Infants born prematurely are at a greater risk for developing both cognitive and motor development delays related to continuing their development outside the normal uterine environment. Because of the potential adverse effects of the Neonatal Intensive Care Unit (NICU) environment on the quality of life of premature infants and their families, researchers have investigated, developed, and tested different developmentally supportive interventions to improve outcomes and decrease the negative effects of the NICU. The majority of successful interventions are based on the Synactive Theory of Individualized Developmental Care (IDC)-a form of patient/family- centered care for the neonate.

Aims and Objectives

To conduct a formative evaluation of the iPad‐Enhanced Shared Care Intervention for Partners (iSCIP) among persons with heart failure (HF), family caregivers and clinicians. Together, persons with HF and family caregivers are referred to as partners.

Background

There is growing awareness of the caregiver's contributions to HF self‐management, social support and reciprocal benefits of interventions that involve both partners. The iSCIP engages both partners in a six‐session psychosocial intervention to address three preventable causes of poor outcomes in a HF population: poor self‐management skills, inadequate social support and underutilisation of palliative care. This dissertation is comprised of three manuscripts for publication reporting on adults living with cystic fibrosis, self-management behaviors and health outcomes. Chapter I explores what is known about adults living with cystic fibrosis (CF) and the factors impacting their self-management, including the complexities of a health maintenance regime required for survival while integrating and balancing the realities of adulthood. Objective: This individual patient data (IPD) meta-analysis aimed to evaluate the effects of psychosocial interventions (PSI) on quality of life (QoL), emotional function (EF), and social function (SF) in patients with cancer, and to study moderator effects of demographic, clinical, personal, and intervention-related characteristics. Methods: Relevant studies were identified via literature searches in 4 databases. We pooled IPD from 22 (n = 4217) of 61 eligible randomized controlled trials. Linear mixed-effect model analyses were used to study intervention effects on the post-intervention values of QoL, EF, and SF (z-scores), adjusting for baseline values, age, and cancer type. Objective: To determine the effectiveness of achieving pregnancy with focused intercourse in the fertile window identified using natural fertility indicators. Methods: 24-cycle prospective effectiveness study. Setting: A North American web-based fertility monitoring service. Participants: 256 North American women aged 20–43 (mean age 29.2 years) seeking to achieve pregnancy. Intervention: Participants identified their fertile window with either electronic hormonal fertility monitoring or cervical mucus monitoring, or both, and recorded their observations on an online fertility tracking system. Main outcome measures: Pregnancies were validated by nurses with an online self-assessed pregnancy evaluation form. Survival analysis was used to determine pregnancy rates. Results: …

For the past decade, there have been many changes to school nursing with the implementation of unlicensed assistant personnel now known as paraprofessionals. This process has brought about several concerns such as mode of delegation, education, training, ability to monitor health-care needs, and organizational effects that trended downward without direction. This qualitative inquiry gave an interpretative meaning to the process of delegation in school nursing, the factors that hinder the delegation process as well as components that are needed to effectively delegate and utilize support staff (paraprofessionals) without detrimental effects to students. Service-learning experiences are often utilized by nursing programs in efforts to increase the cultural competence of nursing students. Through the use of sharing story, the concepts of cultural competence and cultural humility can be explained for students preparing for upcoming intercultural experiences. This case study describes the experience of nursing students and university faculty on their first service-learning trip to rural Kenya and how the intercultural issues were navigated there as students developed characteristics of cultural humility. The focus of this project was to explore child-care teachers’ knowledge of fun physical activities and healthy food choice activities for preschool children. A four-week educational session was implemented in a rural child-care center. The intervention provided education in relationship to the implementation of fun physical activities and healthy food choice activities for child-care teachers during preschoolers structured playtime. The Child Care Teachers’ Knowledge Evaluation (CCTKE) tool developed by the doctoral student was used to collect responses from participants before and after the intervention. Background: With implementation of healthy weight education programs throughout schools, questions have been raised whether a preoccupation with obesity prevention contributes to an increase in the incidence of eating disorders. The purpose of this critical review of research is to compare obesity prevention programs and eating disorder prevention programs and possible unintended negative effects on adolescents with weight-related conditions. This DNP project examined the effect of unit case reports and individualized nurse report cards on nurses’ awareness of and attitudes toward central line-associated bloodstream infection (CLABSI) contributing factors. This project also sought to determine whether CLABSI incidences differed across units and if that was affected by nurses’ awareness of and attitudes toward CLABSI contributing factors. A convenience sample of registered nurses (RNs) across four medical units was included in this project. Of the 79 RNs who participated in providing feedback on this project, 48 RNs completed pre-implementation surveys, resulting in a response rate of 61%, and 53 completed post-implementation …

Breastfeeding is a global health initiative, as defined by the World Health Organization (WHO), because of its numerous health benefits to mothers and infants. Despite breast milk’s documented benefits to mothers and babies, many women are not choosing to breastfeed their infants. A lack of paid maternity leave policy within the United States influences postpartum responsibilities, physically, and psychologically. Background

Community health worker (CHW) interventions are a successful strategy to promote health among HIV-negative and persons living with HIV (PLWH). Psychosocial factors are critical dimensions of HIV/AIDS care contributing to prognosis of the disease, yet it is unclear how CHW interventions improve psychosocial outcomes in PLWH. The purpose of this study was to critically appraise the types, scope, and nature of CHW interventions designed to address psychosocial outcomes in PLWH.

Methods

We performed database searches—PubMed, EMBASE, CINAHL, and Cochrane—to identify randomized controlled trials published in English before April 2017. Fourteen articles met the eligibility criteria.

Falls among the elderly are a significant problem with severe consequences. Residents of long-term facilities account for twenty percent of fatal falls. It is imperative that evidence-based practices be implemented at this level of care to address this problem. A multifaceted evidence-based fall prevention program was implemented at a 70 bed, skilled nursing, long-term care facility located in Florida, whose fall rate of 7.14 is above than the national average (2.74-5.48). Approximately one million people in North America are living with an ostomy, and an additional 100,000 will undergo ostomy surgery annually. Persons living with ostomies, often referred to as ostomates, require specialized care and management to sustain physical health and quality of life. Fecal diversion ostomates, when compared to the general population, are more likely to experience psychological disturbance and higher levels of depression. These ostomates have reported increased loneliness, lowered self-esteem, decreased or absent libido, irritability, suicidal ideation, poor self-image, and symptoms of generalized anxiety disorder and social phobia. Problem Description: Patients of one internal medicine clinic were found to have nearly twice the rate of diagnosed diabetes and poor glycemic control, when compared with national rates. Given this, certain aspects of the patient-provider dyad system, such as inadequate provider time, knowledge, and resources; may have contributed to the ability of some patients to adapt to a lifestyle with consistent diabetes self-care.

Short bowel syndrome is the congenital or surgical alteration of the small bowel resulting in a shorter than normal length of small bowel. This condition affects approximately 20,000 people in the Unites States and is diagnosed in 3-5 times per 100,000 live births annually. Patients with short bowel syndrome are at risk for the development of small bowel bacterial overgrowth, a serious consequence caused by overgrowth of bacteria in the small bowel resulting in symptoms of abdominal pain, abdominal distention, feeding intolerance, malabsorption, weight loss and increased risk of metabolic acidosis.
