Purpose: Drug overdoses are now a leading cause of pregnancy-related deaths in the United States. Despite evidence of rural–urban disparities in substance use, there has not yet been a nationally representative examination of rural–urban differences in perinatal substance use. This study provides a comprehensive examination of rural–urban disparities in perinatal substance use. Methods: This study uses cross-sectional data to examine 3499 pregnant women from the 2015–2019 National Survey on Drug Use and Health (NSDUH). Rural (nonmetro)–urban (metro) differences in past-month tobacco use, alcohol use, binge drinking, illicit drug use, and marijuana use were examined using Rao–Scott chi-square tests and multivariable …

Treatment Of Osteoporosis And Osteoarthritis In The Oldest Old, Daniel Pinto
Treatment Of Osteoporosis And Osteoarthritis In The Oldest Old, Daniel Pinto
Physical Therapy Faculty Research and Publications
Osteoporosis and osteoarthritis are key diseases of musculoskeletal ageing and are increasing in prevalence and burden with the progressively ageing population worldwide. These conditions are thus particularly common in ‘the oldest old’, and there are complexities of managing them within the context of extensive multimorbidity, physical and mental disability, and polypharmacy, the rates for all of which are high in this population. In this narrative review, we explore the epidemiology of osteoporosis and osteoarthritis in the oldest old before examining trials and real-world data relating to the pharmacological treatment of these diseases in older adults, including anti-resorptives and bone-forming agents …

Data indicate that earlier initiation of anti-tumor necrosis factor alpha (anti-TNF-α) biologic medicines may prevent progression to irreversible bowel damage and improve outcomes for patients with inflammatory bowel disease (IBD), particularly Crohn's disease. However, the high cost of such therapies may restrict access and prevent timely treatment of IBD. Biosimilar anti-TNF-α medicines may represent a valuable opportunity for cost savings and optimized patient outcomes by improving access to advanced therapies and allowing earlier anti-TNF-α treatment initiation. Biosimilar anti-TNF-α medicines have been shown to offer consistent therapeutic outcomes to their reference medicines, yet despite entering the IBD treatment armamentarium over 10 …

Hurricane Irma's Impact On Mortality Among The Chronically Ill Populations In Florida, Cody Ray Thornton
Walden Dissertations and Doctoral Studies
Natural disasters pose significant risks to individuals with chronic diseases, exacerbating health disparities through disruptions in healthcare access and systemic vulnerabilities. This study investigated the association between chronic disease-related mortality occurring within 2 months of Hurricane Irma, and whether demographic variables, including age, sex, race, Hispanic ethnicity, poverty, urbanicity, and educational attainment, modified this relationship. This cross-sectional study used the social-ecological model to analyze secondary mortality data from the National Vital Statistics System. Bivariate and multivariate logistic regression revealed that deaths from chronic illnesses, occurring between September 2017 and October 2017, were 2.694 times higher, compared to the same period …

A Disparities Analysis Of Cancer Patients’ Access To Depression Treatment By Health Insurance Status And Type, Stephanie Minbiole-Snider
Walden Dissertations and Doctoral Studies
Research has demonstrated that comorbid conditions like depression in cancer patients reduce quality of life, prolong recovery, and reduce survival time of the cancer patient. Although other studies have investigated the issue of depression among cancer patients, there is a dearth of studies looking at differences in taking depression medication with health insurance as a source of disparity. Using survey data from the National Health Interview Survey (NHIS), and the health belief model as the theoretical framework, this study had two purposes. The first was to determine if there were differences in the likelihood of taking medication for depression based …

Background
High-quality estimates of the epidemiology of the autism spectrum and the health needs of autistic people are necessary for service planners and resource allocators. Here we present the global prevalence and health burden of autism spectrum disorder from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 following improvements to the epidemiological data and burden estimation methods.
Methods
For GBD 2021, a systematic literature review involving searches in PubMed, Embase, PsycINFO, the Global Health Data Exchange, and consultation with experts identified data on the epidemiology of autism spectrum disorder. Eligible data were used to estimate prevalence …

Background: Wilms tumor (WT) is the most common pediatric malignancy of the kidney. Past studies describing WT incidence and survival used surveillance data with < 30% of the US population. We evaluated differences in WT incidence and survival comparing demographic groups and tumor characteristics.
Methods: We analyzed new cases of WT among patients aged < 20 years at diagnosis by using incidence data from US Cancer Statistics (USCS) for 2003-2020 and 5-year relative survival (RS) data from the National Program of Cancer Registries (NPCR) for 2001-2019. To assess incidence trends, average annual percent change (AAPC) was calculated by using joinpoint regression. Relative survival (RS) and all-cause survival were calculated overall and by demographic and clinical variables.
Results: During 2003-2020, 8218 cases of WT were reported in USCS, which represented an age-adjusted incidence rate of 5.7 cases per million. Rates were the highest among females (6.3), children aged 0-4 years (17.2), and non-Hispanic Black patients (7.1). Overall, trends remained stable (AAPC = -0.4, 95% CI: -1.4 to 0.4). Among 7567 cases of WT in NPCR, 5-year RS was 92.6%. Patients with the lowest …
