A Study Of The Clinical Features Of Renal Papillary Necrosis, Jose Martinez May 1978

A Study Of The Clinical Features Of Renal Papillary Necrosis, Jose Martinez

Yale Medicine Thesis Digital Library

Renal papillary necrosis is a disease entity that has been recognized since 1878. Today, one hundred years later, a considerable amount of knowledge has been gathered about its possible etiologies, associated conditions, pathological pic ture, clinical manifestations, radiological features and mode of treatment. Very little is known, however, about prognosis of this disease. The literature is deficient of prognostic studies that may help the clinician predict the course of patients with renal papillary necrosis. This study is aimed to provide some answers to questions in the area of natural history and clinical features of this disease.

Effects Of Cromolyn On Inflammatory Bowel Disease Of Childhood, Yvette Piovanetti May 1978

Effects Of Cromolyn On Inflammatory Bowel Disease Of Childhood, Yvette Piovanetti

Yale Medicine Thesis Digital Library

Methods and Materials: Twelve children with CIBD (7 with CUC and 5 with Crohn's) entered a trial with Cromolyn Sodium. The patient groups consisted mostly of those who had been treated with steroids over a 1-year period without suppression of disease activity, and in whom steroids could not be tapered without exacerbation. On commencement of Cromolyn therapy, steroid dosages ranged from 5-60 mg. q.d. (mean dose 30 mg. q.d.). Multiple complications of steroid therapy were observed in this group. Linear growth retardation was exhibited by 5 patients, 2 with Crohn's and 3 with CUC. One of the patients with CUC …

Studies On Uveitis Part Ii: Hypotheses With Case Reports, Philip C. Hessburg Mar 1978

Studies On Uveitis Part Ii: Hypotheses With Case Reports, Philip C. Hessburg

Henry Ford Hospital Medical Journal

A number of patients with and without uveitis are used to show that this study of aqueous microbiology suggests the following hypotheses: 1. Routine techniques currently used in most microbiology laboratories overlook classical and variant bacteria which might be isolated from clinical specimens with improved microbiological techniques. 2. Bacterial forms may be present in other ocular tissue when not present in the aqueous. 3. Polymicrobial infections probably occur rather frequently, especially in chronic or subacute inflammatory states. 4. Alterations in various host factors may subject some persons to a greater incidence of inflammatory episodes including uveitis. 5. Antibiotics used in …

Effect Of Perfusate Osmolarity On Hearts Preserved Under Hypothermic Pulsatile Perfusion For 24 Hours, Luis H. Toledo-Pereya, Manuel Chee, Richard C. Lillehei Mar 1978

Effect Of Perfusate Osmolarity On Hearts Preserved Under Hypothermic Pulsatile Perfusion For 24 Hours, Luis H. Toledo-Pereya, Manuel Chee, Richard C. Lillehei

Henry Ford Hospital Medical Journal

Canine hearts preserved under hypothermic pulsatile perfusion (24 hours) with an osmolarity of 340 mOsm/L had better preservation and transplantation response than hearts perfused at an osmolarity of 310 and 270 mOsm/L. The hearts perfused at 340 mOsm /L survived an average of 22.6 days after transplantation. This survival was significantly (p>0.05) better than that observed after fresh transplantation (13.4 days average). Hearts perfused at 270 mOsm /L had a significantly (p>0.05) lower survival after transplantation (5.8 days average) than the fresh allografts or the hearts perfused at 310 or 340 mOsm / L. Similar survival times were …

This report describes the case of a 56-year-old man who was seen at Henry Ford Hospital with an abdominal mass confirmed as ganglioneuroma on histopathology. Experience at Henry Ford Hospital and the available literature on the subject are reviewed.

The Quantitative Determination Of 3-Oxo-Bile Acids, W. T. Beher, G. J. Lin, J. Sanfield, S. Stradnieks Mar 1978

The Quantitative Determination Of 3-Oxo-Bile Acids, W. T. Beher, G. J. Lin, J. Sanfield, S. Stradnieks

Henry Ford Hospital Medical Journal

A new method for the rapid quantification of 3-oxo-bile acids has been developed. From 5 to 120 μg of oxo-acid in aqueous or ethanolic media are reduced with sodium borohydride. The resulting 3α- and 3β-hydroxy acids are then oxidized in a reaction catalyzed by 3-hydroxysteroid dehydrogenase, during which nicotinamide adenine dinucleotide (NAD) is reduced to NADH. The absorbance of the reaction mixture is determined at 340 nm and is directly proportional to the amount of 3-oxo-acid in the original sample. The method is fully compatible with that used in determining 3α-hydroxy-bile acids.

Pathology Publications

Abstract

Industrial workers exposed to the organochlorine pesticide, chlordecone (Kepone), had signs of toxicity in several organs. The extent of toxicity was proportional to the levels of this chemical in the tissues. In 22 patients, chlordecone was eliminated slowly from blood (half time of 165±27 days — mean ± S.E.M.) and fat (half time of 125 days, with a range of 97 to 177), chiefly in the stool. Output of chlordecone in bile was 10 to 20 times greater than in stool, suggesting that chlordecone is reabsorbed in the intestine. Cholestyramine, an anion-exchange resin that binds chlordecone, increased its fecal …

Studies On Uveitis Part I: Aqueous Studies, Philip C. Hessburg Dec 1977

Studies On Uveitis Part I: Aqueous Studies, Philip C. Hessburg

Henry Ford Hospital Medical Journal

Aqueous microbiological and cytological studies were performed on 134 anterior chamber taps done on 118 patients both with and without uveitis. Cell wall defective bacterial forms (CWDF) were recovered from about 40% of these aqueous specimens and classical bacteria were recovered from 12%. A statistically significant difference could not be found between the incidence of bacterial cell wall defective forms in the various anatomical types of uveitis. There were hints from this work that the presence of CWDF In the aqueous was more frequent in younger patients with uveitis, early in the uveitis attack, if there were multiple foci of …

Effect Of Steroids On Patients With Postoperative Bowel Anastomotic Dysfunction, Luis H. Toledo-Pereya, Ignacio Guzman, Earl G. Yonehiro

Henry Ford Hospital Medical Journal

Ten patients who developed anastomotic edema following large or small intestinal anastomosis had a rapid recovery after treatment with dexamethasone. The clinical and radiologic picture of adynamic ileus disappeared rapidly. There were no Immediate or long-term (more than two years) complications or side effects associated with the use of dexamethasone. This preliminary report indicates that dexamethasone may be a beneficial agent In the control of anastomotic edema.

Legionnaires' Disease: Description Of A Sporadic Case, Louis D. Saravolatz, Gene Tilchen

Henry Ford Hospital Medical Journal

This first case of Legionnaires' disease seen at Henry Ford Hospital is described. A previously healthy 72-year-old man developed progressive pneumonia in September, 1977; it was unresponsive to treatment with cephalothin. However, he rapidly recovered when the treatment was changed to oral erythromycin. Diagnosis was established by serologic studies done at the Center for Disease Control. This pneumonia occurred in a sporadic form with no evidence of secondary cases.

Long-Term Survival After Onset Of Blast Crisis In Chronic Granulocytic Leukemia: Case Report And Therapeutic Considerations, Ellis J. Van Slyck, Ahmad Samhouri Dec 1977

Long-Term Survival After Onset Of Blast Crisis In Chronic Granulocytic Leukemia: Case Report And Therapeutic Considerations, Ellis J. Van Slyck, Ahmad Samhouri

Henry Ford Hospital Medical Journal

Current analysis indicates a median survival of 3.6 months in 356 cases of chronic granulocytic leukemia after onset of blast transformation. Thirty-five (10.0%) complete remissions (CR), all of short duration, were observed. Closer scrutiny of the reported clinical experience offers clues for improving this poor outlook. A 76% CR rate can be extracted from this total experience by identifying cases treated with cytoslne arabinoside combined with one or more other agents. In isolated reports, weekly vincristine and prednisone have yielded good remission rates. Two general morphologic types of blasts can be distinguished by cytochemical stains and other features: lymphoid and …

Primary Hepatic Tumors In Women On Oral Contraceptives, V. Maciel, H. Allen, C. Grodsinsky

Henry Ford Hospital Medical Journal

Liver cell adenoma and focal nodular hyperplasia have been recognized as rare benign tumors. Their relationship with the use of oral contraceptives has been suggested since 1973. At Henry Ford Hospital, we have had four patients, ages 19, 23, 25,and 30, on oral contraceptives who presented with different clinical pictures suggesting acute abdomen. A hepatic tumor was suspected preoperatively in one patient, was found at laparotomy for biliary tract disease in two other cases, while the fourth patient presented with acute intra-abdominal hemorrhage. Hepatic resection was performed successfully in all with no mortality. The clinical presentation and diagnostic and surgical …
