Transtracheal jet ventilation (TTJV) using a percutaneously inserted intravenous (IV) catheter for the patient who cannot be ventilated or tracheally intubated or, using a jet stylet for changing endotracheal tubes (ETT) in patients for whom subsequent ventilation and/or tracheal reintubation may be difficult, are extremely valuable therapeutic options. The jet ventilation system must have a sufficiently high pressure-oxygen source to drive oxygen through noncompliant tubing and through relatively small IV catheters and/or jet stylets in order to achieve adequate ventilation and oxygenation. Mice carrying the Tight skin (Tsk) mutation harbor a genomic duplication within the fibrillin-1 (Fbn 1) gene that results in a larger than normal in-frame Fbn 1 transcript. In this study, the consequences of the Tsk mutation for fibrillin-containing microfibrils have been examined. Dermal fibroblasts from Tsk/+ mice synthesized and secreted both normal fibrillin (approximately 330 kD) and the mutant oversized Tsk fibrillin-1 (approximately 450 kD) in comparable amounts, and Tsk fibrillin-1 was stably incorporated into cell layers. Immunohistochemical and ultrastructural analyses of normal and Tsk/+ mouse skin highlighted differences in the gross organization and distribution of microfibrillar arrays. Rotary …

Most intracranial aneurysms (IA) are thinner at the apex; and 84% rupture at the apex. (Weir, 1987, Crompton, 1966). Minimum thickness of an unruptured aneurysm is reported 20 microns (Suzuki, et al 1980). This study focused upon physician/patient interaction as it pertains to patient involvement in personal health-care planning decision making, and self-care. The study was a multi-intervention trial that allowed for testing the effectiveness of two instruments used separately or simultaneously, in combination. Participants were 73 female patients, ages 41 to 76, diagnosed with essential hypertension and requiring medication for control.

Patients were randomly assigned to four groups: 1) a control group that was given no intervention, 2) a Pre-Knowledge Intervention (PKI) group that was given an interactive treatment plan with foreknowledge of what their physicians would probably do at their next …

PURPOSE: To examine the ability of protein kinase C (PKC) inhibitors and activators to influence the rate of corneal re-epithelialization in the rat. METHOD: Rat corneas with 3 mm diameter central epithelial abrasions were organ-cultured in control medium or in medium with inhibitors or activators of PKC. RESULTS: In control corneas, the defect was completely re-epithelialized by 25 hr. In the presence of the PKC inhibitors staurosporine (100 nM), sphinganine (50 mumol/l), or H-7 (100 mumol/l) there were significantly larger epithelial defects than in controls after 5-25 hr of incubation. The urgency of dealing with impacted ampullary stones is underlined in the first case reported by Opie6 at the autopsy of a young woman. Impacted ampullary stones that prevent papillary cannulation and standard sphincterotomy make up less than 5% of common bile duct stones seen at ERCP.3 Several series suggest the safe use of needle-knife sphincterotomy to create a choledochoduodenal fistula or to allow subsequent standard sphincterotomy in this situation. These series have also reported failure to extract the stone so that surgical removal was required.3,4Successful sphincterotomy may facilitate the early diagnosis of tumors that cause the ampulla to bulge. …

Penile strangulation caused by a thread or human hair is an uncommon cause of urethral injury described in paediatric piactice. Five children presented with this entity to the Aga Khan University Hospital, Karachi between August, 1991 and August, 1992. Three children had uneventful recoveries after removal of the contricting agent. Two patients developed urethrocutaneaus fistulae and partial amputation of the penis because of late presentation Early recognition and removal of the constricting agent is necessary to prevent serious complications

The hypothetical incidence of reproductive cancers resulting from oral contraceptive use was estimated in several models comparing the cumulative lifetime incidence of cancer of the breast, cervix, ovary and endometrium expected in pill users with the incidence expected in nonusers. The potential number of cancer-free days that would be gained or lost by pill users was com- pared with similar estimates among nonusers. If five years or more of pill use were associated with a 20% increase in the risk of breast cancerbeing diagnosed before age 50, a 20% increase in cervical cancer risk and a 50% reduction in the …

To test the hypothesis that epidermal rather than dermal mosaicism determines Blaschko's lines in hypomelanosis of Ito (HI), we studied the distribution of chromosomal mosaicism in four patients. In two, mosaicism had not been detected in lymphocytes or dermal fibroblasts, but was clearly shown in epidermal keratinocytes; furthermore, the abnormal cell line was confirmed to the hypopigmented epidermis and the normal epidermis contained only normal cells. Negative findings in the other two patients might be because of mosaicism which was undetected either because it was submicroscopic or because it was present in melanocytes, which have not yet been studied. The Role Of Sn-1,2-Diacylglycerol In Insulin-Stimulated Glucose Transport In Rat Skeletal Muscle, Kevin Paul Boggs

Insulin stimulates glucose transport into skeletal muscle upon binding to its cell surface receptor. The signals which stimulate transport are unknown. Studies of insulin's effects on muscle cell lines and adipocytes suggest that increases in sn-l,2-diacyglycerol (DAG) content may mediate insulin-stimulated glucose transport. To test this, the effect of insulin on muscle DAG content was measured. DAG increased in diaphragm and soleus muscles exposed to insulin in vitro and in diaphragm in vivo. However, the supraphysiologic insulin dose (60 nM) and incubation time required (10 min) are not consistent with a role for DAG as a mediator of insulin stimulated …
