Background/aim: Peritoneal dialysis (PD) is generally considered the practical dialysis modality for neonates in the treatment of acute kidney injury (AKI) and metabolic disturbances. e aim of this study was to evaluate the indications, complications, and outcomes of PD between January 2011 and December 2016. Materials and methods: This study included all 56 neonates that underwent PD over six years in our neonatal intensive care unit (NICU). Background/aim: This case-control study aims to assess the strength of associations between reproductive, lifestyle, sociodemographic, and dietary factors as well as other potential breast cancer risks and breast cancer (BC) in a North Cyprus population. Background/aim: Nesfatin-1, an anorexigenic molecule, seems to play a role in appetite regulation and energy homeostasis. e goal of this study was to evaluate the relation of nesfatin-1 with anthropometric and metabolic (ghrelin, leptin) parameters in children with idiopathic chronic malnutrition. Materials and methods: The study included 37 underweight and 38 healthy children who were similar regarding age, sex, and pubertal status. Background/aim: Studies on the epidemiology of rectal bleeding in children are limited in Iran. Our aim was to assess etiologies of rectal bleeding in children in Iran. Materials and methods: We enrolled 730 children with rectal bleeding. All the patients underwent colonoscopy, and 457 were further evaluated with histopathology. Results: According to colonoscopy and histopathology, respectively, in ammatory bowel disease (IBD) (29.4%, 15.8%), nodular hyperplasia (NH) (24.9%, 10%), and juvenile polyposis (JP) (12.6%, 9.9%) were the most common causes of rectal bleeding. Background/aim: Chronic kidney disease (CKD), which is one of the most important health problems worldwide, could be considered as an immune inflammatory disease. A prognostic biomarker may be helpful in determining the progression of CKD in children. We aimed to investigate the serum vasohibin-1 and soluble suppression of tumorigenicity-2 (sST2) levels as potential biomarkers in children with predialysis CKD. Background/aim: Mental disorders may accompany obesity. is study aims to evaluate the association between social anxiety disorder (SAD) and obesity and the risk factors for SAD in obese female patients. Materials and methods: A total of 114 obese patients and 110 healthy controls were included. The Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI), State and Trait Anxiety Inventory (STAI I-II), and Liebowitz Social Anxiety Scale (LSAS) were administered to assess anxiety, depression, and social anxiety levels. Mast cells are classically recognized as the effector cells of Immunoglobulin E (IgE)-mediated hypersensitivity reactions (i.e. allergic reactions). Mast cells also play an important role in the innate immune response to parasitic helminth infection. Allergic disease including allergic rhinitis, asthma, atopic dermatitis, and anaphylaxis, is a major health concern in the United States with greater than 60 million Americans suffering from allergy and asthma. The gut microbiome is intimately related to human health, but it is not yet known which functional activities are driven by specific microorganisms' ecological configurations or transcription. We report a large-scale investigation of 372 human fecal metatranscriptomes and 929 metagenomes from a subset of 308 men in the Health Professionals Follow-Up Study. We identified a metatranscriptomic 'core' universally transcribed over time and across participants, often by different microorganisms. In contrast to the housekeeping functions enriched in this core, a 'variable' metatranscriptome included specialized pathways that were differentially expressed both across participants and among microorganisms. Background: The amino acid taurine (2-Aminoethanesulfonic acid) modulates inhibitory neurotransmitter receptors. This study aimed to determine if the dual action of taurine on GABAC- ρ1R relates to its structure. To address this, we tested the ability of the structurally related compounds homotaurine, hypotaurine, and isethionic acid to modulate GABAC- ρ1R.

Results: In Xenopus laevis oocytes, hypotaurine and homotaurine partially activate heterologously expressed GABAC- ρ1R, showing an increment in its deactivation time with no changes in channel permeability, whereas isethionic acid showed no effect. Background: Stevens-Johnson syndrome (SJS) usually causes ocular surface disease. 35% of these patients develop chronic ocular sequelae including symblepharon, limbal stem cell deficiency, etc. The sequelae from SJS may also affect the alteration of ocular microbiome. Subjects: 20 chronic SJS patients and 20 healthy subjects were enrolled for specimen collection from inferior conjunctiva for microbiome analysis by using conventional culture and next-generation sequencing(NGS) methods Methods: Prospective, age and sex matched analytical study Results: A significant higher proportion of culture-positive specimen was demonstrated in SJS group (SJS group 60%, healthy 10%, p-value=0.001). Purpose: The purpose of this study was to examine the relationship between self-reported sedentary time (ST) and the cumulative risk of preserved ejection fraction heart failure (HFpEF) and reduced ejection fraction heart failure (HFrEF) using a diverse cohort of U.S. adults 45-84 years of age.

Methods: Using data from the Multi-Ethnic Study of Atherosclerosis (MESA), we identified 6,814 subjects (52.9% female). All were free of baseline cardiovascular disease. The traditional CFTR functional defect subcategories (i.e. protein misfolding, gating, conductance, etc.) are sometimes inadequate for determining patients most likely to benefit from emerging modulator treatment. We employ the P67L variant as emblematic of ways a thorough biochemical analysis of a rare CF defect can guide new therapeutic approaches, help predict drug response, and facilitate clinical precision. Historically, P67L has been designated a class IV conductance abnormality (i.e., malformed ion pore). In contrast, our studies demonstrate P67L disrupts CFTR protein maturation, prompts misfolding, causes gating defects, and exhibits wild-type like conductance. In the two decades since MECP2 was identified as the causative gene in the majority of Rett Syndrome (RTT) cases, transgenic mouse models have played a critical role in our understanding of this X-linked neurodevelopmental disease. However, their exclusive use presents a limitation in translating findings from animal models to the clinic. Here, we characterized growth, anatomical, behavioral, and motor deficits in a novel zinc-finger nuclease murine RTT model from birth through adulthood. DNA replication is a highly conserved and tightly regulated process, with a variety of factors acting to ensure faithful duplication and stability of chromosomal DNA. During replication, positive supercoils form ahead of the advancing replication fork. DNA topoisomerase 1 (Top1) acts to prevent DNA damage by relieving the torsional strain created by these supercoils. The camptothecin (CPT) class of chemotherapeutics target Top1, creating a long-lived DNA-Top1-CPT covalent complex, which leads to S-phase specific DNA lesions and cellular death. Background: The timing of initiation of renal replacement therapy (RRT) in severe acute kidney injury (AKI) remains controversial, with early initiation resulting in unnecessary therapy for some patients while expectant therapy may delay RRT for other patients. The furosemide stress test (FST) has been shown to predict the need for RRT and therefore could be used to exclude low-risk patients from enrollment in trials of RRT timing. การแก้ปัญหาเรื่องน้ำหนักเกินในประชากรถือเป็นความท้าทายที่สำคัญด้านสาธารณสุข โดยมีความสนใจที่จะนำสื่อสังคมออนไลน์ เช่น Facebook มาใช้ในการทำกิจกรรมเพื่อส่งผลให้เกิดการปรับเปลี่ยนพฤติกรรมและทำให้น้ำหนักตัวลดลง วัตถุประสงค์แรกของการศึกษานี้คือการทบทวนวรรณกรรมอย่างเป็นระบบเพื่อศึกษาประสิทธิผลของการประยุกต์ใช้กิจกรรมการลดน้ำหนักในผู้ที่มีภาวะน้ำหนักเกินและโรคอ้วนผ่านสื่อสังคมออนไลน์ที่ผ่านมา โดยการสืบค้นงานวิจัยจากฐานข้อมูล PubMed Scopus และ Cochrane Library โดยสืบค้นในช่วงปี พ.ศ. 2548-2557 เลือกงานวิจัยการทดลองแบบสุ่มที่ใช้ภาษาอังกฤษหรือไทย ภายใต้คำสำคัญที่ใช้สืบค้นเกี่ยวกับภาวะน้ำหนักเกินและการใช้สื่อสังคมออนไลน์พบว่ามีงานวิจัยที่เกี่ยวข้องจำนวน 436 บทความ และที่ผ่านเกณฑ์จำนวน 2 บทความ โดยการศึกษาดังกล่าวมีการใช้สื่อสังคมออนไลน์ คือ Facebook หรือ Twitter และพบความแตกต่างในผลการลดลงของน้ำหนักระหว่างกลุ่มทั้งที่ให้ผลอย่างมีนัยสำคัญทางสถิติและที่ไม่พบความแตกต่าง อันจะเห็นได้ว่างานวิจัยทางด้านนี้ยังมีปริมาณน้อยและผลในภาพรวมยังไม่ชัดเจน วัตถุประสงค์ที่สองของการศึกษานี้คือการทดลองเพื่อเปรียบเทียบผลของการประยุกต์ใช้กิจกรรมการลดน้ำหนักในผู้ที่มีภาวะน้ำหนักเกินในกลุ่มทดลองที่ทำกิจกรรมผ่านสื่อสังคมออนไลน์ Facebook เปรียบเทียบกับกลุ่มควบคุมที่ทำกิจกรรมแบบออฟไลน์และใช้หนังสือคู่มือเป็นสื่อ โดยใช้ระยะเวลาศึกษา 6 เดือน แบ่งเป็นช่วงทดลอง 4 เดือน และช่วงติดตามผล 2 เดือน กลุ่มตัวอย่างจำนวน 66 คนเป็นนิสิตระดับปริญญาตรีจากคณะต่าง ๆ ที่ถูกสุ่มเข้าสู่กลุ่มทดลองและกลุ่มควบคุม แต่ละกลุ่มมีอาสาสมัครจำนวน 33 คน ทั้งสองกลุ่มมีพื้นฐานกิจกรรมคล้ายกันคือการเรียนรู้จากหลักสูตรด้านโภชนาการและการออกกำลังกาย การรายงานน้ำหนักประจำวัน การได้รับคำปรึกษาจากผู้เชี่ยวชาญ และกิจกรรมการถามตอบคำถาม ส่วนที่แตกต่างคือการจัดกิจกรรมแบบออนไลน์ผ่าน Facebook หรือแบบออฟไลน์โดยใช้หนังสือคู่มือและติดต่อผ่านทางเอกสาร ข้อมูลพื้นฐานของทั้งสองกลุ่มก่อนเริ่มทดลองไม่แตกต่างกันยกเว้นรายได้เฉลี่ยต่อเดือน ผลการวิจัยพบว่ากลุ่มทดลองมีการร่วมกิจกรรมขอรับคำปรึกษามากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ มีการเพิ่มของคะแนนด้านความรู้อย่างมีนัยสำคัญทางสถิติเมื่อวัดตั้งแต่ก่อนทดลองจนถึงเดือนที่ 6 ทั้งสองกลุ่มมีความพึงพอใจเมื่อสิ้นสุดเดือนที่ 4 และ 6 ในระดับดีมาก และระดับ Body Mass Index (BMI) และ Weight-to-Height Ratio (WHtR) ณ เดือนที่ 4 ของกลุ่มทดลองลดลงมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ Mean diff (95% Cl) เท่ากับ 0.72 (0.13,1.30) และ 0.00758 (0.00157,0.01358) ตามลำดับ แต่การลดลงไม่แตกต่างกันอย่างมีนัยสำคัญเมื่อวัด ณ เดือนที่ 6 อันแสดงให้เห็นว่าการใช้สื่อสังคมออนไลน์ Facebook สามารถส่งผลต่อการลดลงของค่า BMI และ WHtR ในช่วงการทดลองแต่ผลที่ได้อาจไม่ยั่งยืน …

ความสำคัญและที่มาของปัญหาการวิจัย: การฟอกเลือดวิธีฮีโมไดอะฟิวเตรชัน เป็นวิธีการฟอกเลือดที่ดีที่สุดในปัจจุบันของผู้ป่วยที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม โดยมีการศึกษาต่างๆ พบว่า การฟอกเลือดวิธีฮีโมไดอะฟิวเตรชันสามารถกำจัดสารยูรีมิก โดยเฉพาะอย่างยิ่งสารยูรีมิกที่จับกับโปรตีน เช่น อินดอกซิลซัลเฟต และสารยูรีมิกขนาดกลาง เช่น บีต้า 2 ไมโครโกลบูลิน ได้ดีกว่าการฟอกเลือดโดยใช้ตัวกรองมาตรฐาน ทำให้การฟอกเลือดวิธีฮีโมไดอะฟิวเตรชันมีหลักฐานแสดงให้เห็นว่าสามารถลดอัตราเสียชีวิตได้ อย่างไรก็ตามวิธีการฟอกเลือดนี้ไม่เป็นที่นิยมอย่างแพร่หลายในประเทศไทย และมีราคาแพงมากกว่าการฟอกเลือดด้วยตัวกรองมาตรฐาน จึงนำไปสู่การศึกษาปัจจุบันที่เปรียบเทียบประสิทธิภาพของการฟอกเลือดด้วยตัวกรองใหม่รูใหญ่พิเศษ (Novel super high-flux dialyzer, PES 17D alpha, Nipro, Japan) เปรียบเทียบกับการฟอกเลือดวิธีฮีโมไดอะฟิวเตรชัน ในแง่ของการกำจัดสารยูรีมิกที่จับกับโปรตีน และสารยูรีมิกขนาดกลาง โดยการศึกษานีเป็นการศึกษาแบบเทียบเคียง (non-inferiority trial) ระเบียบวิธีวิจัย: การศึกษานี้เป็นการศึกษาเปรียบเทียบกลุ่มไขว้แบบไปข้างหน้า รวบรวมผู้ป่วยที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมอยู่เดิมทั้งหมด 12 ราย แบ่งการรักษาออกเป็น 2 กลุ่ม โดยที่สุ่มผู้ป่วยกลุ่มหนึ่งฟอกเลือดด้วยตัวกรองรูใหญ่พิเศษ และอีกกลุ่มหนึ่งฟอกเลือดด้วยวิธีฮีโมไดอะฟิวเตรชัน แล้วจึงสลับการรักษา 12 สัปดาห์หลังการรักษา โดยผู้ป่วยมีระยะเวลาเข้าศึกษา (run-in) และ ระยะพัก (wash-out) ของการศึกษาเป็นระยะเวลา 4 สัปดาห์ โดยใช้การฟอกเลือดด้วยตัวกรองมาตรฐาน (high flux dialyzer) โดยกำหนดคำถามหลักของการวิจัยคือเปรียบเทียบอัตราการลดลงของอินดอกซิลซัลเฟตของทั้ง 2 การรักษา นอกจากนี้ยังเปรียบเทียบอัตราการลดลงของสารยูรีมิกขนาดกลาง และสารยูรีมิกขนาดเล็ก รวมไปถึงการสูญเสียอัลบูมินทางน้ำยาไตเทียม และตัวชี้วัดทางโภชนาการ โดยการศึกษานี้กำหนดให้จุดตัดของอัตราการลดลงของอินดอกซิลซัลเฟตไม่น้อยไปกว่าร้อยละ 5 จะถือว่าไม่มีความแตกต่างกันของการรักษาทั้ง 2 วิธี ผลการศึกษา: การฟอกเลือดด้วยตัวกรองรูใหญ่พิเศษมีประสิทธิภาพเทียบเคียงกับการฟอกเลือดวิธีฮีโมไดอะฟิวเตรชันในแง่อัตราการลดลงของอินดอกซิลซัลเฟต เป็นร้อยละ 50.9.8±25.03 และ 50.5±41.1 ตามลำดับ โดยมีค่าเฉลี่ยของความต่างร้อยละ 5.87 (ช่วงความเชื่อมั่นร้อยละ 95, -1.63, 13.37). สำหรับอัตราการลดลงของบีต้า 2 ไมโครโกลบูลิน และ อัลฟา 1 ไมโครโกลบูลิน ซึ่งเป็นสารยูรีมิกขนาดกลางไม่แตกต่างกันระหว่างการรักษาทั้ง 2 วิธี โดยมีค่าเฉลี่ยของความต่างร้อยละ 1.98 (ช่วงความเชื่อมั่นร้อยละ 95, -0.21, 4.18) …

การศึกษาผลของการจำกัดเกลือและน้ำต่อความเหนื่อยในผู้ป่วยที่มีภาวะหัวใจวายเฉียบพลัน, นิธินันท์ ฉิมพาลี

วัตถุประสงค์: เพื่อศึกษาเปรียบเทียบระดับความเหนื่อยของผู้ป่วยโดยใช้ visual analog scale (VAS) ในผู้ป่วยที่มีภาวะหัวใจวายเฉียบพลัน ซึ่งได้รับปริมาณเกลือและน้ำแตกต่างกัน วิธีการศึกษา: ผู้ป่วยที่มีภาวะหัวใจวายเฉียบพลันซึ่งเข้ารับการรักษาในโรงพยาบาลจะถูกสุ่มเพื่อได้รับการจำกัดเกลือและน้ำ ดังนี้ กลุ่ม A ได้รับอาหารที่มีเกลือ 1.5 กรัมและจำกัดน้ำดื่มไม่เกิน 1,000 มิลลิลิตรต่อวัน ขณะที่กลุ่ม B ได้รับอาหารที่มีเกลือ 3 กรัมและจำกัดน้ำดื่มไม่เกิน 2,000 มิลลิลิตรต่อวัน ทั้งนี้การรักษาอื่นเป็นไปตามแนวทางเวชปฏิบัติของการรักษาผู้ป่วยที่มีภาวะหัวใจวายเฉียบพลัน ผู้ป่วยจะได้รับการประเมิน ณ ชั่วโมงที่ 0, 24, 48 ของการศึกษา ผลการศึกษาหลักคือ การเปลี่ยนแปลงระดับความเหนื่อยของผู้ป่วยที่ 0 และ 48 ชั่วโมงของการศึกษา โดยวิธีประเมินด้วย VAS วิเคราะห์ผลการศึกษาใช้วิธี Mann-Whitney U test ผลการศึกษา: มีผู้ป่วยเข้าร่วมการศึกษาทั้งสิ้น 35 ราย โดยอายุเฉลี่ย 69.5 ± 15.7 ปีและเป็นเพศชายร้อยละ 45.7 ผลการศึกษาครั้งนี้ไม่พบความแตกต่างอย่างมีนัยสำคัญของผลการศึกษาหลัก อันได้แก่ การเปลี่ยนแปลงของระดับความเหนื่อยจาก VAS ระหว่างผู้ป่วยกลุ่ม A และ B (-2.5 (6.5) เซนติเมตร เปรียบเทียบกับ -0.7 (3.5) เซนติเมตร (median (interquartile range)) ตามลำดับ; p=0.68). นอกจากนี้ยังไม่พบความแตกต่างอย่างมีนัยสำคัญของผลการศึกษารอง ได้แก่ การเปลี่ยนแปลงของน้ำหนักตัวผู้ป่วย, NTproBNP, creatitine, ระดับโซเดียมในปัสสาวะ, ปริมาณการใช้ยาขับปัสสาวะ furosemide และระยะเวลาการเข้ารับการรักษาตัวในโรงพยาบาล ตลอดจนอัตราการเกิดภาวะล้มเหลวของการรักษา และภาวะ cardiorenal syndrome สรุปผลการศึกษา: การจำกัดเกลือและน้ำไม่มีผลต่อการรักษาผู้ป่วยที่มีภาวะหัวใจวายเฉียบพลัน

While by definition tumor cells are considered immortalized, they can undergo senescence when exposed to conventional and targeted cancer therapy. Interleukin-18 (IL-18) is a pro-inflammatory cytokine known to be involved in maintaining metabolic homeostasis; however, also capable of inducing cardiac dysfunction. Additionally, IL-18, has been shown to bind to a novel receptor, the Na-Cl Co-transporter (NCC). We hypothesized that NCC mediates IL-18 metabolic and cardiac signaling in mice. Using male C57BL/6J mice, we compared the metabolic and cardiac function changes after at least 8 weeks of high-saturated fat high sugar diet (Western Diet) in NCC knockout (NCCKO), IL-18 knockout (IL-18KO), and wild-type mice. Background/aim: This article analyzes the incidence and characteristics of peripheral neuropathy in patients with impaired glucose regulation (IGR). Materials and methods: A total of 120 IGR patients and 60 healthy controls were enrolled. All subjects underwent nerve conduction study (NCS) of large fibers and skin sympathetic response (SSR) and contact heat pain evoked potential (CHEP) testing of small fibers with a Medtronic Keypoint machine (Medoc Ltd., Israel). IGR patients were evaluated using the Michigan Neuropathy Screening Instrument (MNSI). Rett syndrome (RTT) is a rare neurodevelopmental disorder that primarily affects females. In 99% of cases, RTT is believed to occur sporadically, or de novo. However, in rare cases, RTT can be passed down from parent to child through gonadal mosaicism or asymptomatic carrier mothers. It is known that having a child with an inherited genetic condition can lead to changes in family planning; however, little research has investigated this phenomenon in sporadic genetic conditions, such as RTT.
