผิวหนัง ที่โรงพยาบาลตติยภูมิ กรุงเทพมหานคร เก็บรวบรวมข้อมูลด้วยแบบสอบถาม ได้แก่ ข้อมูลส่วนบุคคล คุณภาพชีวิต ประสบการณ์การมีอาการและกลวิธีการจัดการกับอาการ แรงสนับสนุนทางสังคม และแบบสังเกตระดับความรุนแรงของผิวหนังถูกทำลาย คุณภาพของเครื่องมือผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน และเครื่องมือส่วนที่ 2-4 มีค่าความเที่ยงเท่ากับ .91, .91 และ .93 ตามลำดับ เครื่องมือชุดที่ 5 ผ่านการตรวจสอบความเท่าเทียมกันของแบบสังเกต ได้ค่าความที่ยงเท่ากับ .90 วิเคราะห์ข้อมูลโดยสถิติเชิงบรรยาย สถิติสหสัมพันธ์เพียร์สัน และสถิติสหสัมพันธ์สเปียร์แมน ผลการวิจัยสรุปได้ดังนี้ 1. คุณภาพชีวิตโดยรวมของผู้ป่วยที่มีภาวะอุดกั้นทางเดินปัสสาวะที่ได้รับการใส่สายระบายปัสสาวะที่กรวยไตผ่านผิวหนังอยู่ในระดับปานกลาง (Mean= 66.34, SD=14.55) 2. ประสบการณ์การมีอาการมีความสัมพันธ์ทางลบ (r= -.54, p<.05) แรงสนับสนุนทางสังคมมีความสัมพันธ์ทางบวก (r= .27, p<.05) และระดับความรุนแรงของผิวหนังถูกทำลายมีความสัมพันธ์ทางลบ (r= -.35, p<.05) กับคุณภาพชีวิตของผู้ป่วยที่มีภาวะอุดกั้นทางเดินปัสสาวะที่ได้รับการใส่สายระบายปัสสาวะที่กรวยไตผ่านผิวหนัง 3. ประสบการณ์การมีอาการของผู้ป่วยที่มีภาวะอุดกั้นทางเดินปัสสาวะที่ได้รับการใส่สายระบายปัสสาวะที่กรวยไตผ่านผิวหนังจำนวน 95 คน ที่พบมากในช่วง 1 สัปดาห์ที่ผ่านมา 5 อันดับแรก คือ ร้อยละ 50.5 มีอาการคันผิวหนังบริเวณที่ปิดพลาสเตอร์รอบสายระบาย รองลงมา คือ ร้อยละ 43.2 มีอาการท้องผูก ร้อยละ 36.8 มีอาการนอนไม่หลับหรือนอนหลับยาก ร้อยละ 36.8 รู้สึกเบื่ออาหาร และร้อยละ 33.7 มีอาการวิตกกังวล 4. กลวิธีการจัดการกับอาการ 5 อันดับแรกที่มีประสิทธิภาพมากที่สุดสำหรับอาการเหล่านั้น คือ เช็ดผิวหนังรอบสายระบายด้วยแอลกอฮอล์เมื่อมีอาการคันผิวหนังบริเวณที่ปิด พลาสเตอร์รอบสายระบาย รับประทานยาระบายเมื่อมีอาการท้องผูก รับประทานยานอนหลับเมื่อมีอาการนอนไม่หลับหรือนอนหลับยาก เปลี่ยนประเภทอาหารเมื่อรู้สึกเบื่ออาหาร และ การเล่าให้ผู้อื่นฟังเมื่อวิตกกังวล

การวิจัยแบบบรรยายเชิงทำนายนี้มีวัตถุประสงค์เพื่อศึกษาอำนาจการทำนายอาการเหนื่อยล้าของผู้ป่วยโรคหลอดลือดสมองจากปัจจัยระดับน้ำตาลในเลือด ความบกพร่องทางการสื่อสาร รอยโรค ความรุนแรงของโรค ความเครียด และกิจกรรมทางกาย โดยใช้กรอบแนวคิดของเลนซ์และคณะ (1987) ร่วมกับการทบทวนวรรณกรรม กลุ่มตัวอย่างคือ ผู้ป่วยโรคหลอดเลือดสมอง อายุระหว่าง 20-59 ปี จำนวน 120 ราย ที่มารับบริการ ณ หอผู้ป่วยนอก แผนกอายุกรรมและศัลยกรรมระบบประสาท โรงพยาบาลแพร่และโรงพยาบาลลำปาง ระหว่างเดือนกรกฎาคม ถึง เดือนตุลาคม พ.ศ. 2560 กลุ่มตัวอย่างได้มาจากการเลือกแบบเจาะจง เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลได้แก่ 1)เครื่องตรวจวัดระดับน้ำตาลในเลือด 2)แบบสอบถามอาการเหนื่อยล้า 3)แบบสอบถามความสามารถในการสื่อสาร 4) แบบสอบถามความเครียด 5)แบบสอบถามกิจกรรมทางกาย แบบสอบที่ 2,3,4 มีค่าดัชนีความตรงตามเนื้อหา เท่ากับ 1.00, .94, และ .88 ตามลำดับ และมีค่าความเที่ยงจากการทดสอบสัมประสิทธิ์แอลฟาครอนบาค ของแบบสอบถามที่ 2,3,4,5 เท่ากับ .86,.81,.83 และ .72 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติพหุคูณแบบขั้นตอน ผลการวิจัยพบว่า 1) ผู้ป่วยโรคหลอดเลือดสมองที่มีอาการเหนื่อยล้า ร้อยละ 37.5 โดยมีค่าคะแนนอาการเหนื่อยล้าเฉลี่ยเท่ากับ 3.63 (S.D.=1.18) 2) ระดับน้ำตาลในเลือดและความเครียดมีความสัมพันธ์ทางบวกกับอาการเหนื่อยล้าของผู้ป่วยโรคหลอดเลือดสมองอย่างมีนัยสำคัญทางสถิติ (r=.32,p<.05 และ r=.68,p<.05ตามลำดับ) 3) กิจกรรมทางกายมีความสัมพันธ์ทางลบกับอาการเหนื่อยล้าของผู้ป่วยโรคหลอดเลือดสมอง อย่างมีนัยสำคัญทางสถิติ (r=-.35,p<.05) 4) ความบกพร่องทางการสื่อสาร รอยโรคและความรุนแรงของโรคไม่มีความสัมพันธ์กับอาการเหนื่อยล้าของผู้ป่วยโรคหลอดเลือดสมอง 5) ความเครียดและระดับน้ำตาลในเลือดสามารถร่วมกันทำนายอาการเหนื่อยล้าของผู้ป่วยโรคหลอดเลือดสมองได้ ร้อยละ49 (R2=.49)

การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง เพื่อเปรียบเทียบผลของโปรแกรมการปรับพฤติกรรมการนอนต่ออาการนอนไม่หลับในผู้ป่วยหัวใจล้มเหลว กลุ่มตัวอย่าง คือ ผู้ป่วยหัวใจล้มเหลว ที่มารับการรักษาแบบผู้ป่วยนอกที่ห้องตรวจโรคหัวใจ และห้องตรวจโรคอายุรกรรม ณ โรงพยาบาลพระมงกุฎเกล้า โดยใช้วิธีการคัดเลือกกลุ่มตัวอย่างตามคุณสมบัติที่กำหนด จำนวน 40 คน สุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 คน โดยจับคู่ให้ทั้งสองมีความคล้ายคลึงกันในเรื่อง ระดับความรุนแรงของภาวะหัวใจล้มเหลว และระดับอาการนอนไม่หลับก่อนการทดลอง โดยกลุ่มควบคุมได้รับการพยาบาลตามปกติ ขณะที่กลุ่มทดลองได้รับโปรแกรมการปรับพฤติกรรมการนอน ของ Troxel et al.(2012) ที่ประกอบด้วยการให้ความรู้เกี่ยวกับสุขวิทยาการนอนหลับ หลักการปรับพฤติกรรมการนอนหลับ (การจำกัดการนอนและการควบคุมสิ่งเร้า) และการให้ผู้ป่วยปฏิบัติตามตารางการนอนหลับอย่างต่อเนื่องที่บ้าน เครื่องมือที่ใช้ในการเก็บข้อมูล คือ แบบสอบถามอาการนอนไม่หลับของ Morin (1993) ฉบับที่แปลเป็นภาษาไทยโดย พัทรีญา แก้วแพง (2547) ได้รับการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิ 4 ท่าน และหาค่าความเที่ยงสัมประสิทธิ์อัลฟาของครอนบาคได้เท่ากับ 0.87 วิเคราะห์ข้อมูลโดยใช้สถิติ ร้อยละ ค่าเฉลี่ย และ t-test ผลการวิจัยสรุปได้ดังนี้ 1. อาการนอนไม่หลับของผู้ป่วยหัวใจล้มเหลวในหลังได้รับโปรแกรมการปรับพฤติกรรมการนอนน้อยกว่าก่อนได้รับโปรแกรมการปรับพฤติกรรมการนอน อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. อาการนอนไม่หลับของผู้ป่วยหัวใจล้มเหลวในกลุ่มที่ได้รับโปรแกรมการปรับพฤติกรรมการนอนน้อยกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การศึกษาวิจัยครั้งนี้เป็นการวิจัยแบบกึ่งทดลองแบบศึกษาสองกลุ่ม วัดก่อนและหลังการทดลอง (two groups pretest-posttest design) โดยมีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการสร้างพลังอำนาจและการออกกำลังกายแบบต้านแรงต่อภาวะเปราะบางของผู้สูงอายุในชุมชน กลุ่มตัวอย่าง คือ ผู้ที่มีอายุเกิน 60 ปีบริบูรณ์ขึ้นไปทั้งชายและหญิงที่มีภาวะเปราะบาง อาศัยอยู่ในชุมชนแห่งหนึ่งใน กรุงเทพมหานคร จำนวน 44 คน แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 22 คน ทำการคัดเลือกกลุ่มตัวอย่าง ด้วยวิธีการจับคู่ในด้าน เพศ อายุและค่าดัชนีมวลกาย โดยกลุ่มทดลองได้รับโปรแกรมการสร้างพลังอำนาจตามแนวคิดของ Zimmerman (2000) และการออกกำลังกายแบบต้านแรงมาใช้ ในขณะที่กลุ่มควบคุมจะได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการทดลอง ประกอบด้วย โปรแกรมการสร้างพลังอำนาจและการออกกำลังกายแบบต้านแรงโดยใช้ดัมเบลล์เป็นอุปกรณ์ในการทดลอง เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบสอบถามข้อมูลส่วนบุคคล แบบทดสอบสมรรถภาพสมองเบื้องต้นฉบับภาษาไทย แบบประเมินความสามารถในการทำกิจวัตรประจำวันพื้นฐานและแบบสอบถามภาวะเปราะบาง ระยะเวลาดำเนินการวิจัยจนถึงเก็บรวบรวมข้อมูลสิ้นสุด 5 สัปดาห์ วิเคราะห์ ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติทดสอบที ผลการวิจัยสรุปได้ดังนี้ 1. ภาวะเปราะบางของผู้สูงอายุหลังการได้รับโปรแกรมการสร้างพลังอำนาจและการออกกำลังกายแบบต้านแรงมีภาวะเปราะบางลดลง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ภาวะเปราะบางของผู้สูงอายุที่ได้รับโปรแกรมการสร้างพลังอำนาจและการออกกำลังกายแบบต้านแรงมีภาวะเปราะบางลดลงกว่ากลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยแบบกึ่งทดลองนี้ เพื่อศึกษาผลของโปรแกรมส่งเสริมการรับรู้ประโยชน์และลดการรับรู้อุปสรรคต่อพฤติกรรมการรับประทานอาหารในผู้ป่วยมุสลิมโรคหลอดเลือดหัวใจ กลุ่มตัวอย่างคือ ผู้ป่วยมุสลิมโรคหลอดเลือดหัวใจเข้ารับบริการที่เเผนกอายุรกรรมโรงพยาบาลปัตตานี คัดเลือกตามคุณสมบัติที่กำหนด แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 22 คน จับคู่ให้มีลักษณะคล้ายคลึงกันในเรื่อง เพศ อายุ และระดับการศึกษา กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมส่งเสริมการรับรู้ประโยชน์และลดการรับรู้อุปสรรคร่วมกับการได้รับการพยาบาลตามปกติ ผู้วิจัยพัฒนาขึ้นโดยประยุกต์ใช้แนวคิดแบบแผนความเชื่อด้านสุขภาพ (Becker, 1974) ใช้ระยะเวลาในการดำเนินกิจกรรม 4 สัปดาห์ เครื่องมือที่ใช้ในการเก็บรวมรวมข้อมูล ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามพฤติกรรมการรับประทานอาหารในผู้ป่วยมุสลิมโรคหลอดเลือดหัวใจ แบบสอบถามการรับรู้ประโยชน์และการรับรู้อุปสรรคของพฤติกรรมการรับประทานอาหารในผู้ป่วยมุสลิมโรคหลอดเลือดหัวใจ ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน มีค่าความตรงตามเนื้อหาเท่ากับ .92 เเละ .83 ตามลำดับ เเละมีค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .70 และ .73 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติที ผลการวิจัยพบว่า 1. คะแนนเฉลี่ยพฤติกรรมการรับประทานอาหารในผู้ป่วยมุสลิมโรคหลอดเลือดหัวใจกลุ่มทดลอง ภายหลังได้รับโปรแกรมส่งเสริมการรับรู้ประโยชน์และลดการรับรู้อุปสรรคสูงกว่าก่อนได้รับโปรแกรมส่งเสริมการรับรู้ประโยชน์และลดการรับรู้อุปสรรคอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนเฉลี่ยพฤติกรรมการรับประทานอาหารในผู้ป่วยมุสลิมโรคหลอดเลือดหัวใจ กลุ่มที่ได้รับโปรแกรมส่งเสริมการรับรู้ประโยชน์และลดการรับรู้อุปสรรคสูงกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การศึกษาครั้งนี้เป็นการวิจัยเชิงคุณภาพมีวัตถุประสงค์เพื่อบรรยายประสบการณ์ของพยาบาลที่ดูแลญาติผู้สูงอายุในครอบครัวที่มีภาวะสมองเสื่อม โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาของ Heidegger ผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพที่มีประสบการณ์ดูแลผู้สูงอายุในครอบครัวที่มีภาวะสมองเสื่อมจำนวน 14 คน โดยคัดเลือกผู้ให้ข้อมูลแบบเจาะจง เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึก การสังเกต การบันทึกภาคสนาม และนำข้อมูลทั้งหมดมาประมวลและวิเคราะห์เนื้อหาตามวิธีการของ van Manen (1990) ผลการศึกษาประสบการณ์ของพยาบาลที่ดูแลญาติผู้สูงอายุในครอบครัวที่มีภาวะสมองเสื่อมมีดังต่อไปนี้ 1. สังเกตเห็นอาการผิดปกติของผู้สูงอายุ ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 1.1) พฤติกรรมการดำเนินชีวิตเปลี่ยนไป 1.2) โวยวาย ตะโกนด่า พูดจาหยาบคาย และ 1.3) หลงลืมหลายเรื่อง ถามต่อเนื่องซ้ำไปซ้ำมา 2. พยาบาลถูกเสนอเป็นผู้ดูแลหลัก ไม่ยึกยักรับได้จากหลายเหตุผล ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 2.1) หน้าที่ของลูกต้องดูแลบุพการี 2.2) ตอบแทนบุญคุณที่ท่านลี้ยงดูเมื่อเยาว์วัย และ 2.3) เป็นแบบอย่างให้ลูกของตนได้ปฏิบัติตาม 3. เริ่มต้นจากดูแลเหมือนคนทั่วไป ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 3.1) มั่นใจว่าทำได้ แค่ดูแลการใช้ชีวิตประจำวัน 3.2) หากไปทำงานวานผู้อื่นดูแลให้ และ 3.3) หงุดหงิดใจหากท่านทำในสิ่งที่ไม่พึงประสงค์ 4. หาความรู้สร้างความเข้าใจ พร้อมรับปรับการดูแลใหม่ ประกอบด้วย 6 ประเด็นย่อย ได้แก่ 4.1) ใส่ใจกิจวัตรประจำวัน 4.2) จัดการและป้องกันอาการไม่พึงประสงค์ 4.3) ดูแลเพื่อคงความสามารถในการช่วยเหลือตนเองได้ 4.4) พาไปตรวจตามแพทย์นัด 4.5) จัดสรรเวลาพาเที่ยวนอกบ้าน และ 4.6) ชวนท่านทำบุญทำทาน ไหว้พระสวดมนต์ 5. ปัญหามากมาย เหนื่อยกายและใจ ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 5.1) รับผิดชอบงานหลากหลาย ร่างกายพักผ่อนไม่พอ และ 5.2) รู้สึกท้อ เครียดง่าย และเหนื่อยใจ 6. หาวิธีการจัดการ เพื่อให้ชีวิตเกิดสมดุล ประกอบด้วย 3 …

Cystic Fibrosis, a life threatening autosomal recessive genetic disease, is characterized by a defective gene resulting in the production of thick mucus that obstructs the lungs and pancreas. CF requires intensive management performed at the home. An initial pilot study was performed to describe knowledge of CF related diabetes (CFRD) in adults with CF. The findings of this study, which demonstrated that adults with CF lacked sufficient knowledge about CFRD confirmed the need to explore additional factors of self-management guided by a theoretical framework. Purpose: To examine students’ perceptions of interprofessional education (IPE) as a result of their participation in a community engagement experience with older adults. Methods: Thirty-six students enrolled in the undergraduate majors of athletic training, exercise science, and nutrition participated in an 8-week fall prevention program in two community dwelling senior centers. A one-group pretest-posttest evaluation design was used to assess the IPE experience of the students. The goal of diabetes education programs (DEPs) is to improve patients' pattern-management habits for those with type 2 diabetes (T2D), though participation in DEPs is lower than expected due to low physician referral rates. This retrospective study examined secondary data of 162 T2D patients who had been referred to a DEP in a community center in RI to determine whether the source of referral affected patient attendance, participation persistence, and outcomes. People living with mental disorders (PLWMDs) are at an increased risk for developing obesity due to poor diet, physical inactivity, and antipsychotic medications. In the United States, the general-population obesity rate is 36% compared to more than 50% for PLWMDs. Mental health professionals (MHPs), focused on addressing psychiatric conditions, seldom recognize and clinically manage obesity. Furthermore, this population is socioeconomically disadvantaged with poor dietary habits while consuming psychiatric medications that stimulate hunger, further exacerbating the risk for obesity. Since 1980, the obesity rate in children 5 to 11 years of age has increased from 7% to 18%. Background
Increasing demand for baccalaureate-prepared nurses has led to rapid growth in the number of baccalaureate-granting programs, and to concerns about educational quality and potential effects on productivity of the graduating nursing workforce. We examined the association of individual productivity of a baccalaureate-prepared nurse with the ranking of the degree-granting institution.
Methods
For a sample of 691 nurses from general medical-surgical units at a large magnet urban hospital between 6/1/2011–12/31/2011, we conducted multivariate regression analysis of nurse productivity on the ranking of the degree-granting institution, adjusted for age, hospital tenure, gender, and unit-specific effects. Background
Cesarean delivery accounts for nearly one-third of all births in the U.S. and contributes to an additional $38 billion in healthcare costs each year. Although Cesarean delivery has a long record of improving maternal and neonatal mortality and morbidity, increased utilization over time has yielded public health concerns and calls for reductions. Observational evidence suggests Cesarean delivery is associated with increased maternal postpartum weight, which may have significant implications for the obesity epidemic. Previous literature, however, typically does not address selection biases stemming from correlations of pre-pregnancy weight and reproductive health with Cesarean delivery.

The purpose of this prospective, descriptive pilot study is to establish a baseline measure for mild cognitive impairment (MCI) in patients aged 65-80 years following elective surgery with general anesthesia (GA). Older adults scheduled for elective surgery routinely undergo GA with a combination of volatile and intravenous anesthetic agents. Current literature shows that patients aged 65-80 who receive GA agents are at increased risk for MCI postoperatively. Data also suggests that further studies are necessary in order to more completely evaluate alternative anesthesia techniques and agents that may prove to reduce the risk and incidence of MCI associated with anesthesia …

Many of the chronic illnesses disproportionately experienced by Native Hawaiians are directly related to poor diets and long-standing obesity beginning in childhood. We report on the findings of in-depth key informant interviews (N = 14) that took place in two Native Hawaiian communities as part of a larger, community-based participatory research study that included a community assessment through individual interviews and focused group discussions, and a pilot intervention targeting pregnant women, their infants, and families. This study explored the perceptions foster parents have about their work as caregivers of foster children who are hospitalized and then discharged from the hospital into a foster home.
