The human gut contains bacteria, viruses, and archaea that influence health depending on age, diet, and location. An individual harbours about 150 bacterial species. An imbalance of these microorganisms, known as dysbiosis, can negatively impact health. Commensal bacteria maintain microbial homeostasis by building short chain fatty acids. The 2-way communication channel that connects the gut and the brain is referred to as the gut-brain axis. Dysbiosis of the gut leads to some autoimmune diseases. A decline of beneficial bacteria causes the progression of rheumatoid arthritis. Gut dysbiosis increases intestinal permeability, allowing toxins and cytokines to enter the bloodstream and damage …

This comprehensive review synthesizes recent advances in cancer nanotherapy, focusing on 6 emerging categories: immunomodulation, gene/RNA delivery, theranostics, radiosensitization, biomimetic carriers, and artificial intelligence (AI)-guided formulations. These innovative platforms demonstrate transformative potential by overcoming key limitations of conventional cancer treatments. Immunomodulatory nanoparticles reprogram tumor microenvironments and convert immunologically ``cold'' tumors to ``hot'' responsive states. Gene delivery systems enable precise editing with >50% efficiency, while theranostic platforms integrate real-time imaging with targeted therapy. Radiosensitizers amplify local radiation dose by twofold, and biomimetic carriers achieve prolonged circulation through immune evasion. Objective: To assess the available data regarding medicinal, surgical, and combination therapy approaches for odontogenic sinusitis linked to OAC and OAF. Oroantral communication (OAC), an atypical connection between the oral cavity and maxillary sinus, commonly arises following upper jaw procedures, trauma, or infection. Chronic odontogenic maxillary sinusitis may result from OAC developing into an oroantral fistula (OAF) if treatment is not received. Recurrent infections and functional difficulties can be avoided by managing these illnesses effectively. Mental well-being of orphaned children is a profound intricate topic that connects with broader concerns of childcare. This systematic review aims to evaluate the literature regarding the relationship between mental health and parental absence. Methods: We conducted a systematic search of PubMed, MEDLINE, and Science Direct in October 2025, with 1017 screened studies. Six studies met the inclusion criteria. Preoperative understanding of complex congenital heart disease is crucial for proper management. Surgical direction of partial anomalous pulmonary venous drainage and atrial septal defect closure when combined by the rare azygos continuation of interrupted inferior vena cava (IVC) may be associated with anomalous systemic to pulmonary venous connection, causing right-to-left shunt. We report a 44-year-old male with a history of congenital heart disease repaired in early childhood. Diagnostic imaging revealed an interrupted IVC with azygos continuation draining into the right upper pulmonary vein to the left atrium. CT play an essential role in recognition of venous anomalies with unexplained cyanosis.

Background: Deep peroneal neurectomy (DPN) evolved as a surgical treatment alternative for patients with painful midfoot arthritis who may not tolerate more invasive surgery such as arthrodesis. This scoping review aims to display current evidence of postoperative recovery complications and patient-reported outcomes specific to alternative surgical treatment of DPN.
Methods: A scoping review was completed using PRISMA-ScR reporting standards. Sickle cell disease (SCD) is the most common inherited blood disorder in the United States. The American Society of Hematology Research Collaborative (ASH RC) has built an SCD Data Hub (DH) to facilitate research and quality improvement using real-world data. To our knowledge, we report here the first analyses from the DH, focusing on cohort demographics and accurate identification of SCD diagnosis type. DH sites have data use agreements with ASH RC to extract and transfer electronic health record (EHR) data at least quarterly. Purpose
The purpose of this study is to determine the dosimetric impact on rectal dose by comparing two arc versus three arc VMAT plans on prostate patients who have received SpaceOAR.
Methods
A retrospective planning technique was performed on eight prostate cancer patients who had been previously treated with three-arc VMAT plans. For each plan, a two-arc plan was generated by removing the third arc and re-optimizing the plans using identical constraints and objectives. Introduction: Bidirectional deep brain stimulation (DBS) devices can continuously record neural activity while delivering stimulation. Due to memory limitations on the implanted device on which data are stored, frequent data downloads are required to prevent data loss. In this study, we present a novel collaborative data collection method utilizing a nationwide network of device representatives to allow frequent data transfer that reduces data loss without imposing travel burden on patients.

Objectives: The purpose of this study was to evaluate the diagnostic accuracy, imaging speed, radiation dose, and clinical utility of second- and third-generation dual source computed tomography scanners, in comparison with echocardiography and magnetic resonance imaging in diagnosing congenital heart disease in paediatric patients.
Material and Methods: A systematic literature search was conducted in PubMed, Embase, Scopus, and the Cochrane Central Register of Controlled Trials from 1 January 2006 to 30 April 2025. The clinical value of serial bone age (BA) determinations in children during growth is limited by the manual rater variability (precision 0.63 years). The objective of this work was to determine the precision of automated bone age and bone health index (BHI) measurements by BoneXpert and to establish the time interval at which the automated method can detect a significant treatment effect. The data were from a case-control trial (oxandrolone/placebo) following 90 boys with Klinefelter syndrome (KS) with five visits over 2 years, recording X-rays of both hands. BACKGROUND: Iron deficiency anemia in pregnancy is a significant health concern. Reticulocyte hemoglobin equivalent (Ret-He) and immature reticulocyte fraction (IRF) are emerging as early markers, yet their response to different iron therapies in pregnancy is underexplored.
OBJECTIVE: To determine whether intravenous single-dose iron administration produces a more favorable effect on reticulocyte indices than oral iron therapy in anemic pregnant women.

Background: Chemotherapy-induced nausea and vomiting is common in patients who receive highly emetogenic chemotherapy (HEC). Olanzapine acts on multiple neurotransmitters and effectively reduces delayed-phase nausea.
Aim: To compare the safety and efficacy of olanzapine-based vs neurokinin-1 receptor antagonist-based antiemetic regimens in adults receiving HEC.

Currently, there are 12 known species of Pediococcus, with P. Acidilactici and P. pentosaceus being the most well-known. P. pentosaceus is a Gram-positive, lactic acid bacterium, and is most often recognized for its wide usage as a probiotic and fermenter of wines and cheeses. It is also studied for its anti-inflammatory effects and antimicrobial properties against many common pathogens, such as Salmonella, Clostridium difficile, E.coli, etc. However, the Pediococcus species can be an opportunistic pathogen in immunocompromised patients, and its resistance to glycopeptides and tetracycline limits treatment options.

The aim of this study is to determine the efficacy and safety profile of total navicular implants based on current literature.
Navicular insults such as high-energy trauma, osteonecrosis, and Muller-Weiss syndrome may result in avascular necrosis or a critical sized bone defect, conditions that are highly difficult to manage given current treatment modalities. Total navicular implants may offer a solution to these problems. While 3D printing of implants remains investigational, its use in highly individualized cases involving the talus has had promising results with conditions like avascular necrosis, tumors, trauma, and arthritis. Introduction: Minimally invasive surgery (MIS) for bunion correction has gained popularity due to favorable outcomes and reduced recovery times. However, most MIS techniques rely on commercial instrumentation for osteotomy, reduction, and fixation. This retrospective study evaluates patients treated with a novel tri-planar distal metatarsal osteotomy performed without specialized MIS guides, using only standard surgical tools and a novel device.

RAB12 is a small GTPase and a validated substrate of LRRK2, a kinase genetically linked to Parkinson's disease (PD). While RAB12-LRRK2 signaling has been implicated in ciliogenesis and immune regulation, the neuronal function of RAB12 remains largely unexplored. Here, we investigated the role of RAB12 in synaptic physiology using Rab12 knockout (KO) mice. Rab12 KO mice developed normally but exhibited increased locomotor activity in adulthood. Electrophysiological recordings from striatal slices revealed enhanced presynaptic release probability and increased excitatory drive onto medium spiny neurons. Multifocal papillary thyroid cancer (PTC) can arise from independent primaries with discordant drivers in parallel clonal evolution rather than a single-clone pattern. We present a 31-year-old female with multifocal PTC harboring 3 distinct oncogenic alterations: a germline RET p.V804M mutation, low-frequency EML4-ALK fusion, and BRAF V600E mutation. The RET and ALK alterations were identified in a midpole nodule, whereas BRAF positivity was seen in a separate lower pole tumor. Ultrasound revealed multiple right-lobe thyroid nodules; the dominant 2.1-cm lesion was hypoechoic with calcifications. Increased oxygen levels at birth regulate myocyte bioenergetic and structural maturation controlled by mitochondrial cyclophilin D (CypD). We evaluated mechanisms of neonatal hypoxic cardiac dysfunction by exposing neonatal mice to 12% oxygen and studied cardiac bioenergetics, myocyte maturation, and function. Hypoxia decreased the activity/assembly of electron transport chain complex I, uncoupled oxidative phosphorylation, increased proliferation, decreased differentiation, increased ventricular mass, and decreased cardiac function. CypD inhibition rescued most hypoxia-mediated effects and increased cardiac function. In conclusion, neonatal hypoxia alters cardiac bioenergetics, myocyte maturation, and cardiac function through CypD-dependent pathways, providing potential therapeutic targets for neonatal cardiac dysfunction.

BACKGROUND: It is unclear whether the rate of intravenous fluid (IVF) administration in sepsis influences glycocalyx breakdown. This study aimed to measure the effect of rapid versus slow fluid bolus in a septic pig model on the integrity of the glycocalyx by measuring the serum levels of heparan sulfate and syndecan-1, breakdown products of the glycocalyx. Because endothelial glycocalyx is composed of apical cell membrane-bound proteoglycans such as syndecan-1, covalently bound with long linear glycosaminoglycans such as heparan sulfate, shedding of these different glycocalyx components is closely related. A high school soccer player experienced sudden cardiac arrest during competition and required immediate CPR and AED use. Following resuscitation, physicians ruled out commotio cordis and identified findings consistent with arrhythmogenic cardiomyopathy, supported by a prolonged T-wave on EKG while all other laboratory and catheterization results were normal. The athlete later received an ICD and pacemaker, prompting significant physical, emotional, and lifestyle adjustments. This capstone project examines the emergency response, the role of trained medical personnel at sporting events, and the implications of early cardiac screening for adolescent athletes. Low energy availability (LEA) is a clinically significant concern in adolescent female athletes due to its impact on growth and development, bone health, hormonal function, performance, recovery, and cognitive outcomes. Despite established associations, a clear longitudinal understanding of how LEA relates to injury and illness rates across an academic year remains limited, representing a clinical gap in athletic training and sports medicine literature. This case follows the rehabilitation of a 22-year-old collegiate lacrosse player recovering from a complex knee injury. During the initial evaluation, the athlete presented with pain, swelling, and limited active range of motion, secondary to pain. The rehabilitation protocol used a combination of ACL, MCL, and meniscus standardized rehabilitation parameters. The primary goal of rehabilitation was to safely return the athlete play the following season. Introduction: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are increasingly prescribed for glycemic control and weight reduction in patients with Type 2 Diabetes Mellitus (T2DM). Patients with T2DM exhibit elevated fracture risk despite preserved bone mineral density due to impaired bone quality and microarchitectural compromise. Whether GLP-1 RA therapy exacerbates or mitigates skeletal risk remains clinically relevant to podiatric care.

This study utilized Monte Carlo (MC) simulations to optimize radiation doses in pediatric multidetector computed tomography (MDCT) head scans by analyzing key parameters like tube current (mA), tube voltage (kV), pitch, and slice thickness. The findings indicate that reducing tube current significantly lowers the Computed Tomography Dose Index (CTDIvol) and Dose Length Product (DLP), effectively minimizing patient radiation exposure. Higher pitch values (0.7–0.9) further reduced radiation by decreasing beam overlap, while using a thinner slice thickness (0.6 mm) improved dose efficiency. Background: Pediatric patients with systemic lupus erythematosus (SLE) face elevated risk for invasive pneumococcal disease, yet fewer than 10% are estimated to be current on pneumococcal vaccine (PCV) recommendations.
Local Problem: This quality improvement (QI) project aimed to develop and implement a standardized pneumococcal immunization screening and counseling process in a pediatric rheumatology clinic.
Method: Three consecutive PDSA cycles guided implemention across a nine-week intervention period using a pre-post quasi-experimental design.
Interventions: A multicomponent strategy was employed, including a PCV decision algorithm, EHR Smart Phrase, PCP referral template, and patient education brochure.
