The Phenomenology Of Cushing's Syndrome: One Patient's Account, Andrew Armstrong, J. David Fachnie
Henry Ford Hospital Medical Journal
Cushing's syndrome caused by ectopic secretion of adrenocorticotropic hormone (ACTH) is often a serious disease and a diagnostic dilemma. In the reported patient, the source of ACTH proved to he a benign pulmonary carcinoid tumor. The patient describes his trying experiences through the six months from initial diagnosis to definitive therapy.

Prophylatic Cord Care: Is It Necessary?, Karen Coyle
Prophylatic Cord Care: Is It Necessary?, Karen Coyle
Theses : Honours
Since the evolution of 'open' cord care in 1974, the umbilical cord of the newborn has been treated prophylactically with many different forms of antibacterial solutions. Recent studies have demonstrated that cord separation is facilitated by bacteria and many of the current treatments, such as the use of alcohol, actually delay separation. The purpose of this study was to compare cleaning the cord with alcohol to dry cord care, a method of cord care which is now being considered as an acceptable alternative to prophylactic umbilical cord treatment. A convenience sample of 205 babies was taken from a small private …

Evaluation Of A Patient Education Program For Patients With Non-Insulin Dependent Diabetes (Type Ii), Helen M. Walker
Theses : Honours
The purpose of this study was to evaluate a group outpatient program for patients with non-insulin dependent diabetes (Type II). It endeavoured to establish whether at the completion of a patient education program, and again at the 3 month follow-up period, the patients were complying with : (a) the recommended dietary plan, (b) the exercise program, and (c) were monitoring blood glucose levels at home, as evidenced by weight loss and metabolic control of their diabetes. The study also investigated factors influencing non-compliance. The theoretical rationale used for this study was Pender's Health Promotion Model, which theorises that health-promoting behaviour …

Malignant External Otitis And Mastoiditis Associated With An Igg4 Subclass Deficiency In A Child., Ricardo Castro, Md, Norman Robinson, Md, Joel Klein, Md, William Geimeier, Md
Department of Pediatrics Faculty Papers
We have presented the first child in Delaware with malignant external otitis associated with IgG4 deficiency. Our patient needed three courses of intravenous antibiotics and twice required mastoidectomy, but has recovered completely following the restoration of the natural barrier between the internal and external ear, using a fascial graft from the large temporalis muscle. Some hearing deficit remains.

It has been assumed that endogenous synthesis by the platelet precursor cell, the bone marrow megakaryocyte, is the major source of platelet a-granule protein. To test this hypothesis, we used mRNA phenotyping to detect in megakaryocytes the presence of mRNA transcripts specific for various proteins. Our results indicate that megakaryocytes synthesize platelet factor 4, a protein relatively specific for platelets, but do not express mRNA transcripts for the fibrinogen, albumin, or IgG found in a-granules. We have previously shown that mega- karyocytes endocytose circulating proteins, including fibrino- gen, albumin, and IgG, and incorporate them into a-granules. Thus, platelets appear to …

Entry Of The Hearing Impaired Into The Health Care System, Kathleen Yaremchuk, Jonathan Schmidt, Linda Dickson
Entry Of The Hearing Impaired Into The Health Care System, Kathleen Yaremchuk, Jonathan Schmidt, Linda Dickson
Henry Ford Hospital Medical Journal
The current system of referral of patients complaining of hearing loss usually requires a dual appointment with otolaryngology and audiology. Most of these patients have sensorineural hearing loss for which there is rarely surgical or medical treatment. This study tests the hypothesis that these patients would receive appropriate treatment and that health care dollars would be saved if an audiological assessment could identify those patients who require medical or surgical care. One hundred consecutive patients whose sole complaint was hearing loss were evaluated using audiograms and "hearing abilities questionnaires" by five audiologists using subjective and objective criteria to determine the …

Management Of Voice Disorders, Alex F. Johnson, Barbara H. Jacobson, Michael S. Benninger
Henry Ford Hospital Medical Journal
Voice disorders are commonly seen in general medical practice. In some cases voice disorders represent the presenting symptom for serious underlying disease. It is important for clinicians from internal medicine, pediatrics, and family practice to be able to identify those factors in the history or observed vocal symptoms which suggest need for referral for comprehensive voice evaluation as well as to understand the distinct but complementary roles of the specific disciplines (otolaryngology and speech-language pathology) involved in diagnosis and treatment of patients with voice disorders.

Gallstones are usually silent. Less commonly, patients with cholelithiasis develop symptoms and/or complications; biliary fistula occurs in 3% to 5% of the cases. When a large stone is passed and occludes the duodenum, gastric outlet obstruction (the Bouveret syndrome) may result. In reported cases, the stones are usually larger than 2.5 cm. The usual presenting symptoms are those of bowel obstruction: abdominal pain, nausea, and vomiting. Less commonly, the patients experience melena and, rarely, hematemesis. We describe a patient who had the largest stone reported to cause hematemesis rather than bowel obstruction and to be diagnosed endoscopically. The 5 X …

Surgical Removal Of Metastatic Renal Adenocarcinoma To The Midbrain Tectum: A Case Report, Frank J. Tomecek, James I. Ausman, Ghaus M. Malik
Henry Ford Hospital Medical Journal
A patient with adenocarcinoma of the kidney metastatic lo the quadrigeminal plate of the midbrain presented with acute obstructive hydrocephalus and early tonsillar herniation. Because the majority of these carcinomas are resistant to radiation therapy, only limited treatment choices were available. Initially, a ventriculoperitoneal shunt relieved the hydrocephalus and neurologic symptoms. After a short course of improvement, with decompressed ventricles demonstrated by postoperative computed tomography, the patient developed additional neurologic signs, leading to the decision to excise the metastatic tumor. Convalescence was complicated but the patient survived for six months, succumbing to respiratory failure presumably caused by lung metastases.

Rubella Susceptibility And Continuing Risk Of Infection In Pregnancy, Saadiya Rasul, Mohammad Khurshid, Javaid Rizvi, Shahbanu Rizvi
Section of Haematology/Oncology
Sera of 2000 pregnant women attending the antenatal clinic of the Aga Khan University Hospital were tested for rubella antibodies. Of these 1684(84.2%) were Immune and 316(15.8%) were susceptible to rubella. Majority (99.8%) of the women were Asian in origin. Susceptibility decreased with increasing age and parity. We conclude that apart from vaccination of all young children greater attention should be paid to immunization of women of child-bearing age
Audiological In-Service Regarding Hearing Impairment And Its Impact On Communication In The Geriatric Population, Marie Barlow Lassell
Audiological In-Service Regarding Hearing Impairment And Its Impact On Communication In The Geriatric Population, Marie Barlow Lassell
Dissertations and Theses
At least two studies have been found conducted in the area of audiological in-services and their impact on the people to whom they were presented. In 1981, Dancer and Keiser studied the effects of empathy training on geriatric-care nurses and in 1985, Dampier, Dancer, and Keiser studied changing attitudes of college students toward older persons with hearing loss. Both of the studies investigated the effect of in-service training on empathy, rather than using the traditional academicallyoriented in-service program approach. Both studies found statistical results indicating a significant positive change in the subjects' feelings toward hearing impaired geriatrics. Additionally, Dancer et …
Closing In On The Men2a Locus, Nancy E. Simpson, Kenneth K. Kidd
Closing In On The Men2a Locus, Nancy E. Simpson, Kenneth K. Kidd
Henry Ford Hospital Medical Journal
The mapping of the locus for multiple endocrine neoplasia type 2A (MEN 2A) to chromosome 10 using linkage is briefly reviewed including a discussion of linkage strategy and reference to some of the exclusions before the assignment. The subsequent development of the map of the centromeric region of the chromosome and the linking of what appear to be the four closest flanking markers and the centromeric alphoid sequence to the disease locus are reviewed. To date no recombination has been observed between the centromeric marker and the MEN2A locus among, at least, 26 informative meioses, 11 of which are phase …

Hereditary Medullary Thyroid Carcinoma: Genetic Analysis Of Three Related Syndromes, Hagay Sobol, Steven A. Narod, Isabelle Schuffenecker, Chris Amos, R. Alan B. Ezekowitz, Gilbert M. Lenoir
Henry Ford Hospital Medical Journal
Hereditary medullary thyroid carcinoma (MTC) appears in three forms: 1) in association with pheochromocytomas and parathyroid hyperplasia (multiple endocrine neoplasia type 2 A [MEN 2A]); 2) with pheochromocytomas, neuromas of the mucous membranes, and a marfanoid appearance (MEN 2B): and 3) without pheochromocytoma. Despite these differences in presentation, age of onset, and clinical severity, limited genetic studies suggest that the three MTC variants may be due to inherited mutations at the same gene locus. We present further data in support of the hypothesis that allelic variation may underlie the diversity of these endocrine neoplasia syndromes.

Henry Ford Hospital Medical Journal
Multiple endocrine neoplasia type 2A (MEN 2A) is a rare hereditary disease transmitted in families as an autosomal dominant trad. We have identified a family in which the expression of a rare autosomal dominant form of cutaneous lichen amyloidosis appears to cosegregate with MEN 2A. In this family the skin lesion presented as multiple infiltrated papules overlying well demarcated plaques over the scapular area (right or left). Immunohistochemical studies demonstrated amyloid which stained for keratin but not calcitonin. A total of 19 members were screened. Three members of the family have the characteristic skin lesion and MEN 2A; two additional …
