Small bowel adenocarcinoma is a rare but well-known complication of Crohn's disease. The diagnosis of small bowel adenocarcinoma remains difficult since its presentation is highly variable and mimics active or obstructive Crohn's disease. The diagnosis is often delayed and typically detected only at surgery in an advanced stage with a poor prognosis. We report a case of metastatic ileal adenocarcinoma in a patient with Crohn's disease with prolonged survival. Our case describes serial promising treatment options of these advanced malignancies and raises a possible role for checkpoint immunotherapy.

People living with food-related illnesses find themselves subjugated by commonly held ideologies causing awkwardness in social situations. The current study is a qualitative analysis addressing how people with celiac disease (CD) navigate social situations in light of dominant beliefs that influence behaviors. Initially, I identify macro-level patriarchal, religious, sexist, ableist and etiquette-related commensality ideologies that disadvantage those with CD. Drawing from the communication narrative sense making (CNSM) theory that supports storytelling and memorable messages as a sense-making tool for individuals diagnosed with chronic illness and their family members, this work highlights retrospective stories and memorable messages from 20 randomly selected …

Background
China has the largest number of cases of diabetes with a high rate of uncontrolled blood sugar. Many studies show that family members’ involvement is related to better glycemic control. However, there is a significant problem with low participation of family members. The widespread use of WeChat provides an opportunity for family members to support their diabetic loved ones in their type 2 diabetes self-management practices. The main aim of this study is to examine the effectiveness of a family-based intervention via WeChat.
Methods
A parallel, two-group, randomized controlled trial will be conducted in the central urban area of …

Blood-based biomarker discovery for psychotic disorders has yet to impact upon routine clinical practice. In physical disorders antibodies have established roles as diagnostic, prognostic and predictive (theranostic) biomarkers, particularly in disorders thought to have a substantial autoimmune or infective aetiology. Two approaches to antibody biomarker identification are distinguished: a "top-down" approach, in which antibodies to specific antigens are sought based on the known function of the antigen and its putative role in the disorder, and emerging "bottom-up" or "omics" approaches that are agnostic as to the significance of any one antigen, using high-throughput arrays to identify distinctive components of the …

Type II diabetes mellitus (T2DM) is a prevalent disease in our country. Bullard et al. (2018) report approximately 21 million adults have T2DM in the United States. “The total estimated cost of diagnosed diabetes in 2017 is $327 billion, including $237 billion in direct medical costs and $90 billion in reduced productivity” (Yang et al., 2018). Treatment of T2DM is individualized to each patient based on their co-morbidities, fiscal responsibility, and route of administration options. Sulfonylureas and Glucagon-like Peptide-1s (GLP-1) are two classes of antidiabetic drugs that are available for use as second line treatment options after metformin. This review …

Physician Assistant Scholarly Project Posters
• Obesity is a rising epidemic, and is one of the most common, costly and preventable health problems the world is experiencing today (Center for Disease Control and Prevention, [CDC], 2017).
• From this, it is important to establish an effective treatment plan that can be implemented across the nation for maximum patient benefit.
• Lifestyle changes such as diet and exercise are important to any health care plan and are equally necessary to reduce health risks. However, in certain patients, lifestyle changes are not enough due to the maladaptive biologic process in their brain. Thus, additional treatment options need …

Journal Articles: Pulmonary & Critical Care Med
BACKGROUND: Cachexia is associated with increased mortality risk among chronic obstructive pulmonary disease (COPD) patients. However, low body mass index (BMI) as opposed to cachexia is often used, particularly when calculating the BODE (BMI, Obstruction, Dyspnea and Exercise) index. For this reason, we examined mortality using a consensus definition and a weight-loss definition of cachexia among COPD cases and compared two new COPD severity indices with BODE.
METHODS: In the current report, the consensus definition for cachexia incorporated weight-loss > 5% in 12-months or low BMI in addition to 3/5 of decreased muscle strength, fatigue, anorexia, low FFMI and inflammation. The …

It has been examined that poor access to nutritious foods and limited opportunities for physical activity is prevalent among children from underserved families. These deficiencies contribute to increasing childhood obesity rates and emphasize the need for early interventions. Establishing nutritional literacy and understanding the importance of an active lifestyle in early childhood can lead to long-term health benefits and reduction in chronic disease later in life (HHS & USDA, 2015; Tsai et al., 2011; Wang et al., 2011). The InSports Foundation serves as a comprehensive non-profit organization that addresses many of these issues by providing children from underserved families with …
Opioid Agonist Therapy And Detoxification: Improving Outcomes For Fetuses Born To Addicted Mothers, Kao Lee
Opioid Agonist Therapy And Detoxification: Improving Outcomes For Fetuses Born To Addicted Mothers, Kao Lee
Theses and Graduate Projects
The use of opioids has increased significantly in the last decade. Concern for the undertreatment of pain emerged in the mid-1990s, prompting providers to treat pain as a fifth vital sign. The pharmaceutical industry marketed their products as having significantly less abuse potential than reality. Prescriptions for pain medications soared with prescription opioids leading the way. Prescription opioids became ubiquitous in medicine cabinets and reports of abuse rose. Casually administered, illicit sale, theft, and doctor shopping became more common as reports of overdose also rose. By 2010, the United States government was issuing strategies to combat the opioid crisis.

An Evaluation Of Home-Based Primary Care (Hbpc): Impact On Cost And Outcomes And Patient Satisfaction In The United States And Abroad, Isaac Pierre
Theses and Graduate Projects
Over the past century, the role of medical home visits in patient care has undergone significant changes. Historically, physicians routinely delivered medical care to sick patients in their own homes. In 1930, for example, such calls represented 40% of physician-patient encounters. As healthcare providers developed an increasing reliance on technology and as payment models began to prioritize volumes and efficiency, the provision of health care shifted to hospitals and office-based settings. An analysis of house calls in 1972 found that the number of in-home visits amounted to a mere 5% of physician-patient encounters and these rates continued to decrease to …

Maternal Health Interventions In Rural, Resource-Limited Countries, Katherine Tilton
Theses and Graduate Projects
Pregnant women are continuing to die at unacceptable rates around the globe. According to the World Health Organization, complications during pregnancy and childbirth are the leading cause of death and disability among reproductive age women in developing countries. Maternal mortality is often a reflection of a combination of causes: medical conditions while giving birth (ie. hemorrhage, eclampsia, and sepsis); other pre-existing medical conditions (ie. malaria, diabetes, HIV/AIDS); and socio-economic factors (ie. access to contraception, religious beliefs, and political agendas) that when combined, largely compromise health care, most often in the poor, rural and marginalized communities. Nearly all maternal deaths (99%) …

This Phase II, multicenter, open-label, single-arm trial was conducted in nine patients enrolled between 12/2014 and 12/2015. Subjects were treated with high-dose IPI 10 mg/kg intravenous (IV) every 3 weeks for four doses starting at week 1 and high-dose IL-2 (600,000 IU/kg IV bolus every …
Using The Ketogenic Diet As An Adjuvant To Cancer Therapy: A Systematic Review, Melanie L. Risola
Using The Ketogenic Diet As An Adjuvant To Cancer Therapy: A Systematic Review, Melanie L. Risola
Honors Undergraduate Theses
Cancer is the second leading cause of death in the United States. Evidence shows that a conventional western diet may contribute to the proliferation of cancer cells, affecting their prognosis. The aim of this review is to examine the efficacy and safety of using the ketogenic diet as an adjuvant to traditional cancer therapy. The systematic literature search was performed in October 2018 on two search engines: EBSCOhost (Medline, CINHAL, Cochrane Central Register of Controlled Trials) and Web of Science using the following key terms: ketogenic diet, high fat & low-carbohydrate diet, Atkins diet, cancer or neoplasms+. The search limitations …

Effects Of A Plant-Based Vegan Diet On The Risk Of Cancer: An Integrative Review Of The Literature, Chelsie M. Silavent
Honors Undergraduate Theses
Cancer, an aggressive chronic disease, impacts the lives of millions of people every day. There are numerous proposed triggers related to the diagnosis and much remains to be learned about the causes. Associated with this disease's variability is the challenge to identify a single causative agent that lead to its prevention. Specific topics that need additional evidence relate to environmental factors and lifestyle behaviors in the development, treatment and, in some instances, even suppression of disease progression. Specifically, a diet that excludes animal-based products but consists of a variety of fruits and vegetables (i.e., plant-based vegan diet) is reported to …

Master of Medicine in Anaesthesiology
Background: Propofol is widely used for the induction and maintenance of general anaesthesia and offers many key attractive pharmacological qualities that make it suitable for these indications. However, pain on injection is one of its major drawbacks and can be very distressing to patients. There is a paucity of studies that have looked at the effect of lornoxicam on propofol injection pain either as a sole intervention or in combination with any other method. Primary objective: To determine whether premedication with intravenous lornoxicam had any effect on the intensity of propofol injection pain at induction of general anaesthesia in adult …

The last decade has seen a sharp increase in the number of scientific publications describing physiological and pathological functions of extracellular vesicles (EVs), a collective term covering various subtypes of cell-released, membranous structures, called exosomes, microvesicles, microparticles, ectosomes, oncosomes, apoptotic bodies, and many other names. However, specific issues arise when working with these entities, whose size and amount often make them difficult to obtain as relatively pure preparations, and to characterize properly. The International Society for Extracellular Vesicles (ISEV) proposed Minimal Information for Studies of Extracellular Vesicles (“MISEV”) guidelines for the field in 2014. We now update these “MISEV2014” guidelines …

Purpose: Nonadherence to indicated therapy reduces treatment effectiveness and may increase cost of care. HUMIRA Complete, a Patient Support Program (PSP), aims to reduce nonadherence in patients prescribed adalimumab (ADA). The objective of this study was to assess the relationship between participation in the PSP and prescription abandonment rates among ADA-treated patients.
Patients and methods: This longitudinal study using patient-level data from AbbVie's PSP linked with medical and pharmacy claims data included patients ≥18 years with an ADA-approved indication, ≥1 pharmacy claim for ADA, and available data ≥3 months before and ≥6 months after the index date (defined as the …

Buried bumper syndrome is a rare but potentially severe complication of percutaneous endoscopic gastrostomy tube insertion. Though this complication is uncommon, it may lead to pressure necrosis, bleeding, perforation, peritonitis, sepsis, or death. Each case of buried bumper syndrome is unique in terms of patient comorbidities and anatomic positioning of the buried bumper. For this reason, many approaches have been described in the management of buried bumper syndrome. In this case report, we describe the case of an adolescent Caucasian female who developed buried bumper syndrome three years after undergoing percutaneous endoscopic gastrostomy insertion. We review diagnosis and management of …
