Mechanisms contributing to childhood obesity begin during early life, including infant feeding practices and an excessive rate of infant growth. Parental decisions regarding their infant contribute to these mechanisms. Parental self-efficacy (PSE) is the belief parents have in their ability to accomplish the tasks of parenting and is associated with infant outcomes. A high sense of PSE, breastfeeding self-efficacy, or self-efficacy for feeding has been found to be associated with healthy infant feeding practices. However, this research is limited in scope and has not been conducted in populations at greatest risk for childhood obesity such as low-income or African-American infants. …

Background: Objectively tracking sedentary behavior (SB) and physical activity (PA) is becoming increasingly important as research continues to show the negative effects with increasing SB and decreasing PA. Objectives: The purpose of this study was to evaluate three commercial activity trackers with young adults regarding how they accurately measure SB and PA behaviors in a laboratory setting. Methods: 50 college-aged participants wore three wrist-based activity trackers (Fitbit Surge, Apple Watch, and Basis Peak) and two ActiGraph accelerometer devices during a series of SB and PA behaviors for five-minute intervals in a laboratory setting. Managing diabetes in the geriatric long-term care population can be challenging, yet important because diabetes is a chronic, progressive disease. The purpose of this project was to identify clinical practice guidelines for managing Type 2 diabetes in geriatric veterans and to develop a class to educate providers on diabetes management in the geriatric long-term care population at a community living and rehabilitation center. Background We examined the relationship between timing and duration of folic acid (FA) supplementation in achieving red blood cell (RBC) folate levels in early pregnancy which are optimal (>906 nmol/l) for the prevention of neural tube defects (NTDs). Methods Clinical, FA supplementation and dietary folate details were computerized at the first antenatal visit. Maternal blood samples were analysed for RBC and serum folate. Results Of the 502 women, 98.2% (n = 493) reported taking FA. There was a positive correlation between duration of supplementation and both RBC folate (r = 0.43, P < 0.001) and serum folate (rho = 0.29, P < 0.001). The optimal RBC folate level was achieved in 80.4% (n = 46) of women who started FA 400 μg 4–8 weeks before their LMP compared with only 53.6% (n = 153) in women who started 4–8 weeks after their LMP (P < 0.001). African American men are disproportionately affected by type 2 diabetes and have a 3 times higher rate of 3 serious complications of diabetes, including blindness, amputations, and end-stage renal disease, compared to other groups. Purpose: To explore and describe lymphoma survivors’ thoughts and perceptions of the components of a nurse led lymphoma survivorship clinic intervention.
Methods: An exploratory, qualitative descriptive study using interviews from 10 participants who had transitioned post-treatment into the survivorship phase via a nurse-led lymphoma survivorship clinic intervention.
Results: Thematic analysis revealed three major themes: Reassurance and individualised care; Information and support; and Empowerment. Participants described the reassurance they gained from having contact with a health professional post-treatment who individualised information and support. This descriptive correlational study aimed to identify the predicting factors and to examine direct and indirect relationships of preschoolers' parental feeding behaviors (PFB), urban Thailand. The conceptual framework was developed guided by Orem's self-care theory. A Multi-stage sampling was used to recruit 443 parents of preschoolers from 11 schools in Bangkok and city municipalities in all regions of Thailand. Six questionnaires demonstrated acceptable content and construct validity, and reliability were used to ask all participants to complete all questionnaires. การวิจัยนี้มีวัตถุประสงค์ เพื่อศึกษาระยะเวลาการนอนหลับของทารกเกิดก่อนกำหนดภายหลังจำหน่ายจากโรงพยาบาล โดยศึกษาความสัมพันธ์ระหว่างปัจจัยภายใน คือ เพศ อายุครรภ์ ประเภทของนม และปัจจัยภายนอก คือ ความเครียดของมารดา สัมพันธภาพระหว่างมารดา-ทารก รายได้ของครอบครัว และระดับการศึกษาของมารดา กับระยะเวลาการนอนหลับของทารกเกิดก่อนกำหนด กลุ่มตัวอย่าง คือ ทารกเกิดก่อนกำหนดอายุครรภ์ 28-36 สัปดาห์ที่รับไว้ในโรงพยาบาลครั้งแรกและมารดาจำนวน 130 คู่ จากวิธีการสุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือวิจัยประกอบด้วย แบบสอบถามข้อมูลทั่วไปของมารดาและทารก ตารางบันทึกระยะเวลาการนอนหลับ แบบสอบถามความเครียดของมารดา และแบบสอบถามสัมพันธภาพระหว่างมารดา-ทารก ซึ่งผ่านการตรวจสอบความตรงตามเนื้อหา แบบสอบถามมีค่าความเที่ยงเท่ากับ 0.90 และ 0.89 ตามลำดับ วิเคราะห์ข้อมูลด้วยค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน และการทดสอบไคสแควร์ ที่ระดับความมีนัยสำคัญทางสถิติ .05 ผลการวิจัยสำคัญสรุปได้ดังนี้ 1. ระยะเวลาการนอนหลับของทารกเกิดก่อนกำหนดอยู่ในระดับไม่เหมาะสมกับวัย ( x = 880.51 นาที/วัน, SD = 123.59) 2. เพศ ประเภทของนม และระดับการศึกษาของมารดา มีความสัมพันธ์กับระยะเวลาการนอนหลับของทารกเกิดก่อนกำหนด อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 ( 2 = 15.405, 12.438 และ 15.123 ตามลำดับ) 3. อายุครรภ์ และสัมพันธภาพระหว่างมารดา-ทารก มีความสัมพันธ์ทางบวกกับระยะเวลาการนอนหลับของทารกเกิดก่อนกำหนดอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 (r= .491 และ .265 ตามลำดับ) 4. ความเครียดของมารดา มีความสัมพันธ์ทางลบกับระยะเวลาการนอนหลับของทารกอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 (r= -.364) ข้อความรู้จากผลการวิจัยให้ประโยชน์ต่อการปฏิบัติการพยาบาล ในการเตรียมมารดาก่อนการจำหน่ายทารกเกิดก่อนกำหนด พยาบาลสามารถส่งเสริมระยะเวลาการนอนหลับที่เพียงพอของทารกเกิดก่อนกำหนดที่บ้าน โดยการเสริมสร้างสัมพันธภาพระหว่างมารดา-ทารก และให้มารดาเรียนรู้วิธีจัดการความเครียดที่บ้านโดยเฉพาะขณะเลี้ยงลูกด้วยนมแม่

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยทำนายความเหนื่อยล้าของผู้ป่วยหลังการเกิดโรคหลอดเลือดสมองระยะแรก ได้แก่ ระดับฮีโมโกลบิน ภาวะโภชนาการ อาการนอนไม่หลับ และกิจกรรมทางกาย กลุ่มตัวอย่างคือ ผู้ป่วยหลังเกิดโรคหลอดเลือดสมองชนิดแตกและตีบตันครั้งแรก อายุระหว่าง 18-59 ปี มีระยะเวลาการเจ็บป่วยตั้งแต่ 3 เดือนจนถึง 1 ปี เพศชายและเพศหญิงที่มาเข้ารับบริการในแผนกผู้ป่วยนอกอายุรกรรมประสาทและศัลยกรรมประสาท โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทยและโรงพยาบาลตำรวจ จำนวน 100 ราย ที่ได้มาจากการคัดเลือกแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลประกอบด้วย แบบสอบถามข้อมูลทั่วไปและข้อมูลการเจ็บป่วย ภาวะโภชนาการ อาการนอนไม่หลับ กิจกรรรมทางกาย และความเหนื่อยล้า ซึ่งได้รับการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิและตรวจสอบความเที่ยงได้ค่าสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ .86, .80, .86 และ .78 ตามลำดับ สถิติที่ใช้วิเคราะห์ข้อมูล ได้แก่ สถิติเชิงบรรยาย ค่าสหสัมพันธ์พอยท์ไบซีเรียล ค่าสัมประสิทธิ์ของเพียร์สัน และการถดถอยพหุคูณแบบขั้นตอน ผลการวิจัยสรุปได้ดังนี้ 1. ร้อยละ 63 ของกลุ่มตัวอย่างผู้ป่วยหลังการเกิดโรคหลอดเลือดสมองระยะแรกมีความเหนื่อยล้า (Mean = 4.42, S.D. = 1.63) 2. ระดับฮีโมโกลบินและอาการนอนไม่หลับสามารถร่วมกันทำนายความเหนื่อยล้าของผู้ป่วยหลังการเกิดโรคหลอดเลือดสมองระยะแรกได้อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 โดยมีอำนาจพยากรณ์ได้ร้อยละ 30 และสามารถสร้างสมการพยากรณ์ในรูปคะแนนมาตรฐานได้ดังนี้ (Zความเหนื่อยล้า ) = -.42Z ระดับฮีโมโกลบิน + .21Z อาการนอนไม่หลับ

การวิจัยนี้เป็นงานวิจัยแบบกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมการดูแลตนเองภายหลังการผ่าตัดลดน้ำหนักของผู้ป่วยโรคอ้วนภายหลังได้รับโปรแกรมส่งเสริมการดูแลตนเอง และเปรียบเทียบพฤติกรรมการดูแลตนเองภายหลังการผ่าตัดลดน้ำหนักของผู้ป่วยโรคอ้วน ระหว่างกลุ่มที่ได้รับโปรแกรมส่งเสริมการดูแลตนเองกับกลุ่มที่ได้รับการพยาบาลตามปกติ กลุ่มตัวอย่างคือ ผู้ป่วยโรคอ้วนทั้งเพศชายและหญิง อายุ 18 – 59 ปี ภายหลังได้รับการผ่าตัดลดน้ำหนักที่โรงพยาบาลจุฬาลงกรณ์ จำนวน 30 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 15 คน จับคู่ด้วยเพศ อายุ และชนิดการผ่าตัด กลุ่มควบคุมได้รับการพยาบาลตามปกติ กลุ่มทดลองได้รับโปรแกรมส่งเสริมการดูแลตนเอง ที่ผู้วิจัยสร้างขึ้นตามกรอบแนวคิดทฤษฎีการดูแลตนเองของ Orem ประกอบด้วย 2 ระยะ คือ ระยะการให้ความรู้และการสนับสนุนภายหลังการผ่าตัดขณะอยู่โรงพยาบาล และระยะการให้การสนับสนุนเมื่อกลับบ้าน เก็บรวบรวมข้อมูล 8 สัปดาห์ ด้วยแบบวัดพฤติกรรมการดูแลตนเองภายหลังการผ่าตัดลดน้ำหนัก มีค่าสัมประสิทธิ์แอลฟ่าครอนบาคเท่ากับ .81 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและสถิติทดสอบที ผลการวิจัยสรุปได้ดังนี้ 1. ผู้ป่วยโรคอ้วนภายหลังได้รับการผ่าตัดลดน้ำหนักกลุ่มที่ได้รับโปรแกรมส่งเสริมการดูแลตนเอง มีพฤติกรรมการดูแลตนเองอยู่ในระดับดี (Mean = 4.13) 2. ผู้ป่วยโรคอ้วนภายหลังได้รับการผ่าตัดลดน้ำหนัก กลุ่มที่ได้รับโปรแกรมส่งเสริมการดูแลตนเองมีพฤติกรรมการดูแลตนเอง (Mean = 4.13 ± SD = 0.25) ดีกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ (Mean = 3.68 ± SD = 0.31) อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยเชิงคุณภาพเพื่อบรรยายประสบการณ์ของผู้ดูแลในครอบครัวในการดูแลทารกโรคปอดเรื้อรัง (Chronic Lung Disease: CLD) ที่บ้าน ผู้ให้ข้อมูลคือผู้ดูแลในครอบครัวที่ทำหน้าที่หลักในการดูแลทารก CLD หลังจำหน่ายออกจากโรงพยาบาลไม่เกิน 2 ปี เก็บข้อมูลโดยการสัมภาษณ์แบบเจาะลึกร่วมกับการบันทึกเทป ข้อมูลอิ่มตัวจากผู้ให้ข้อมูลจำนวน 14 ราย นำข้อมูลที่ได้มาถอดความแบบคำต่อคำ วิเคราะห์ข้อมูลตามวิธีของ Colaizzi ผลการวิจัยพบว่าประสบการณ์ของผู้ดูแลในครอบครัวในการดูแลทารกโรคปอดเรื้อรังที่บ้าน ประกอบด้วย 4 ประเด็นหลัก ดังนี้ 1) ยากลำบากในการดูแล มียาหลายตัว ต้องให้ตรงเวลา ต้องสังเกตการหายใจอย่างใกล้ชิด หาทางช่วยเหลือเมื่อท้องผูก คิดหาวิธีป้อนนม ลดการแหวะและอาเจียน พยายามทำทุกทางให้ลูก/หลาน น้ำหนักขึ้น ทำอะไรไม่ถูกเมื่อมีอาการเปลี่ยนแปลง และมีค่าใช้จ่ายสูงในการดูแล 2) เหน็ดเหนื่อยเมื่อต้องดูแลลูก/หลานที่ไม่สบาย 3) หลากหลายความรู้สึกที่เกิดขึ้น ทั้งความรู้สึกที่เป็นทุกข์และความรู้สึกที่เป็นสุข และ 4) กำลังใจและความช่วยเหลือจากคนรอบข้าง ที่มาจากการสร้างกำลังใจให้ตนเอง และการได้รับกำลังใจและความช่วยเหลือจากคนรอบข้าง การวิจัยให้ข้อมูลเชิงลึกถึงการดูแลทารกแรกเกิด CLD ที่บ้าน ซึ่งมีความซับซ้อนและมีผลกระทบต่ออารมณ์และการเผชิญความเครียดของผู้ดูแล แต่ผู้ดูแลยังคงทำหน้าอย่างต่อเนื่องได้ด้วยกำลังใจที่ตนเองสร้างขึ้นรวมไปถึงกำลังใจและความช่วยเหลือจากคนรอบข้างที่ร่วมในปรากฏการณ์ของการดูแล การนำข้อมูลมาใช้วางแผนจำหน่ายจะช่วยเตรียมความพร้อมในการทำหน้าของผู้ดูแลให้สามารถรับมือกับปัญหาสุขภาพของทารกที่บ้านและลดการกลับเข้าโรงพยาบาลซ้ำ

Objective: To initiate a parenting support program for parents of children with Autism Spectrum Disorder (ASD) in Richland and Lexington Counties. Background: Parents of children with ASD have increased stress and a poorer sense of well-being relative to parents of children with typical development. The purpose of this quality improvement project is to compare the use of ultrasound to guide placement of peripheral intravenous (USGPIV) catheters versus standard techniques in difficult access patients, as measured by the number of attempts required to obtain venous access and total cost related to means of obtaining peripheral venous access between a nurse driven USGPIV and VAT team consults or physician assistance. Traumatic brain injury (TBI) is a leading cause of death and permanent disability worldwide. The American College of Surgeon’s Trauma Quality Improvement Program (TQIP) has developed a set of recommendations for the management of trauma-related injuries, including TBI. The objective of this evidenced-based practice project was to implement provider- and workflow-based strategies to improve adherence to TQIP recommended guidelines for the placement of intracranial pressure (ICP) monitors. The primary outcome measured was number of ICP monitors placed post-intervention.

Nurses play a central role in preparing patients for discharge. Diabetes affects one-third of all hospitalized patients, with readmission rates 20% higher for patients with diabetes. Low health literacy affects patients' ability to understand education provided during a hospitalization, especially in diabetic patients who are required to perform complex self-care activities.
