The transition of advanced practice providers (APPs) from student to provider, or from an experienced provider to a new organization or location, has been identified as a high-stress period with potential turnover for new-hire APPs. APP preceptors play a key role in this transition. Limited time to teach during the clinical day and a limited knowledge base among new hires were two of many factors associated with increased preceptor burden. Online pharmacies improve access to medicines by offering convenient ordering and home delivery, but their growth also creates important risks for medication safety and public health. This review examines the benefits and hazards associated with online medicine supply, including access without a valid prescription, falsified and substandard products, inappropriate medicine use, privacy breaches, and weak cross-border regulatory oversight. The available evidence indicates that illicit online sellers can expose consumers to products of uncertain identity, quality, and dose, while vulnerable users may have limited ability to distinguish legitimate services from unlawful vendors. Frusemide is a loop diuretic used for hypertension, heart failure among many other indications. It has an absorption window in the stomach and it's a class IV drug according to BCS with low bioavailability.

Purpose: The primary purpose of this study was to explore the services mothers receive during hospital‑based labor and delivery and to identify perceived gaps in inpatient perinatal care. A secondary aim was to examine how these unmet needs may represent opportunities for occupational therapy involvement within hospital maternity settings. Methods: A convergent parallel mixed methods design was used with an anonymous electronic survey. Participants were eligible if they were 18 years or older and had given birth in a hospital within the past five years. Purpose: This pilot study was completed to determine whether individuals with paraplegia can perform activities of daily living (ADL) and instrumental activities of daily living (IADL) in their homes using a personal exoskeleton. The physiological effects of using an exoskeleton for these tasks were investigated as well. Methods: In this mixed-methods observational study, participants with paraplegia were observed using an exoskeleton to complete ADLs and IADLs in their homes. This paper presents the results of a developmental causal analysis of the adult personality of U.S. vice president JD Vance, from the conceptual perspective of personologist Theodore Millon.
Psychodiagnostically relevant data relating to Vance’s personality were collected from biographical sources and media reports and synthesized into a personality profile using the Millon Inventory of Diagnostic Criteria (MIDC), which yields 34 normal and maladaptive personality classifications congruent with DSM-III-R, DSM-IV, and DSM-5-TR.
The findings of the personality assessment were previously reported in a separate paper. Vance’s primary personality patterns were found to be Dominant/controlling and Ambitious/confident, supplemented by …

BACKGROUND: Slower chronic kidney disease (CKD) progression allows more time for transplant preparation. Whether differences in CKD progression by race/ethnicity associate with preemptive or living donor transplantation in youth has not been well studied.
METHODS: We examined the association between time spent with low eGFR (between 10-30 mL/min/1.73 m2) and odds of preemptive or living donor transplantation among youth with CKD. eGFR was estimated using the bedside Schwartz (if < 18 years) and CKD-EPI 2021 equations (if ≥ 18 years) and the CKiD U25 equation in sensitivity analysis. Time spent with low eGFR was compared by race/ethnicity.
RESULTS: Among 333 youth with CKD (median age 11 years [IQR 7,14]), median time spent with low eGFR was 28.8 months. 77% were preemptively waitlisted, and 45% received preemptive …

The thalamus is a critical subcortical hub that relays sensorimotor information and regulates higher-order cognitive processes. Accurate delineation of thalamic nuclei is essential for elucidating disease mechanisms and tracking clinical progression. In this study, we compared two segmentation approaches implemented in FreeSurfer: the conventional structural method and a joint framework that integrates diffusion tensor imaging. Magnetic resonance imaging (MRI) data from 24 healthy controls (HC), 27 patients with cognitively normal Parkinson’s disease (PD-CN), and 33 Parkinson's disease patients with mild cognitive impairment (PD-MCI) were analyzed. Introduction: Older adults exhibit unique risks associated with poor mental health and substance use. The Beyond 50 study is a longitudinal cohort investigating the determinants of healthy ageing, including relationships between physical and psychosocial health and substance use. We aim to characterise the Beyond 50 cohort and explore patterns of physical and psychosocial health and substance use. Methods: Baseline data collection was completed between September 2023 and August 2024. Annual follow-up data collection commenced in September 2024. This article discusses the potential of digital health interventions (DHIs) in mental health care, emphasizing the importance of designing them to foster genuine humanistic care. It highlights current challenges in measuring engagement and outcomes, advocating for integrating human-centered endpoints such as dignity, autonomy, and therapeutic alliance into evaluation frameworks. The authors call for mechanistic research to identify effective engagement strategies and context-specific implementation approaches. Ultimately, successful DHIs should demonstrate both clinical efficacy and the preservation of humane, person-centered care, ensuring ethical and meaningful mental health support through technology.

Estrogen receptor-positive (ER+) breast cancer exhibits a marked propensity for skeletal metastasis; however, the molecular drivers of bone colonization remain incompletely defined. We investigated the tumor suppressor neurofibromin (NF1), a dual repressor for RAS and ER signaling, whose inactivation promotes endocrine therapy (ET) resistance and is associated with inferior relapse-free survival. NF1 copy number loss was detected in 62% of ER+ patients who subsequently developed metastases and was associated with an increased likelihood of bone metastases at initial diagnosis. In mouse xenograft models, NF1-depleted ER+ breast cancer cells demonstrated enhanced dissemination to skeletal sites following surgical resection of primary tumors. …

RATIONALE AND OBJECTIVES: To evaluate access to contrast-enhanced MRI among patients with acute or chronic renal failure after implementation of a more permissive contrast policy and determine whether any patients developed nephrogenic systemic fibrosis (NSF).
MATERIALS AND METHODS: This retrospective study included all patients who had an order for contrast-enhanced MRI in our healthcare system between July 1st, 2023 and June 30th, 2025. Each exam was categorized as completed or cancelled, and multivariable analysis was performed to identify factors associated with exam cancellation. Potential diagnoses of NSF were identified by searching the problem list and encounter diagnoses using ICD-10 codes. …

Beyond Lipid-Lowering: Pleiotropic Effects Of Statins In Inflammation, Coagulation, And Cell Proliferation – Mechanisms And Clinical Implications, Kasidid Lawongsa, Yutthana Pansuwan, Supanut Kumjan

Statins are widely used lipid-lowering agents that significantly reduce cardiovascular morbidity and mortality through inhibition of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase. Beyond their cholesterol-lowering properties, increasing evidence demonstrates that statins exert several pleiotropic effects independent of lipid reduction. These effects include anti-inflammatory, anti-thrombotic, and anti-proliferative actions mediated primarily through inhibition of the mevalonate pathway and downstream signaling molecules such as Rho, Rac, and Ras. Experimental and clinical studies have suggested potential benefits of statins in various non-cardiovascular conditions, including autoimmune diseases, venous thromboembolism, sepsis-associated coagulopathy, and certain malignancies. Background: The murine double minute 2 (MDM2) gene encodes an E3 ubiquitin ligase that targets p53 for degradation, thereby influencing the pathogenesis of oral squamous cell carcinoma (OSCC).
Objective. The present study investigates MDM2 mRNA expression and its association with clinical and pathological characteristics in OSCC patients.
Methods: We conducted an observational-analytical study using a cross-sectional design. The study involved 39 OSCC patients. RNA extraction, cDNA synthesis, and quantitative PCR (qPCR) were performed. We quantified relative MDM2 mRNA expression using the Livak method (2-ΔΔCT) and GenEX MultiD software version 6.0. Malocclusion is a prevalent oral health problem in children associated with oral bad habits, yet the extent to which parental and child knowledge, attitudes, and practices (KAP) influence orthodontic status remains insufficiently understood. This cross-sectional study investigated relationships between parental and child KAP, oral habits, and malocclusion in school-aged children, involving 67 child–parent pairs from an elementary school in Surabaya, Indonesia. Parental and child KAP as well as child-reported oral habits were assessed using a validated questionnaire.
