Microbial Monitoring, Harry P. Dalton
Microbial Monitoring, Harry P. Dalton
MCV/Q, Medical College of Virginia Quarterly
The need for monitoring both areas for alteration in microbial flora, which may result in invasion or adverse effect on internal organs of the host, has been an accepted scientific fact of modern medicine for years. Questions remain, however, on how much monitoring needs to be done. What areas of the patients and the environment should be tested, and what are the practical uses of the results of such monitoring? Before attempting to answer these questions, let me give you an overview of how microbial monitoring has been used at the Medical College of Virginia hospitals and other institutions to …
The Pharmacology And Clinical Use Of Lidocaine And Procainamide, J. Thomas Bigger Jr., Elsa-Grace V. Giardina
The Pharmacology And Clinical Use Of Lidocaine And Procainamide, J. Thomas Bigger Jr., Elsa-Grace V. Giardina
MCV/Q, Medical College of Virginia Quarterly
Both procainamide and lidocaine are useful for acutely treating cardiac arrhythmias, and procainamide can be useful in chronic antiarrhythmic regimens. Successful management of cardiac arrhythmias requires knowledge of: 1) the mechanism and natural history of the arrhythmia, 2) the physiologic state of the patient, and 3) the cardiac effects, pharmacodynamics, and general pharmacology of the antiarrhythmic drugs.
Pharmacology Of Antiarrhythmics: Quinidine, Beta-Blockers, Diphenylhydantoin, Bretylium, Albert J. Wasserman, Jack D. Proctor
Pharmacology Of Antiarrhythmics: Quinidine, Beta-Blockers, Diphenylhydantoin, Bretylium, Albert J. Wasserman, Jack D. Proctor
MCV/Q, Medical College of Virginia Quarterly
The electrophysiologic effects of the antiarrhythmic drugs, presented elsewhere in this symposium, form only one of the bases for the selection of a therapeutic agent in any given clinical situation. The final choice depends on the following factors: 1. The specific arrhythmia; 2. Underlying heart disease, if any; 3. The degree of compromise of the circulation, if any; 4. The etiology of the arrhythmia; 5. The efficacy of the drug for that arrhythmia; 6. The toxicity of the drug, especially in the given patient with possible alterations in volume of distribution, biotransformation, and excretion; 7. The electrophysiologic effects of the …
Ambulatory Care For Emphysema And Chronic Bronchitis, Thomas L. Petty
Ambulatory Care For Emphysema And Chronic Bronchitis, Thomas L. Petty
MCV/Q, Medical College of Virginia Quarterly
The immense problem of chronic airway obstruction (CAO)--emphysema and chronic bronchitis--which has now reached epidemic proportions, presents to the practitioner of medicine an increasing number of suffering persons asking for care. These patients are dyspneic, anxious, bewildered by their predicament, sometimes demanding and frightened about prospects for future comfortable life.
The Pathophysiology Of Respiratory Failure In Chronic Obstructive Pulmonary Disease, John L. Patterson, James P. Baker, Walter J. O'Donohue Jr.
The Pathophysiology Of Respiratory Failure In Chronic Obstructive Pulmonary Disease, John L. Patterson, James P. Baker, Walter J. O'Donohue Jr.
MCV/Q, Medical College of Virginia Quarterly
The most important pathophysiologic aspects of chronic and acute obstructive pulmonary disease involve disturbances in ventilation with resulting derangement of gas exchange in the lung. There is considerable variability in the type of abnormality, the time of its appearance in relationship to the history of the disease, and in the progression of the abnormality in blood gases.

Urinary Incontinence: A Problem To Hold On To, John H. Texter Jr.
MCV/Q, Medical College of Virginia Quarterly
In this presentation attention will be focused on the problem of the urinary incontinence. We will look briefly at the various types and causes for this distressing disorder. Mention will also be made as to treatment with particular emphasis on the surgical correction (or attempts at correction) of the postoperative urine leakage such as one sees following prostatectomy or trauma.
Surgery Of The Adrenal Glands, Arthur W. Wyker Jr.
Surgery Of The Adrenal Glands, Arthur W. Wyker Jr.
MCV/Q, Medical College of Virginia Quarterly
This paper is a surgical overview of adrenal gland disorders and emphasizes new developments in diagnosis and management. Adrenal disorders are relatively rare but clinically, they are very important. If they go undetected, they can produce severe metabolic disturbances which may be fatal.
The Optimum Treatment For Undescended Testis, John K. Lattimer
The Optimum Treatment For Undescended Testis, John K. Lattimer
MCV/Q, Medical College of Virginia Quarterly
In summary, there are many reasons why orchiopexy should be done, and certainly, the cancer worry is a very distinct one. Yet, I think the psychological reason, the "locker room appearance" situation, is the most frequent, and therefore, an important factor. I do think that we should exercise our favored position of being able to offer hormone treatment first. If that does not work, then go on with surgery as required. The timing, in my opinion, should be to start at the fourth birthday and to have the program completed by the fifth birthday. Although physiological considerations do enter into …

This report redescribes the adult respiratory distress syndrome and discusses factors which influence prognosis and presents principles of management.
Medical Management Of Stone Disease, M. J. V. Smith
Medical Management Of Stone Disease, M. J. V. Smith
MCV/Q, Medical College of Virginia Quarterly
The purpose of this review is to try to take a practical approach toward the management of patients with renal calculous disease. This will be a personal approach; I will, however, try to avoid injecting my prejudices and research interests. I want to show that proper management can be achieved even in the most unsophisticated environment.

Newer Techniques In Clinical Cytology: Aspiration Biopsy And Brushing Cytology, William J. Frable
Newer Techniques In Clinical Cytology: Aspiration Biopsy And Brushing Cytology, William J. Frable
MCV/Q, Medical College of Virginia Quarterly
Since its introduction some twenty years ago the Papanicolaou technique for cervical-vaginal cytology has had a tremendous impact on the detection of precancerous lesions and in situ cancer in the female genital tract. It has forced the clinician and pathologist to study the "early lesions" of cancer, particularly in the female genital tract and has brought about a better understanding of their biology. Cytology has over the years been extended to other areas, particularly the respiratory tract. Two recent areas of emphasis are the thin-needle aspiration biopsy and direct brushing of mucosal surfaces through the various fiber optic scopes now …

Bedside Assessment Of Left Ventricular Function In The Respiratory Intensive Care Unit, Clifton L. Parker
Bedside Assessment Of Left Ventricular Function In The Respiratory Intensive Care Unit, Clifton L. Parker
MCV/Q, Medical College of Virginia Quarterly
The measurement of the central venous pressure is a widely used technique for gaining information concerning the relationship of the blood volume as it relates to cardiac function. This concept has been widely popularized, and the central venous pressure measurement is extremely useful; however, it has too frequently been thought to be a direct measurement of cardiac function.
Some Important Factors In Community Dialysis Program, Allan A. Hoffman
Some Important Factors In Community Dialysis Program, Allan A. Hoffman
MCV/Q, Medical College of Virginia Quarterly
The purpose of this paper is to acquaint physicians who are not nephrologists with the experience of one of their colleagues in the operation of a hemodialysis service as a part-time endeavor. My intention is not to present detailed or advanced scientific data on dialysis techniques or on the care of chronic renal failure but to discuss how and under what circumstances the urologist, internist, or general physician might significantly augment the present treatment care system for end-stage chronic renal disease. I will attempt to describe a mode and philosophy of operation applicable to the community hospital and private practice …
Verbal Sequencing Ability As A Predictor Of Reading Disability, Paul Elmer Quin
Verbal Sequencing Ability As A Predictor Of Reading Disability, Paul Elmer Quin
Dissertations and Theses
Early identification of children with reading and learning problems seems imperative. By using early identification predictors, a high risk group of children with possible reading problems could be located. This would facilitate placement and appropriate educational strategies for this group of children. By proper placement and planning, educators then would be able to set up remedial and/or preventive programs for these children before the pattern of unsuccessful attempts and improper training becomes irreversible. This study was designed to determine if verbal sequencing ability was a valid predictor of reading ability for a group of 31 preschool children. Secondarily, the investigation …

The Basic Principles Of Acid-Base Regulation, Orhan Muren
MCV/Q, Medical College of Virginia Quarterly
In summary, arterial gas studies including pH determinations can be vitally important in the diagnosis and management of patients with a variety of serious medical problems. These studies should be interpreted in the light of clinical and other necessary laboratory findings.
How To Get Patients On And Off Respirators, Terring W. Heironimus Iii
How To Get Patients On And Off Respirators, Terring W. Heironimus Iii
MCV/Q, Medical College of Virginia Quarterly
This title implies that one has already decided that one's patient needs to be artificially ventilated, that is, that he is in respiratory failure. How does one diagnose respiratory failure? Much of what I have to say in this regard is in terms of arbitrary limits, values, and guidelines. Since these guidelines are arbitrary, there may exist legitimate grounds for differences of opinion about some of them. However, we have found these guidelines to be quite helpful, and experience would indicate that they are reasonable.

Renal Hypertension, Eugene F. Poutasse
Renal Hypertension, Eugene F. Poutasse
MCV/Q, Medical College of Virginia Quarterly
The advances in our knowledge of renal hypertension over the last 75 years represents outstanding accomplishments in experimental and clinical medicine. A review of the highlights of this important investigation in hypertension should begin with Tigerstedt and Bergman's work in Scandinavia which demonstrated in 1898, that extracts of kidney possessed a hypertensive action. They gave the name "renin" to the impure substance which produced this effect. For the next 30 years much work was done on the vasoconstrictor action of extracts of the adrenal medulla, the nervous system and the kidney.
Surgery Of The Genitalia In Children, Charles J. Devine
Surgery Of The Genitalia In Children, Charles J. Devine
MCV/Q, Medical College of Virginia Quarterly
I shall confine my paper to the external male genitalia. Most of the problems that we have dealt with have been congenital from simple meatal strictures to more severe, although not necessarily more serious, situations. In the last 18 years, I have worked closely with Dr. Charles Horton, and much of this report represents our joint efforts.
Quality Control In The Office Laboratory, Joseph H. Riddick Jr.
Quality Control In The Office Laboratory, Joseph H. Riddick Jr.
MCV/Q, Medical College of Virginia Quarterly
First, how can the quality of the office laboratory be assured and documented? Second, how can this information be disseminated to legislators and the general public? The first is an easily solved technical problem toward which this paper is directed. The second and probably the most difficult is a media problem beyond the scope of a technical journal.
Evaluation Of Prepackaged "Kits", Rex B. Conn Jr.
Evaluation Of Prepackaged "Kits", Rex B. Conn Jr.
MCV/Q, Medical College of Virginia Quarterly
Prepackaged laboratory kits for performing diagnostic procedures are frequently the most suitable alternative in the selection of laboratory methods, especially in the physicians' offices and small laboratories. Because of the previous lack of governmental regulations covering the manufacture of kits, many kits now on the market do not perform adequately and may produce misleading results. Each laboratory must evaluate each type of kit before it is put into routine use. This evaluation should include a review of published experimental data, comparison of results using the kit to results using a reference method and an experimental evaluation of the kit in …
