The purpose of the present research was to summarize from the pediatric literature the norms for speech and language development and to determine if the pediatrician is utilizing these norms as criteria for making referrals to speech and language pathologists. Research tends to show a discrepancy between the incidence of speech and language disorders and the rate of referrals made by physicians.

The methodology involved developing a three-part questionnaire to determine what screening instruments are being used in the pediatric office in regard to speech, language, and hearing, and what informal criteria are utilized for making referrals for a speech …

A Comparison Of Self-Esteem And Health Locus Of Control In Adolescents With Insulin Dependent Diabetes Mellitus, Janice Mace

This study was a descriptive survey establishing a baseline level of self-esteem and health locus of control in insulin dependent diabetes mellitus (IDDM) adolescents. The purpose of the research was to study and describe health behaviors of IDDM adolescents in regards to self-concept and health locus of control. These behaviors are believed to have life-long ramifications on the IDDM adolescent's health status.

The sample consisted of 91 IDDM adolescents, 34 male and 57 female, in a nonrandom convenience cluster attending a 10-day summer camp.

Women And Chronic Renal Failure: Some Neglected Issues, Nancy G. Kutner, Heather L. Gray

It has been assumed until recently that chronic renal failure is more prevalent among men than among women, but data now indicate that at least half of all renal patients are women. The literature continues to focus on adjustment problems of male patients, especially sexual adjustment and job-loss problems, and to assume that women can adjust more easily because of their ability to maintain the homemaker role. However, women patients whose work status is that of homemaker are found to have the highest depression scores, and job loss results in low satisfaction for those who have held meaningful outside jobs. …

The Epidural Defect On Myelography: Some Unusual Causes, Keenan L. Carroll

This report describes four cases of epidural defect found on myelography that illustrate some unusual causes of this defect. These include a type I chondrosarcoma, primary hemangioendothelioma, hematoma, and an epidural abscess. Surgery was performed, and all four patients have recovered with little or no remaining neurological deficit. If there is anything unusual about the epidural defect or if the clinical findings do not agree with the radiographic films, the radiologist should be alert for other, less common causes of the defect.

Second-Year Follow-Up Study On Systematic Treatment Of Chronic Pain With Antidepressants, Dietrich Blumer, Mary Heilbronn

Systematic therapy with antidepressant drugs can be an effective treatment for chronic pain patients with the pain-prone disorder. This survey follows the pain status, medications, depressive symptoms, work status, and treatment of 104 patients treated for chronic pain at Henry Ford Hospital's Pain Clinic during a two-year period for chronic pain in the absence of a significant somatic disorder. No patients were excluded from treatment and the dropout rate of 64% after two years was not surprising. The pain of those who complied with therapy improved significantly (62%), compared to that of the patients who had dropped out of the …

The presence of clinically undetectable medullary thyroid cancer can be revealed by characterisdc patterns of calcitonin in serum after the injection of a provocative agent, pentagastrin or calcium ion. However, in some reports, medullary thyroid cancer was uncovered in a test with pentagastrin but not in one with calcium ion, and vice versa. To reduce the number of erroneous responses, each provocative agent has been employed in separate tests. Combining pentagastrin and calcium into one stimulus should give the advantage of each agent while requiring less time and fewer analyses in a single test. Surgical Management Of Multicentricity, Nipple-Areola Involvement, And Bilaterality In Breast Carcinoma, F. Samhouri, M. Block, A. Kambouris

During a five-year period, 457 women who underwent partial, segmental, or total mastectomy were assessed for multicentricity (31%), nipple-areola involvement (17%), and bilateral breast carcinoma (9%). These factors determined the extent of surgery performed and the possibilities for later breast reconstruction. The frequency of nipple-areola involvement in breast cancer strongly indicates the need for alternatives to their preservation for breast reconstruction. The study concludes that total mastectomy for invasive carcinoma is the preferred method of treatment for early breast cancer until further clinical trials have assessed the value of segmentectomy.

Preliminary Experience With Endoscopic Papillotomy, Bernard Schuman

Endoscopic papillotomy is a non-operadve technique used to manage choledocholithiasis and papillary stenosis. Considerable experience in Europe and initial results in a few medical centers in the United States indicate that it is effective and safe. Six papillotomy procedures were performed at Henry Ford Hospital without complication, and with a successful outcome in five. Endoscopic papillotomy is the procedure of choice for retained common bile duct stones less than 2 cm in diameter.

Acute And Massive Intraparenchymal Hemorrhages Of The Posterior Fossa, Dennis E. Wilkins

Acute, massive cerebellar and pontine hemorrhages are discussed in the context of the routine availability of cranial computerized tomography. The diagnosis of these disorders is emphasized. An updated clinical review of the edologic, pathogenetic, and therapeutic aspects is also presented, with particular attention to the importance of hypertensive cerebrovascular disease as the most common underlying disorder.

Prenatal Diagnosis Of Fetal Cystic Hygromas Associated With Generalized Lymphangiectasis, Chang Y. Lee, Beatrice L. Madrazo, Daniel L. Van Dyke, Jewel Smith

Ultrasonography has made possible the prenatal diagnosis of many congenital fetal abnormalities. This report describes two cases of bilateral cystic hygromas of the neck associated with generalized lymphangiectasis that were diagnosed by ultrasound. Ultrasonic scans revealed moderate polyhydramnios: thick, edematous placenta and edematous fetus with large cystic mass occupying both sides of the neck and extending to the upper chest wall, ascites, and pleural effusion at gestational ages of 21.5 and 24 weeks, respectively. In one case, chromosomal study from amniotic fluid cell culture revealed X chromosome monosomy, often associated with lymphatic anomalies. The prenatal diagnosis was confirmed at birth: …

Three case histories of patients with malfunctions of Smeloff-Cutter mitral ball valve prostheses are presented here - one underwent successful replacement. The clinical diagnosis was made by observing a marked variation in the A2OC interval and intermittent absence of the opening click. Phonocardiograms were diagnostic in all three cases. while echocardiograms and even angiograms did not uniformly diagnose prosthetic valve dysfunction. Periodic phonocardiographic evaluations may be helpful in early detection of the prosthetic valve dysfunction. Once the diagnosis is established, immediate surgical treatment is imperative to prevent sudden death.

In the early decades of the twentieth century prenatal care became an accepted part of routine obstetrical care. This thesis is a description of the evolution of the prenatal care idea in the United States during these years. Two approaches are used in this description. An internal approach to prenatal care notes the maturation of procedure and technique. An external approach notes the inpatient and outpatient suggestions for providing prenatal care, the suggestions for educating the public to value and expect prenatal care and the medical profession to provide such care, and the varied promoters of such care, with their …

As a part of a larger research project with comatose patients the problem stated for this portion of the research study was to investigate the potential for identifying whether or not the quality of nursing care for comatose patients could be improved by nursing therapy with environmental enrichment and meaningful systematic orienting stimuli given by professional nurse clinicians and a patient's significant others. The effects of the orienting stimuli were measured by observable changes in arousal patterns in serial electroencephalograms (EEGs) , level of arousal as scored on the Glasgow Coma Scale (GCS), and selected measures of recovery outcome (RO). …

1. Isolated Amphiuma small intestine exposed on both surfaces to buffered or unbuffered media generated gradients of pH under short-circuited conditions consistent with secretion of HCO3(-). 2. When unbuffered mucosal medium was maintained at pH 7.4 by addition of acid, alkalinization of the mucosal medium occurred at a rate of 1-2 microequiv/hr cm2 under short-circuit conditions (Isc) and was reduced by anoxia, acetazolamide or removal of CO2. 3. A clinical trial is currently in progress to determine the efficacy of combined fractions of hyperthermia and radiation. The protocol consists of two parts. First, four fractions of microwave-induced hyperthermia (45.0° ± 0.5°C) are applied for 1 1/2 hours to the volume encompassing the tumor, each separated by 72 hours. After a one-week rest, a second series of four fractions is administered again at 72- hour intervals. Each fraction consists of a 400 rad dose of radiation followed within 20 minutes by hyperthermia (42.5 ± 0.5°C) for 1 1/2 hours. Currently, we have treated 62 patients with 82 fields with …
