การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาหาความสัมพันธ์ระหว่างการสร้างพลังอำนาจ ในการปฏิบัติการพยาบาล และเอกสิทธิ์ทางการพยาบาลตามการรับรู้ของพยาบาลวิชาชีพ โรงพยาบาลศูนย์ สังกัดกระทรวงสาธารณสุข กลุ่มตัวอย่างคือ พยาบาลวิชาชีพจากโรงพยาบาลศูนย์ 9 แห่ง จำนวน 352 คน เลือกตัวอย่าง ด้วยวิธีสุ่มแบบหลายขั้นตอน เครื่องมือที่ใช้ในการวิจัย เป็นแบบสอบถามวัดการสร้างพลังอํานาจในการปฏิบัติการพยาบาล และเอกสิทธิ์ทางการพยาบาล ตามลำดับ ซึ่งผู้วิจัยสร้างขึ้นเอง ตรวจสอบความตรงโดยผู้ทรงคุณวุฒิ 8 คน ค่าความเที่ยง ของเครื่องมือเท่ากับ 92, 91 ผู้วิจัยวิเคราะห์ข้อมูลโดยใช้โปรแกรมสําเร็จรูป SPSS-PC \nผลการวิจัยพบว่า 1. ค่าเฉลี่ยของการสร้างพลังอำนาจในการปฏิบัติการพยาบาล ตามการรับรู้ ของพยาบาลวิชาชีพโดยรวมและรายด้าน คือความมีอิสระ การมีสัมพันธภาพที่มีความไว้วางใจ และการได้รับการสนับสนุนอยู่ในระดับสูง 2. ค่าเฉลี่ยของเอกสิทธิ์ทางการพยาบาลตามการรับรู้ ของพยาบาลวิชาชีพ อยู่ในระดับสูง 3. อายุ และประสบการณ์ทำงานของพยาบาลวิชาชีพ มีความสัมพันธ์ทางบวกกับเอกสิทธิ์ทางการพยาบาลตามการรับรู้ของพยาบาลวิชาชีพ อย่างมีนัยสำคัญ ทางสถิติที่ระดับ .01 4. แผนกที่ปฏิบัติงานของพยาบาลวิชาชีพ ไม่มีความสัมพันธ์ทางบวกกับพยาบาล ตามการรับรู้ของพยาบาลวิชาชีพ 5. การสร้งพลังอำนาจในการปฏิบัติการพยาบาลรายด้าน คือ การได้รับความเป็นอิสระ การมีสัมพันธภาพที่มีความไว้วางใจ และการได้รับการสนับสนุนโดยรวม และรายด้าน มีความสัมพันธ์ทางบวกกับเอกสิทธิ์ทางการพยาบาลตามการรับรู้ของพยาบาลวิชาชีพ

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาระดับความพึงพอใจในงานของพยาบาลวิชาชีพ โรงพยาบาลทั่วไป โรงพยาบาลชุมชน สังกัดกระทรวงสาธารณสุข ภาคเหนือ 2) หาความสัมพันธ์ ระหว่างการใช้อำนาจการบริหารของหัวหน้าหอผู้ป่วย อํานาจการประกอบวิชาชีพกับความพึงพอใจ ในงานของพยาบาลวิชาชีพและ 3) ศึกษาตัวแปรที่สามารถร่วมกันทํานายความพึงพอใจในงาน ของพยาบาลวิชาชีพ ในโรงพยาบาลทั่วไป และโรงพยาบลชุมชน กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพ จำนวน 700 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามการใช้อำนวยการบริหาร ของหัวหน้า หอผู้ป่วย และการใช้อํานาจการประกอบวิชาชีพของพยาบาล ที่สร้างขึ้นโดยผู้วิจัย และแบบวัดความพึงพอใจในงาน \nผลการวิจัยที่สำคัญ สรุปได้ดังนี้ \n1. ความพึงพอใจในงานของพยาบาลวิชาชีพ อยู่ในระดับปานกลาง ความพึงพอใจในงานของ พยาบาลวิชาชีพโรงพยาบาลทั่วไป สูงกว่าของพยาบาลโรงพยาบาลชุมชน อย่างมีนัยสำคัญทาง สถิติที่ระดับ .05 \n2. กลุ่มพยากรณ์ที่สามารถร่วมกันพยากรณ์ความพึงพอใจในงานของพยาบาลวิชาชีพ โรงพยาบาลทั่วไป อย่างมีนัยสำคัญทางสถิติระดับ .05 คือ การใช้อำนาจการบริหารของหัวหน้า หอผู้ป่วยด้านอำนาจการให้รางวัล อำนาจความเชี่ยวชาญ อำนาจอ้างอิง อำนาจโดยชอบธรรม อำนาจการบังคับ และอำนาจในการประกอบวิชาชีพด้านอำนาจการปกป้องช่วยเหลือ อำนาจ การบูรณาการ อำนาจการมีส่วนร่วม อำนาจการเปลี่ยนแปลง การแก้ไขปัญหา อายุและประสบการณ์ การทํางาน มีสัมประสิทธิ์การพยากรณ์ร้อยละ 73.33 (R2 = 7333) \n3. กลุ่มพยากรณ์ที่สามารถร่วมกันพยากรณ์ความพึงพอใจในงานของพยาบาลวิชาชีพชุมชน อย่างมีนัยสําคัญทางสถิติระดับ .05 คือ การใช้อำนาจการบริหารของหัวหน้าหอผู้ป่วยด้านอำนาจ การให้รางวัล อำนาจโดยชอบธรรม อำนาจความเชี่ยวชาญ อำนาจอ้างอิง อํานาจโดยชอบธรรม อำนาจการบังคับ และอำนาจในการประกอบวิชาชีพด้านอำนาจการบูรณาการ อำนาจการเปลี่ยนแปลง อำนาจการสร้างภาวะที่ดีขึ้น การแก้ไขปัญหา และอายุ มีสัมประสิทธิ์การพยากรณ์ร้อยละ 69.49 (R2 =.6949)

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อวิเคราะห์ระบบการประเมินผล การปฏิบัติงานของพยาบาลประจำการ โรงพยาบาลของรัฐ เขตกรุงเทพมหานคร ในส่วนที่เกี่ยวข้องกับส่วนนำเข้าของระบบ กระบวนการประเมินผลการปฏิบัติงาน และผลลัพธ์ของระบบกลุ่มตัวอย่างคือ แบบฟอร์มการ - ประเมินผลการปฏิบัติงานของพยาบาลประจําการทุกชุดที่ใช้อยู่ในโรงพยาบาล เขตกรุงเทพมหานคร และหัวหน้าหอผู้ป่วยในโรงพยาบาลของรัฐ เขตกรุงเทพมหานคร 8 แห่ง จํานวน 201 คน คัดเลือกโดยการสุ่มแบบหลายขั้นตอน เครื่องมือที่ใช้คือ แบบวิเคราะห์ แบบฟอร์มการประเมินผล และแบบสอบถามการประเมินผลการปฏิบัติงานของพยาบาลประจําการ ซึ่งผู้วิจัยสร้างขึ้น และตรวจสอบหาความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 10 ท่าน และได้ค่าความเที่ยงของแบบสอบถาม 1 เท่ากับ 87 วิเคราะห์ด้วยค่าร้อยละ และการทดสอบไคว์-สแคว์ \nผลการวิจัยพบว่า \n1. โรงพยาบาลของรัฐเขตกรุงเทพมหานครทุกสังกัดมีการประเมินผลการปฏิบัติงาน อย่างเป็นระบบ ผู้ประเมินผลการปฏิบัติงานของพยาบาลประจำการส่วนใหญ่คือ หัวหน้าผู้ป่วย (97.5) แบบฟอร์มการประเมินผลที่ใช้ส่วนใหญ่คือ แบบคุณลักษณะและประสิทธิผล (50%) รองลงมา คือแบบพฤติกรรม (38.5%) วิธีการประเมินผลที่ปรากฏในแบบฟอร์มการประเมินคือ การพิจารณา 2. ตามเครื่องมือวัด (75%) ที่เหลือเป็นการให้คะแนนดิบ แบบฟอร์มการประเมินผลมีส่วนประกอบ \nของแบบฟอร์มไม่ครบทั้ง 5 ส่วน โดยมีส่วนของสรุปผลการประเมินมากที่สุด (75%) \n2. กระบวนการการประเมินผลการปฏิบัติงานพบว่า มีกระบวนการการประเมินผล การปฏิบัติงานครบทั้ง 5 องค์ประกอบ มีวัตถุประสงค์ของการประเมินส่วนใหญ่ เพื่อเป็นข้อมูล ป้อนกลับให้พยาบาลประจําการ (81.1) หัวหน้าหอผู้ป่วยส่วนใหญ่มีวิธีการรวบรวมข้อมูลการ ปฏิบัติงานของพยาบาลประจำการคือ สังเกตการทำงานแล้วจดบันทึก (88%) ส่วนใหญ่หัวหน้าหอผู้ป่วยจะแจ้งและอภิปรายผลการประเมินกับพยาบาลประจำการเป็นรายบุคคล (79.7%) การนำผลการประเมินไปใช้ส่วนใหญ่คือ เป็นหลักฐานในการเปลี่ยนแปลงพฤติกรรม พยาบาลประจําการ (77.6%) รองลงมาคือ เพื่อเป็นข้อมูลประกอบการเลื่อนตำแหน่ง (66.1) \n3. ผลการเปรียบเทียบกระบวนการประเมินผลการปฏิบัติงาน ในด้านการรวบรวมข้อมูล การปฏิบัติงาน และด้านการแจ้งและอภิปรายผลการประเมินในโรงพยาบาลของรัฐ เขตกรุงเทพ มหานคร จำแนกตามสังกัดพบว่า มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

The purpose of this experimental study was to investigate whether a prenatal teaching intervention on position and attachment of the baby on the breast had any effect on postpartum nipple pain and trauma, and breastfeeding rates at six weeks. The study explores literature pertaining to the emergence of the CMH movement and the deinstitutionalisation of the mentally ill. A conceptual framework was developed by adapting Lewin's change theory. CMH nurses practising in WA (n= 130), were invited to participate in the study and were asked to complete a survey questionnaire. This resulted in a 66% response rate (n = 86).
