Background: Older adults with Alzheimer’s Disease and Related Dementias (ADRD) experience a higher risk of falls. However, research lacks evidence on the effect of fall exposure on cognitive impairment and dementia. We investigated the association of falls and depressive symptoms with cognitive impairment and dementia in a nationally representative sample. We experienced a 45-year-old male with ADPKD who developed spontaneous retroperitoneal hemorrhages with hypertension in the right subcapsular renal space without trauma. After his condition worsened with conservative management, he was treated by renal transarterial embolization (TAE). Over the next eight years, he experienced bilateral bleeding at three different sites and underwent repeated endovascular treatment with renal TAE to stop the bleeding. After the fourth occurrence, the patient underwent hemodialysis. Although renal cysts and urinary tract bleeding occur frequently in ADPKD, retroperitoneal hemorrhage is rare; however, TAE is both effective and a good way to preserve renal function.

Macrophage-mediated phagocytosis of tumor cells elicits potent antitumor immunity. Nonetheless, sole-blockade of the anti-phagocytosis molecule CD47 has yielded insufficient therapeutic outcomes. Here, we report that glioblastoma (GBM) cells expressed abundant levels of phagocytosis checkpoint CD24. We further show that dual blockade of CD24 and CD47 synergistically enhances the pro-phagocytic activity of macrophages, thereby improving tumor antigen cross-presentation and activating the cyclic GMP-AMP synthase-stimulator of interferon genes (cGAS-STING) pathway. This innate immune activation facilitates T cell infiltration into tumors and sensitizes tumors to anti-PD1 therapy, improving survival outcomes in murine GBM models, including immunosuppressive tumors reflecting human GBM-like features. INTRODUCTION: Peripherally inserted central catheters (PICCs) are commonly used in clinical practice. Historical PICC placement methods (e.g. fluoroscopy-guided placement or blind insertion with confirmatory chest X-ray) have many limitations and are associated with longer wait times, treatment delays, and potential risks to patient safety. Background: Prostate cancer (PC) is the most common cancer in men in the United States with a 5-year relative survival rate for people with distant metastases of 36%. We conducted a single institution, retrospective cohort study of patients with metastatic PC to investigate whether certain gene mutations can be used as predictive biomarkers.
Methods: 200 patients with metastatic hormone sensitive (mHSPC) and castration resistant (mCRPC) prostate cancer who had a FoundationOne report and were treated from June 2007 to October 2024 were included in the study. The study of aging in past populations lies at the intersection of bioarchaeology and paleodemography, offering insight into both biological processes and lived social experience. Despite this potential, older adults are often rendered “invisible” in skeletal assemblages due to methodological and cultural biases. This thesis addresses this gap through a rare, community-based study of aging among the Székely population of Transylvania. By integrating osteological analysis of individuals (11th–19th centuries) from the archaeological Papdomb church cemetery with demographic data derived from the modern cemetery headstones (ca.1850–present), this research examines patterns of aging, longevity, and elder visibility across a culturally and spatially …

OBJECTIVE: Iteratively develop educational tools (instructional video and flyer) to improve midstream clean catch (MSCC) urine sample collection using patient and healthcare professionals' input.
DESIGN: Multi-method qualitative study.
SETTING: Outpatient clinics in Houston, Texas, United States.
PARTICIPANTS: Adult patients recruited from public and private clinics (
METHODS: Twelve patient interviews and three focus groups with HCPs (May 2024-November 2024). Interviews discussed patient experiences using the educational tools to guide urine specimen collection. Focus groups elicited HCPs' perspectives on the comprehensibility and utility of the tools in their respective clinics. We identified themes using directed content analysis.

OBJECTIVE: Lack of ergonomic practices in the operating room results in significant musculoskeletal strain. This study obtains a current assessment of self-reported musculoskeletal strain associated with endoscopic endonasal and open skull base surgery as reported by members of the North American Skull Base Society (NASBS).
STUDY DESIGN: Online Survey.
SETTING: Online.
METHODS: A survey assessing current symptoms in endoscopic skull base surgery was distributed to members of the NASBS.
RESULTS: Sixty-six responses were completed and analyzed. 95% of respondents indicated they had musculoskeletal strain at the neck and C-spine. There was no significant difference in average operating time per week …

Gastric cancer (GC) ranks as the fifth most common cancer type, contributing significantly to cancer-related mortality. This paper presents three cases of patients treated with preoperative radiochemotherapy (preopRTCT) at the National Cancer Center who experienced recurrences of GC after 15, 11 and 19 years. In all cases follow-up ended years before recurrence. Relapse was diagnosed late and treatment outcome was poor. We believe that extending the follow-up period for gastric cancer patients beyond the initial treatment is necessary especially for younger patients with large tumors. Late recurrences, often diagnosed at advanced stages, require differentiation from second primary malignancies, a process …

This article presents a camouflage orthodontic treatment utilizing the improved superelastic Ti-Ni alloy wire (ISW) to address a case of skeletal Class II malocclusion with two missing mandibular incisors. A 28-year-old female patient with a chief complaint of poor dental alignment sought orthodontic evaluation. Clinical examination revealed a Class III molar relationship, mild crowding, and lingually positioning of tooth 35. Radiographic analysis confirmed a skeletal Class II pattern. The treatment plan included the application of an ISW curve over the maxillary arch to control anterior tooth inclination. This case report presents the orthodontic management of a 23-year-old male with severe hypodontia, specifically the congenital absence of all premolars and bilateral peg laterals. The patient sought treatment due to dissatisfaction with previous orthodontic outcomes, characterized by noticeable spacing in the upper anterior region. Clinical evaluation revealed a skeletal Class II relationship and several dental anomalies, prompting the need for an interdisciplinary approach.

Background: Management of severe malocclusion in patients with chronic periodontitis poses significant challenges due to compromised periodontal support and alveolar bone loss. Periodontally accelerated osteogenic orthodontics (PAOO) may facilitate tooth movement while promoting bone regeneration in such cases.
Case presentation: A 33-year-old male with chronic periodontitis and severe malocclusion presented for orthodontic correction. Initial treatment included comprehensive periodontal therapy—oral hygiene control, open-flap debridement, and bone grafting—prior to orthodontic intervention. Facial asymmetry accounts for approximately two-thirds of orthognathic surgical patients and may involve discrepancies in any of the three spatial planes. Conventional bilateral sagittal split osteotomy (BSSO) has limitations in correcting patients with significant discrepancies in bilateral ramus angles. This paper proposes a novel surgical approach for the correction of facial asymmetry using BSSO combined with an anterior midline mandibular split osteotomy and genioplasty.

Purpose: This article introduces a digital workflow for creating a miniscrew-assisted rapid palatal expander (MARPE) guide, to virtually determine the proper MARPE position and to precisely transfer it into the reality. The most common extraction pattern in orthodontic patients with crowding of the teeth is the four first bicuspids. This is usually done in cases with severe crowding or bimaxillary protrusion. In some instances, orthodontists have decided, instead of extracting the first bicuspids, to extract the second bicuspids, for various reasons that we shall discuss in this review. Several other extraction patterns have also been implemented in the past, including second molars, lower incisors, and even asymmetric extractions. Socioeconomic deprivation and environmental heat exposure each increase cardiovascular risk, yet evidence is limited on how these stressors co-occur and jointly shape disease burden within cities. Mapping their overlap can inform equity-oriented planning and needs-based allocation of health and social protection resources.
Methods:
We conducted an ecological geospatial analysis of 2,513 census tracts in Los Angeles County. Introduction: Advanced-stage high-grade serous ovarian cancer (HGSC) is a disease that is difficult to manage due to its heterogeneous clinical behavior. No reliable prediction of response to chemotherapy is currently available and the overall survival rate remains poor. Herein, we sought to determine the molecular mechanisms by which microRNAs (miRNAs) confer chemoresistance in ovarian cancer and demonstrate the efficacy of targeting miRNAs to sensitize HGSC to cisplatin treatment.

Study Design
Single-center retrospective review.
Objective
To investigate the association between BMI Z-scores and postoperative complications in pediatric patients with neuromuscular scoliosis undergoing spinal instrumentation.
Methods
Pediatric patients who underwent spinal instrumentation for neuromuscular scoliosis from July 2012 to June 2016 with a minimum 2-year follow-up were included. BMI and BMI Z-scores were calculated, and patients were stratified into CDC-defined BMI-for-age percentiles: underweight (< 5th percentile), normal (5th to < 85th percentile), overweight (85th to < 95th percentile), and obese (≥95th percentile). Logistic regression assessed associations of BMI, BMI Z-score, and BMI groups with postoperative complications.
Results
In total, 147 patients were included. Average follow-up was 3.2 ± 1.4 years. The most common diagnosis was cerebral palsy (39.5%). Complications occurred in 65 (44.2%) patients: 32 (21.8%) had a surgical site infection (SSI), …

Extreme heat is an ongoing hazard for outdoor sports. In response, schools, recreation departments, and fitness clubs are increasingly installing shade pavilions over venues like football fields and tennis courts. While shading is known to improve thermal comfort and reduce heat stress, its effects on wet bulb globe temperature (WBGT) and related activity modification guidelines in sports remain underexplored. This study uses a modeling approach to quantify the impact of shade pavilions on WBGT and activity modification across a range of U.S. climates.

UNLABELLED: Type II odontoid fractures are the most common cervical injury in geriatric populations, yet optimal management remains debated. This study compared the safety and effectiveness of non-surgical management (NSM), anterior dens screw (ADS) fixation, and posterior arthrodesis (PA) in these patients.
