PURPOSE: The purpose of this study was to evaluate the impact of a registered Nurse Navigator (NN) on hospital 30-day readmissions for patients with heart failure at Norton Healthcare (NHC) in Louisville, Kentucky.
METHODS: This study involved two phases. Phase I was a retrospective descriptive design utilizing a medical record review of 159 patient charts. Group 1, 54 charts, included patients with heart failure who were discharged from NHC for heart failure related illnesses and were seen in a primary care clinic that utilized a NN after discharge from the hospital. The improved management of diabetes is a critical clinical and economic challenge facing our healthcare system. Educating patients with diabetes to self-manage their disease is a vital component of diabetes care. The purpose of this DNP practice improvement project was to evaluate an innovative health care delivery method of individualized diabetes self-management education (DSME) in primary care. OBJECTIVE: The purpose of this study is to evaluate the Enhanced Recovery After Surgery (ERAS) pathway utilized at two Norton Healthcare facilities for colorectal and gynecological surgeries. The specific aim is to examine the impact on patient outcomes, clinical effectiveness, and costs.
METHODS: This was a multi-center, pre-post implementation retrospective study of the impact of ERAS pathways on colorectal surgery patients at Norton Audubon Hospital (NAH) and gynecological surgery patients at Norton Women’s and Children's Hospital (NWCH). The sample included 399 patients including patients from both hospitals, pre- and post-ERAS.

PURPOSE: The purpose of this quality improvement project was to assess provider and staff knowledge about health literacy. Specifically, the knowledge of health literacy and perceived importance, current usage, and benefits of the use of a health literacy assessment tool at a primary care clinic was evaluated pre-intervention. The perceived importance of health literacy as well as satisfaction with, frequency of use, and recommendation of the chosen health literacy assessment tool for future use was evaluated post-intervention.

Problem: The CDC estimates one person every 20 minutes every day acquires an HPV-related cancer. Kentucky’s HPV associated cancer burden is among the highest in the nation. Adolescent HPV vaccination rates in Kentucky are far below HealthyPeople 2020 goals. Barriers are multifaceted and include provider, patient and system barriers. The AFIX model is an evidenced based quality improvement program that addresses key provider barriers. Delirium is an acute, temporary cognitive disorder that is characterized by a change in mental status, altered level of judgement, and changes in perception and behavior. Elderly patients who undergo open cardiac surgery are particularly at risk for developing delirium postoperatively. Advancements in screening, early detection, and treatment have increased breast cancer survival. In the United States, the 5-year relative survival rate for breast cancer is 89%. With more women surviving breast cancer, there are now more than 3.5 million breast cancer survivors (BCS), a figure estimated to increase to 4.3 million by 2020. Survivors experience long-term symptoms of their cancer and treatment which may last years and even decades after diagnosis and treatment. Although sexually transmitted infections (STI) are not officially reported via the route of transmission, the CDC (2016) reports that individuals who provide fellatio have the highest risk of oral transmission of a STI. Therefore, females who engage in fellatio are at risk for acquiring a STI. Purpose: To evaluate the effectiveness of a text messaging program (TMP) to improve glucose control, retinopathy screening (RS) rates, and self-care behaviors in patients with uncontrolled type 2 diabetes.
Methods: A single-group design with a quasi-systematic random sample (n=20) received educational/exhortational text messages on their cellular phones for 3 months. Subjects, 12 of whom identified as a minority ethnicity, were mostly male, aged 27-73 years.
Results: Glucose control and RS rates improved significantly. Subjects (>70%) reported changes in self-care behaviors.
Conclusion: Leveraging ubiquitous technology, a TMP for patients with limited access to healthcare education, holds promise

The targeted transplant center's abdominal organ transplant unit had difficulty providing
adequate education to patients prior to discharge, which had resulted in a 24% readmission rate within 30 days due to infections. Patients and caregivers were unavailable to receive education despite multiple attempts, which made it challenging for health care providers to complete this aspect of their job, resulting in a negative impact on patients' long-term outcomes. Ineffectively managed chronic diseases such as diabetes mellitus (DM) increase overall health care expenditures and negatively affects health outcomes such as exacerbations, functional decline, disability, and death. The purpose of this systematic review (SR) was to review the DM outcomes reported by patient-centered medical homes (PCMHs). The goal was to determine how care coordination and evidence-based clinical management impacted financial and health outcomes. Improvements in technology have allowed people with tracheostomies to live at home on mechanical ventilation (HMV). Quality of life (QOL) for HMV users has been studied quantitatively, but few qualitative studies have been published. The purpose of this phenomenological study was to explore QOL for individuals with tracheostomies on prolonged HMV focusing on activities of daily living (ADLs) and the role of decision- making. Background: Despite advances in cancer diagnosis and treatment have significantly improved survival rates, patients post-treatment-related health needs are often not adequately addressed by current health services. Background: Addressing the burden of poor physical health and the subsequent gap in life expectancy experienced by people with mental illness is a major priority in mental health services. To equip mental health staff with the competence to deliver evidence-based interventions, targeted staff training regarding metabolic health is required. Purpose: To explore the post-treatment experiences and preferences for follow-up support of lymphoma survivors.
Methods: Two focus groups were conducted with 17 participants to explore informational, psychological, emotional, social, practical and physical needs, 6-30 months post-treatment for lymphoma. Perceptions regarding a potential model of survivorship care were also elicited.
Results: Thematic content analysis revealed five key themes: Information; Loss and uncertainty; Family, support and post-treatment experience; Transition, connectivity and normalcy, and Person-centred posttreatment care. Participants described a sense of loss as they transitioned away from regular interaction with the hospital at the end of treatment, but also talked about the …

This study aimed to describe the meaning of "self-care" from the perspectives of older persons living with diabetes and explored the self-care behaviors of older persons living with diabetes. The qualitative method was applied in this study. Data were collected by in-depth interview of 16 older persons with Type 2 diabetes in Vietnam. The aim of this study was to test the effect of the Uncertainty Management Program (UMP) on quality of life among Vietnamese women at 3-weeks post mastectomy. The quasi-experimental design with posttest only was conducted in 115 women with post mastectomy who were assigned in experimental group (n=57) and control group (n=58). The experimental group received the UMP and routine care while the control group received only routine care. Participants were assessed at 3-weeks post mastectomy using the modified Quality of Life Index Scale Vietnamese version with Cronbach's alpha coefficient of .81. การวิจัยครั้งนี้เป็นการวิจัยแบบบรรยายเชิงทำนาย มีวัตถุประสงค์เพื่อศึกษาการฟื้นหายของผู้ป่วยหลังผ่าตัดมดลูกผ่านกล้องทางหน้าท้อง และศึกษาปัจจัยทำนายการฟื้นหายของผู้ป่วยหลังผ่าตัดมดลูกผ่านกล้องทางหน้าท้องจากปัจจัยด้านระยะเวลาของอาการปวดท้องน้อยก่อนผ่าตัด ดัชนีมวลกายก่อนผ่าตัด การเคลื่อนไหวร่างกายหลังผ่าตัด ความวิตกกังวลก่อนผ่าตัด และความรู้ในการปฏิบัติตนก่อนผ่าตัด กลุ่มตัวอย่าง คือ ผู้ป่วยที่เข้ารับการผ่าตัดมดลูกผ่านกล้องทางหน้าท้องในระยะ 24-48 ชั่วโมงหลังผ่าตัดในโรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย จำนวน 82 คน และโรงพยาบาลราชวิถี จำนวน 27 คน รวม 109 คน คัดเลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบหลายขั้นตอน เก็บข้อมูลโดยใช้แบบสอบถาม 7 ส่วน ได้แก่ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามระยะเวลาของอาการปวดท้องน้อยก่อนผ่าตัด 3) แบบบันทึกดัชนีมวลกายก่อนผ่าตัด 4) แบบสอบถามการเคลื่อนไหวร่างกายหลังผ่าตัด 5) แบบสอบถามความวิตกกังวลก่อนผ่าตัด 6) แบบสอบถามความรู้ในการปฏิบัติตนก่อนผ่าตัด 7) แบบประเมินการฟื้นหายหลังผ่าตัด ตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน มีดัชนีความตรงเชิงเนื้อหาเท่ากับ 1.00, 1.00, 0.77, 1.00, 1.00 และ 0.77 ตามลำดับ ส่วนที่ 2 มีค่าสัมประสิทธิ์เพียร์สันของการวัดซ้ำเท่ากับ .983 ส่วนที่ 3 มีค่าสัมประสิทธิ์แคปปาเท่ากับ 1.00 ส่วนที่ 4, 5 และ 7 มีค่าสัมประสิทธิ์แอลฟาครอนบาคเท่ากับ .878, .806 และ 802 ตามลำดับ และส่วนที่ 6 มีค่า KR-20 เท่ากับ .67 วิเคราะห์ข้อมูลโดยใช้สถิติวิเคราะห์ถดถอยพหุคูณแบบขั้นตอน ผลการศึกษา พบว่า 1) การฟื้นหายของผู้ป่วยหลังผ่าตัดมดลูกผ่านกล้องทางหน้าท้องอยู่ในระดับปานกลาง (mean=17.367,SD=3.099) 2) ปัจจัยที่ทำนายการฟื้นหายของผู้ป่วยหลังผ่าตัดมดลูกผ่านกล้องทางหน้าท้อง ประกอบด้วย 4 ตัวแปร ได้แก่ ความวิตกกังวลก่อนผ่าตัด (Beta=-.358) ความรู้ในการปฏิบัติตนก่อนผ่าตัด (Beta=.415) การเคลื่อนไหวร่างกายหลังผ่าตัด (Beta=.261) และดัชนีมวลกายก่อนผ่าตัด (Beta=-.257) สามารถร่วมกันทำนายการฟื้นหายของผู้ป่วยหลังผ่าตัดมดลูกผ่านกล้องทางหน้าท้องได้ร้อยละ …

การวิจัยครั้งนี้เป็นการวิจัยเชิงบรรยาย มีวัตถุประสงค์เพื่อศึกษาภาวะเปราะบางของผู้สูงอายุที่อยู่ในบ้านพักคนชราภาครัฐ และศึกษาความสัมพันธ์ระหว่างปัจจัยคัดสรร คือ อายุ เพศ โรคร่วม ภาวะซึมเศร้าและการสนับสนุนทางสังคม กับภาวะเปราะบางในผู้สูงอายุที่อยู่ในบ้านพักคนชราของภาครัฐ เขตกรุงเทพมหานครและปริมณฑล จำนวน 150 คน ด้วยวิธีการเลือกตามสะดวกจากบ้านพักคนชราของภาครัฐในเขตกรุงเทพมหานครและปริมณฑลที่มีทั้งเพศชายและเพศหญิง ทั้งหมด 3 แห่ง เครื่องมือที่ใช้ในงานวิจัย คือ แบบประเมินภาวะสมองเสื่อม แบบประเมินภาวะซึมเศร้า แบบสอบถามการสนับสนุนทางสังคมและแบบประเมินภาวะเปราะบางซึ่งผ่านการตรวจสอบโดยผู้ทรงคุณวุฒิ หาความเที่ยงของแบบประเมินภาวะสมองเสื่อม แบบประเมินภาวะซึมเศร้า แบบสอบถามการสนับสนุนทางสังคมด้วยวิธีสัมประสิทธิ์อัลฟาครอนบาค มีค่าความเที่ยงเท่ากับ .80, .83, .80 ตามลำดับ ส่วนแบบประเมินภาวะเปราะบางหาความเที่ยงด้วยวิธีใช้ผู้ประเมินร่วม (Inter-rater method) มีค่าความเที่ยงเท่ากับ .96 วิเคราะห์ข้อมูลส่วนบุคคล โดยใช้การแจกแจงความถี่ ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน วิเคราะห์ปัจจัยที่สัมพันธ์ระหว่าง อายุ เพศ โรคร่วม ภาวะซึมเศร้า การสนับสนุนทางสังคมกับภาวะเปราะบางในผู้สูงอายุ โดยใช้สถิติ Binary Logistic Regression ผลการวิจัยสรุปได้ดังนี้ 1. ผู้สูงอายุในบ้านพักคนชราของภาครัฐ ในเขตกรุงเทพมหานครและปริมณฑล พบว่า มีภาวะเปราะบาง ร้อยละ 58.7 2. อายุ มีความสัมพันธ์ทางบวกกับภาวะเปราะบางของผู้สูงอายุในบ้านพักคนชราของภาครัฐ ในเขตกรุงเทพมหานครและปริมณฑล อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 โดยอายุที่เพิ่มขึ้น 1 ปี จะมีโอกาสเกิดภาวะเปราะบาง คิดเป็นร้อยละ 10 3. การสนับสนุนทางสังคมมีความสัมพันธ์ทางลบกับภาวะเปราะบางของผู้สูงอายุในบ้านพักคนชราของภาครัฐในเขตกรุงเทพมหานครและปริมณฑล อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 โดยผู้สูงอายุที่ได้รับการสนับสนุนทางสังคมเพิ่มขึ้น 1 หน่วย จะมีโอกาสเกิดภาวะเปราะบางลดลงร้อยละ 5

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาสมรรถนะพยาบาลเวชศาสตร์ความดันบรรยากาศสูง โดยใช้เทคนิคเดลฟาย (Delphi tecnique) ผู้ให้ข้อมูลคือผู้เชี่ยวชาญจำนวน 20 คน ประกอบด้วย กลุ่มแพทย์ที่รักษาด้วยออกซิเจนความกดบรรยากาศสูง (Hyperbaric Oxygen Therapy: HBOT) จำนวน 5 คน กลุ่มผู้บริหารการพยาบาลด้าน HBOT จำนวน 6 คน กลุ่มอาจารย์พยาบาลด้าน HBOT จำนวน 3 คน และกลุ่มพยาบาลที่ปฏิบัติงานด้าน HBOT จำนวน 6 คน วิธีดำเนินการวิจัยประกอบด้วย 3 รอบ คือ รอบที่ 1 การสัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับสมรรถนะพยาบาลเวชศาสตร์ความดันบรรยากาศสูง รอบที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วสร้างเป็นแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าแนวโน้มความสำคัญของสมรรถนะพยาบาลเวชศาสตร์ ความดันบรรยากาศสูง และรอบที่ 3 นำข้อมูลที่ได้จากรอบที่ 2 มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่าง ควอไทล์ จัดทำแบบสอบถามให้ผู้เชี่ยวชาญยืนยันความคิดเห็น เพื่อสรุปเป็นสมรรถนะพยาบาลเวชศาสตร์ความดันบรรยากาศสูง ผลการวิจัยพบว่า สมรรถนะพยาบาลเวชศาสตร์ความดันบรรยากาศสูง ประกอบด้วย 4 ด้าน ดังนี้ 1) ด้านการปฏิบัติการพยาบาลเฉพาะทางเวชศาสตร์ความดันบรรยากาศสูง จำนวน 14 สมรรถนะ 2) ด้านการให้ความรู้ และการสื่อสาร จำนวน 10 สมรรถนะ 3) ด้านการจัดการความเสี่ยง จำนวน 9 สมรรถนะ และ 4) ด้านการพัฒนาคุณภาพ จำนวน 6 สมรรถนะ

การวิจัยครั้งนี้เป็นเชิงบรรยาย มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างเพศ การรับรู้ประโยชน์ของการปฏิบัติพฤติกรรมส่งเสริมสุขภาพ การรับรู้อุปสรรคของการปฏิบัติพฤติกรรมส่่งเสริมสุขภาพ การรับรู้ความสามารถของตนเอง และการสนับสนุนทางสังคม กับพฤติกรรมส่งเสริมสุขภาพของผู้สูงอายุที่พักอาศัยในอาคารสูง กรุงเทพมหานคร ตามแนวคิดของ Pender (2002) กลุ่มตัวอย่าง คือ ผู้สูงอายุในชุมชนอาคารทรัพย์สิน 26-7 จำนวน 117 คน เครื่องมือที่ใช้ในการเก็บข้อมูล คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามพฤติกรรมส่งเสริมสุขภาพ แบบสอบถามการรับรู้ประโยชน์ของการปฏิบัติพฤติกรรมส่งเสริมสุขภาพ แบบสอบถามการรับรู้อุปสรรคของการปฏิบัติพฤติกรรมส่งเสริมสุขภาพ แบบสอบถามการรับรู้ความสามารถของตนเอง และแบบสอบถามการสนับสนุนทางสังคม ได้ผ่านการตรวจความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ และมีค่าความเที่ยงของแบบสอบถามเท่ากับ .88, .88, .80, .85 และ .95 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย สถิติที และค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน ผลการวิจัยสรุปได้ดังนี้ 1. ผู้สูงอายุที่พักอาศัยในอาคารสูง กรุงเทพมหานครมีพฤติกรรมส่งเสริมสุขภาพโดยรวมอยู่ในระดับสูง (Mean=3.05, SD= .32) 2. การรับรู้ประโยชน์ของการปฏิบัติพฤติกรรมส่งเสริมสุขภาพ การรับรู้ความสามารถของตนเอง และการสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกกับพฤติกรรมส่งเสริมสุขภาพของผู้สูงอายุที่พักอาศัยในอาคารสูง กรุงเทพมหานครอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r= .658, .632 และ .627 ตามลำดับ) 3. เพศ และการรับรู้อุปสรรคของการปฏิบัติพฤติกรรมส่งเสริมสุขภาพไม่มีความสัมพันธ์กับพฤติกรรมส่งเสริมสุขภาพของผู้สูงอายุที่พักอาศัยในอาคารสูง กรุงเทพมหานครอย่างมีนัยสำคัญทางสถิติ .05
