Hidden Patterns: Revealing The Noise Exposure Habits Of College Musicians Throughout The Year, Maya Vallejo
Hidden Patterns: Revealing The Noise Exposure Habits Of College Musicians Throughout The Year, Maya Vallejo
Holster Scholar Projects
College musicians face elevated risk for noise-induced hearing loss (NIHL) due to extensive rehearsal, headphone use, and performance activity, yet most objective sound-exposure studies rely on personal dosimeters that capture only brief windows (1–3 weeks) during the academic year, and overlook headphone-specific exposure. This study leveraged historical Apple Watch and AirPods sound data from participants in the ongoing AMI study (Skoe lab, University of Connecticut) to compare environmental and headphone noise exposure between academic-year and academic-break periods in college-age lifelong, former, and non-musicians. Sound levels (dBA) were calculated using duration-weighted, log-power averaging to account for the logarithmic nature of decibels. …

Background: Primary Biliary Cholangitis (PBC) is an autoimmune cholangiopathy with polygenic architecture and unknown aetiology. Evidence links Neanderthal-derived genetic variants to autoimmune conditions; however, their contribution to PBC susceptibility remains unexplored.
Objective: We investigate whether archaic alleles influence PBC risk.
Design: Genotype data from four PBC cohorts were analysed: CANUK (4615 cases, 9233 controls), OLD IT (444 cases, 901 controls), NEW IT (255 cases, 579 controls) and MAYO (891 cases, 621 controls). Association testing with 235 592 Neanderthal Informative Markers (NIMs) was performed, adjusting for population stratification. Heritability was estimated using RHE-mc and temporal haplotype dynamics were assessed using ancient …

Background and objectives: Electronic health records (EHRs) contain valuable information for research and decision-making, but much resides in unstructured notes that are challenging to analyze at scale. We developed SPELL (Snippet-Primed rEgex LLM Pipeline), a scalable natural language processing workflow that combines regular-expression-based snippet retrieval with locally hosted large language model (LLM) inference to extract structured variables from large collections of clinical narratives.
Methods: SPELL uses task-specific regular expressions to retrieve short context windows ("snippets") from unstructured texts and applies task-prompted LLM inference on snippets rather than full documents. All processing occurs within institutional computing environments. Accuracy was evaluated on …

Objective: STXBP1-related disorder (STXBP1-RD) and SYNGAP1-related disorder (SYNGAP1-RD) are two common genetic synaptopathies that are associated with epilepsy, developmental delay, intellectual developmental disorder, and behavioral problems. Both STXBP1-RD and SYNGAP1-RD are potential targets for disease-modifying therapies, but there is limited information in the literature describing the natural history of either disorder, which impedes outcome selection for future clinical trials. The objective of this study is to develop a framework to better define and outline the clinical spectrum and longitudinal trajectories of STXBP1-RD and SYNGAP1-RD natural history, including development, behavior, seizure histories, and electrophysiology.
Methods: Here, we describe a protocol, regulatory …

Elevated intratumoral interstitial fluid pressure (IFP) may negatively impact the efficacy of neoadjuvant chemotherapy (NAC). This study aimed to predict the breast cancer response to NAC based on intratumoral IFP using 3D contrast-enhanced ultrasound (CEUS) with subharmonic-aided pressure estimation (SHAPE). This prospective study enrolled breast cancer patients undergoing NAC from two clinical sites. 3D CEUS was performed at 0%, 10%, and 30% of NAC completion using a GE Logiq E9 with Definity. The pressure gradients between the tumor and adjacent normal tissue were estimated by SHAPE, utilizing the inverse linear relationship between the ultrasound contrast agent’s subharmonic signal amplitude and …

The accumulation of senescent cells is a recognized hallmark of biological aging and is associated with the onset of multiple chronic medical conditions. Senescent cells exhibit a distinct secretory profile, known as the senescence-associated secretory phenotype (SASP), which can propagate cellular senescence to neighboring and distant tissues. Measuring SASP factors in blood serves as a practical proxy for cellular senescence burden and may help track disease states and intervention outcomes. We developed and validated a composite SASP Score by integrating large-scale population proteomics data with a semi-supervised deep learning framework. The analytical workflow included: (1) selection of biologically curated SASP …

Bone morphogenetic protein receptor type 2 (BMPR2) encodes an evolutionarily conserved serine/threonine kinase that phosphorylates type-1 BMP receptors to mediate intercellular communication upon ligand binding. Loss-of-function variants in BMPR2 are known to cause pulmonary arterial hypertension and other cardiovascular disorders. Here, we report a rare recurrent missense variant in BMPR2 (c.1126G>A [GenBank: NM_001204] [p.Glu376Lys]) identified in six individuals, who all present with neurodevelopmental phenotypes including autism spectrum disorder and global developmental delay. Trio analysis demonstrates de novo occurrence of this variant in at least five of the individuals. Using the fruit fly Drosophila melanogaster, we show that this variant …

Numerous adaptations to interventions have been included in cognitive behavioral therapy (CBT) for autistic youth. This study examines the degree to which CBT adapted to the social needs of autistic youth confers significant benefit by promoting social responsiveness. A secondary analysis was conducted on a multisite randomized clinical trial (Wood et al. in JAMA Psychiatry 77:474-483, 2020) comparing adapted CBT with standard-of-practice CBT and treatment-as-usual (TA). Autistic youth (N = 167; aged 7-13) with maladaptive and interfering anxiety participated. The adapted CBT (BIACA) uses a modular format, with an emphasis on supplementing common CBT practice elements (reframing and graded exposure) …

Surgeons in training share notable parallels with competitive athletes, including rigorous training, skill specialization, and functioning under pressure. Surgical educators have leveraged this comparison, incorporating techniques from sports psychology, such as mental imagery and mindfulness, to improve trainees' individual performance and well-being. Surgical trainees and athletes both rely on effective teamwork for optimal outcomes, suggesting that residency leaders can glean valuable team-level insights from sports psychology. This article examines how the structure of surgical residency influences team dynamics, using concepts from sports psychology to identify evidence-based strategies for improving program cohesion and resident well-being. The surgical literature typically conceptualizes surgery …

Problem: Falls are a persistent patient-safety concern in skilled nursing facilities (SNFs), contributing to injury, functional decline, reduced quality of life, healthcare utilization, and costs. At the project site, a short-term rehabilitation unit in an urban SNF, 13 resident falls occurred during the baseline period, with 23% resulting in injury, and prevention relied on routine risk assessments, environmental checks, and post-fall reporting rather than proactive strategies.
Aim: To decrease the overall fall rate by 20%, reduce fall-associated injuries from 23% to 15% or less, and raise staff adherence to evidence-based fall-prevention interventions to 90% or higher within 12 weeks.

Objectives
To characterize assisted living (AL) direct care worker (DCW) training requirements across the United States and how these regulations have changed over time. Specifically, we examine state-level profiles of training regulations, including duration, frequency, content, source of materials, evaluation, and changes in regulatory coverage.
Design
Cross-sectional and longitudinal descriptive study using state policy data and AL directories from 2019, 2021, and 2023.
Setting and Participants
All US states' AL training–related regulations; all ALs and AL beds governed by those regulations.
Methods
We used a mixed-methods approach with an explanatory sequential design (QUAN → qual), first conducting health services regulatory …

Caribbean Hearing Health Readiness: A Data-Guided Method For Doctor Of Audiology Study-Abroad And Service Learning, Chelsea Tynes, Antony Joseph
AuD Capstone Projects - Communication Sciences and Disorders
Access to audiology care is essential, as unmanaged hearing loss carries significant communicative, social, and health consequences. Across many Caribbean nations, access to audiologic care remains limited, with substantial variation in workforce, diagnostic services, hearing-health infrastructure, and system readiness. The Caribbean Audiology Research Project (CARP), established by the Hearing Loss Prevention Laboratory, characterized hearing-health capacity across eight Caribbean countries: Antigua and Barbuda, Barbados, Dominica, Grenada, St. Kitts and Nevis, St. Lucia, St. Vincent and the Grenadines, and The Bahamas. Publicly available data on the audiology workforce, healthcare infrastructure, diagnostic and rehabilitative services, public-health programs, demographic indicators, and logistical factors were …

Objective The objective of this article was to conduct a comprehensive systematic review and meta-analysis on the molecular epidemiology of Mycobacterium tuberculosis in the Middle East and North Africa. Methods PubMed, Scopus, Web of Science, Embase, and Google Scholar databases were used for literature search on the molecular epidemiology, clustering rate and recent transmission index of Mycobacterium tuberculosis complex (MTBC) published between 1 January 2000 to 31 December 2025. The study quality was assessed using the Newcastle-Ottawa Scale (NOS) and meta-analysis was carried out using R statistics version 4.5.3. Results Fifty-nine studies reported 13,194 Mycobacterium tuberculosis (M. tuberculosis) isolates of …

Resistant Hypertension Secondary To Renal Arteriovenous Malformation: A Case Of Cure After Nephrectomy, Alexis Garza, Maximo A. Alvarez, Paul Kim, David Alonzo
School of Medicine Publications
BACKGROUND: Evaluation of multidrug-resistant hypertension in young adults requires consideration of uncommon secondary etiologies, including renovascular disease and congenital renal vascular anomalies. Renal arteriovenous malformations (rAVMs) represent a rare cause of secondary hypertension, accounting for less than 0.1% of renal vascular abnormalities. They most commonly present with hematuria, flank pain, or, in high-flow lesions, high output cardiac failure. Hypertension as the principal manifestation is uncommon.
CASE REPORT: We describe a 33-year-old woman with longstanding uncontrolled hypertension requiring 5 antihypertensive agents who presented for evaluation of refractory blood pressure control. Initial duplex ultrasound and angiography identified a large complex left renal …

Midwifery Student Attrition In Australia: A Scoping Review, Terri Downer, Tanya Capper, Olav Muurlink
Research outputs 2022 to 2026
Problem: Global literature suggests that attrition rates from entry-to-practice midwifery programs are increasing. However, little is known about student attrition in the Australian context.
Background: Australia faces a growing midwifery shortage, prompting calls to increase program enrolments. Evidence from the United Kingdom shows high and rising attrition in entry-to-practice programs. In Australia, the scale of midwifery student attrition and the factors influencing it remain largely underexplored.
Aim: To identify and synthesise existing Australian literature on factors influencing midwifery student attrition.
Methods: A search of PubMed, Medline, CINAHL Ultimate, Embase, MIDIRS, EBSCO, and Google Scholar databases was conducted with key data …
