Schistosomiasis, also known as bilharzia, is the third most common parasitic infection worldwide and a major source of morbidity and mortality in developing countries. Caused by trematodes in the genus Schistosoma, this parasitic disease has no vaccine therapy and resources for drug development are scarce. Despite an unknown mechanism of action, praziquantal has become the primary source of treatment due to its efficacy against all species of Schistosoma. However, with increased drug resistance in endemic areas and low activity against immature parasites, relying on this single drug is unsustainable and risky. ความเป็นมาและความสำคัญของปัญหา: การเคลื่อนย้ายเพื่อการรักษาฉุกเฉินในอุตสาหกรรมน้ำมันและก๊าซธรรมชาตินอกชายฝั่งมีค่าใช้จ่ายสูงและมีความเสี่ยง การศึกษาในต่างประเทศพบว่าสาเหตุการเคลื่อนย้ายเพื่อการรักษาฉุกเฉินที่เกิดจากการเจ็บป่วยมีแนวโน้มเพิ่มขึ้น ในประเทศไทยยังไม่มีการศึกษาเกี่ยวกับสาเหตุและค่าใช้จ่ายของการเคลื่อนย้ายเพื่อการรักษาฉุกเฉินในอุตสาหกรรมน้ำมันและก๊าซธรรมชาตินอกชายฝั่ง วัตถุประสงค์ของการศึกษานี้คือเพื่อระบุสาเหตุและศึกษาเกี่ยวกับค่าใช้จ่ายของการเคลื่อนย้ายเพื่อการรักษาฉุกเฉินในอุตสาหกรรมน้ำมันและก๊าซธรรมชาติในอ่าวไทย วิธีดำเนินการวิจัย: การทบทวนข้อมูลย้อนหลังของการเคลื่อนย้ายเพื่อการรักษาฉุกเฉินในอุตสาหกรรมน้ำมันและก๊าซธรรมชาตินอกชายฝั่งทะเลจากฐานผลิต/ขุดเจาะน้ำมันและก๊าซธรรมชาตินอกชายฝั่งอ่าวไทย ตั้งแต่ปี พ.ศ. 2559 ถึง 2562 เป็นระยะเวลา 36 เดือน ผลการวิจัย: ในช่วงเวลาศึกษาวิจัยมีผู้ป่วยที่ถูกเคลื่อนย้ายจำนวนทั้งสิ้น 416 ราย สาเหตุของการเคลื่อนย้ายเพื่อการรักษาส่วนใหญ่ร้อยละ 84.13 เป็นการเจ็บป่วย พบว่าร้อยละ 60.1 ของการเคลื่อนย้ายทั้งหมดเป็นสาเหตุที่ป้องกันไม่ได้หรือป้องกันได้ยาก มีเพียงร้อยละ 39.9 เป็นสาเหตุที่ป้องกันได้ ค่าใช้จ่ายในการเคลื่อนย้ายเพื่อการรักษาอยู่ระหว่าง 10,000 บาท ถึง 880,000 บาทต่อราย ค่าใช้จ่ายในการเคลื่อนย้ายเพื่อการรักษาที่เกิดขึ้นจากสาเหตุที่ป้องกันได้เท่ากับ 17,160,000.00 บาทในช่วง 36 เดือนนั้น สรุปผลการวิจัย: การลดค่าใช้จ่ายจากการเคลื่อนย้ายเพื่อการรักษา สามารถทำได้โดยใช้วัคซีนเพื่อป้องกันการเกิดโรคที่ป้องกันได้ด้วยวัคซีน คัดกรองไม่ให้มีผู้ที่มีความเสี่ยงเกิดโรคแทรกซ้อนจากโรคประจำตัวหรือโรคที่มีอยู่ก่อนขึ้นไปทำงานบนแท่นขุดเจาะและฐานผลิตน้ำมันและก๊าซธรรมชาตินอกชายฝั่ง หรือเพิ่มขีดความสามารถให้สามารถรักษาผู้ป่วยเหล่านั้นได้ที่แท่นขุดเจาะและฐานผลิตน้ำมันและก๊าซธรรมชาตินอกชายฝั่ง อุตสาหกรรมน้ำมันและก๊าซธรรมชาตินอกชายฝั่งอาจพิจารณาค่าใช้จ่าย-ผลได้ของวิธีเหล่านี้เทียบกับสภาพปัจจุบัน

ความต้องการการแพทย์ทางเลือก (Complementary and Alternative Medicine: CAM) เพิ่มขึ้นทั่วโลกเช่นเดียวกับการแพทย์แผนไทย (Thai Traditional Medicine: TTM) ภายใต้แผนยุทธศาสตร์ระยะยาวต่าง ๆ ของกระทรวงสาธารณสุข แต่การส่งเสริมการใช้ TTM ถูกจำกัดโดยการแพทย์แผนปัจจุบันที่เป็นกระแสหลักของระบบสุขภาพในประเทศไทยมาอย่างยาวนาน ดังนั้นการยอมรับ TTM ของแพทย์แผนปัจจุบัน โดยเฉพาะอย่างยิ่งในแพทย์เฉพาะทางจึงเป็นปัจจัยสำคัญในการส่งเสริมการใช้ TTM แต่ที่ผ่านมาไม่เคยมีการประเมินอย่างเหมาะสมด้วยเครื่องมือสำรวจที่มีมาตรฐาน การศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาและทดสอบนำร่องด้วยแบบสอบถามมาตรฐานในการเข้าถึงการยอมรับและการใช้ TTM/CAM ของแพทย์เฉพาะทางในประเทศไทย โดยทำการวิจัยแบบผสมผสานเชิงสำรวจบุกเบิก ประกอบด้วยการวิเคราะห์อภิมาน การสัมภาษณ์เชิงลึก การพัฒนาและทดสอบแบบสอบถาม และการทดสอบนำร่องด้วยแบบสอบถามมาตรฐานในแพทย์เฉพาะทางภายใต้ 8 ราชวิทยาลัยในประเทศไทย ผลการวิเคราะห์อภิมานจากบทความวิชาการที่ตีพิมพ์ในปี พ.ศ. 2545 - 2560 จำนวน 1,924 เรื่อง มีบทความที่ผ่านเกณฑ์การคัดเลือกจำนวน 25 เรื่อง พบว่าความชุกของการยอมรับการแพทย์ทางเลือกของแพทย์เฉพาะทางด้วยโมเดลแบบสุ่มมีค่าเท่ากับร้อยละ 54 (95% CI: 36%-73%) และความชุกของการยอมรับการแพทย์ทางเลือกที่มีค่าสูงที่สุดคือแพทย์เวชศาสตร์ครอบครัว ตามด้วย จิตแพทย์และแพทย์ประสาทวิทยา ประสาทศัลยแพทย์ สูตินรีแพทย์ กุมารแพทย์ วิสัญญีแพทย์ แพทย์เวชศาสตร์ฟื้นฟู อายุรแพทย์ และศัลยแพทย์ แบบสอบถามการยอมรับและการใช้ TTM/CAM ของแพทย์เฉพาะทางที่พัฒนาขึ้นมีทั้งหมด 3 ตอน รวม 78 ข้อ มีความเที่ยงตรงเชิงเนื้อหา (IOC มีค่าตั้งแต่ 0.50 ขึ้นไป) และมีค่าความเชื่อมั่นของแบบสอบถามในระดับดีมาก (Cronbach’s Alpha เท่ากับ 0.96) ปัจจัยการยอมรับ TTM/CAM จากการวิเคราะห์องค์ประกอบเชิงสำรวจประกอบด้วย 3 ด้าน คือ ด้านกิจกรรม ด้านมุมมองเชิงบวก และด้านมุมมองเชิงลบ ซึ่งการวิเคราะห์กลุ่มแฝงแสดงให้เห็นว่าแพทย์เฉพาะทางสามารถจัดกลุ่มตามประเภทของผู้ยอมรับการแพทย์ทางเลือกจำนวน 4 กลุ่มคือ ผู้ปฏิบัติด้านการแพทย์ทางเลือก ผู้แนะนำการแพทย์ทางเลือก ผู้สนับสนุนการแพทย์ทางเลือก และผู้ไม่สนับสนุนการแพทย์ทางเลือก การทดสอบนำร่องในแพทย์เฉพาะทางจำนวน 243 คน จาก 8 ราชวิทยาลัยแสดงให้เห็นถึงรูปแบบต่าง ๆ …

วัตถุประสงค์ เพื่อศึกษาความชุกและปัจจัยของอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่าง (musculoskeletal discomfort : MSD) ในบุคลากรเภสัชกรรมในโรงพยาบาลจุฬาลงกรณ์ วิธีการศึกษา การศึกษาเชิงพรรณนาแบบภาคตัดขวาง ณ จุดเวลาใดเวลาหนึ่ง ในบุคลากรเภสัชกรรมในโรงพยาบาลจุฬาลงกรณ์ โดยมีจำนวนตัวอย่างมีทั้งสิ้น 197 คน การเก็บข้อมูลใช้แบบสอบถามที่ให้ผู้เข้าร่วมการศึกษากรอกด้วยตนเอง ซึ่งประกอบด้วยข้อมูลปัจจัยด้านบุคคล ปัจจัยด้านงาน และการประเมินอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่างโดยใช้แบบสอบถามนอร์ดิก ผลการศึกษา บุคลากรเภสัชกรรมมีความชุกของอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่างค่อนข้างสูง ซึ่งมีความชุกโดยรวมคือ ร้อยละ 83.8 และร้อยละ 93.4 สำหรับช่วง 7 วัน และ 12 เดือน ตามลำดับ ตำแหน่งที่มีความชุกมากที่สุดคือคอ และรองลงมาคือไหล่ กลุ่มของตำแหน่งที่มีความชุกของอาการมากที่สุดคือแนวแกนลำตัว เหตุผลที่มากที่สุดของความคิดว่าอาการปวดมาจากการทำงานคือการนั่งทำงานเป็นเวลานาน ปัจจัยที่สัมพันธ์เชิงลบกับโอกาสเป็น MSD โดยรวมในช่วง 7 วัน คือ การออกกำลังกาย และปัจจัยที่สัมพันธ์เชิงลบและเชิงบวกกับโอกาสเป็น MSD ของแนวแกนลำตัว คือ สัดส่วนของเวลาพักและระยะเวลาการทำงาน ตามลำดับ สรุป บุคลากรเภสัชกรรมในโรงพยาบาลจุฬาลงกรณ์มีความชุกของอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่างสูง จึงควรมีการเฝ้าระวังทางสุขภาพด้านอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่าง แนะนำให้สร้างเสริมสุขภาพโดยการออกกำลังกาย พิจารณาปรับเพิ่มสัดส่วนของช่วงเวลาพักหรือลดระยะเวลาการทำงาน รวมถึงการปรับเปลี่ยนอิริยาบถเป็นระยะเมื่อต้องนั่งนาน เพื่อป้องกันอาการผิดปกติของระบบกล้ามเนื้อและกระดูกโครงร่างในบุคลากรกลุ่มนี้

โรคเบาหวาน (Diabetes mellitus, DM) เป็นภาวะผิดปกติที่ร่างกายมีระดับน้ำตาลในเลือดสูงกว่าปกติ จนนำไปสู่อาการแทรกซ้อนต่าง ๆ อาทิ โรคไตวายเรื้อรังระยะสุดท้าย และโรคไขมันพอกตับที่ไม่ได้เกิดจากแอลกอฮอล์ (Non-alcoholic fatty liver disease, NAFLD) เพื่อควบคุมระดับน้ำตาลในเลือด ผู้ป่วยโรคเบาหวานจะได้รับการแนะนำให้บริโภคอาหารในแต่ละมื้อลดลง และเพิ่มมื้ออาหารว่างเป็นอาหารที่มีค่าดัชนีอาหารต่ำ เป็นที่ทราบกันดีว่าพืชในตระกูลถั่วและงามีประโยชน์ต่อสุขภาพหลายประการ การศึกษาในพืชตระกูลถั่วพบว่าสามารถป้องกันภาวะไขมันในเลือดสูงรวมถึงบรรเทาภาวะโรคเบาหวานได้ และยังมีการศึกษาที่พบว่างา (Sesame) ช่วยลดคอเลสเตอรอล (Cholesterol) ในเลือด นอกจากนี้ถั่วเขียวทอดและงาคั่วเป็นหนึ่งในอาหารกินเล่นที่นิยมของคนไทย สมควรที่จะส่งเสริมผู้ป่วยโรคเบาหวานบริโภคถั่วเขียวทอดหรืองาคั่วเป็นอาหารว่าง หากแต่ในปัจจุบันยังไม่มีหลักฐานเพียงพอที่จะทำให้เชื่อมั่นได้ว่า ถั่วเขียวทอดและงาคั่ว สามารถลดความเสี่ยงในการเกิดภาวะแทรกซ้อนของโรคเบาหวานได้จริงหรือไม่ ในงานวิจัยครั้งนี้จึงมีจุดประสงค์ที่จะศึกษาผลของการบริโภคถั่วเขียวทอดและงาดำคั่วต่อการดำเนินโรคเบาหวานชนิดที่ 2 ในหนูทดลอง โดยการแบ่งหนูทดลองออกเป็น 4 กลุ่ม คือ หนูปกติกลุ่มควบคุม (Normal control group; NC) หนูเบาหวานกลุ่มควบคุม (Diabetes mellitus control group; DMC) หนูเบาหวานที่บริโภคถั่วเขียวทอด (Fried mung bean group; FMB) และหนูเบาหวานที่บริโภคงาดำคั่ว (Roasted black sesame group; RBS) หนูทดลองนอกเหนือจากกลุ่มปกติควบคุมจะถูกกระตุ้นให้เป็นเบาหวาน และบริโภคอาหารไขมันสูง พบว่าในสัปดาห์ที่ 12 ของการทดลอง ระดับน้ำตาล HbA1C ระดับไขมันและคริอะตินินในเลือดในหนูทดลองแต่ละกลุ่มไม่มีความแตกต่างกัน แต่เมื่อทำการศึกษาพยาธิสภาพของไตและตับด้วยกล้องจุลทรรศน์แบบแสง และกล้องจุลทรรศน์อิเล็กตรอนแบบส่องผ่าน พบว่าหนูเบาหวานกลุ่มควบคุมมีปริมาณไขมันสะสมในตับสูง และมีความหนาของเยื่อฐานโกลเมอรูลัส (Glomerular basement membrane) และความกว้างของขาเซลล์โพโดไซต์ (podocyte foot process) มากกว่าหนูปกติกลุ่มควบคุมอย่างมีนัยสำคัญ ซึ่งเป็นภาวะแทรกซ้อนระยะแรกของตับและไตในโรคเบาหวาน และพบว่าหนูที่ได้รับถั่วเขียวทอดและงาดำคั่ว ไม่พบความผิดปกติทางพยาธิสภาพของตับและไตดังกล่าว จากผลการทดลองทั้งหมดในการวิจัยครั้งนี้อาจกล่าวได้ว่าการบริโภคถั่วเขียวทอดหรืองาดำคั่วสามารถบรรเทาพยาธิสภาพของไตและตับที่เกิดจากโรคเบาหวานในหนูทดลองที่เป็นเบาหวานชนิดที่ 2 ได้

ที่มา: ผู้ป่วยโรคปอดอุดกั้นเรื้อรังที่มีภาวะออกซิเจนต่ำขณะออกกำลังกาย ส่งผลให้ออกกำลังได้ลดลง กลไกการเกิดภาวะนี้ส่วนหนึ่งเกิดจาก dead space เพิ่มขึ้น การให้ออกซิเจนอัตราไหลสูงผ่านทางสายจมูกสามารถลด dead space ได้ และยังให้ออกซิเจนในความเข้มข้นคงที่ในขณะที่หายใจแรงขึ้น จึงน่าจะเพิ่มสมรรถภาพการออกกำลังกายและลดอาการเหนื่อยของผู้ป่วยได้ วัตถุประสงค์: เพื่อศึกษาผลของการให้ออกซิเจนอัตราไหลสูงผ่านทางสายจมูกขณะออกกำลังกายในผู้ป่วยโรคปอดอุดกั้นเรื้อรังที่มีภาวะออกซิเจนต่ำขณะออกกำลังกายเปรียบเทียบกับการให้ออกซิเจนอัตราไหลต่ำผ่านทางสายจมูก และไม่ให้ออกซิเจน วิธีการศึกษา: ศึกษาโดยวิธี randomized crossover study ในอาสาสมัครโรคปอดอุดกั้นเรื้อรังที่มีภาวะออกซิเจนต่ำขณะออกกำลังกาย โดยให้อาสาสมัครออกกำลังโดยปั่นจักรยานที่ความหนักที่ร้อยละ 70 ของความหนักสูงสุดที่อาสาสมัครสามารถทำได้ โดยสุ่มลำดับประเภทออกซิเจนที่ให้ขณะออกกำลังกาย ได้แก่ออกซิเจนอัตราไหลสูง, ออกซิเจนอัตราไหลต่ำ และไม่ใช้ออกซิเจน วิเคราะห์เปรียบเทียบระยะเวลาของการปั่นจักรยาน อาการเหนื่อยประเมินจาก Borg’s dyspnea scale (Borg-D) ของทั้งสามกลุ่ม และค่าอื่นๆที่วัดขณะออกกำลัง โดยวิธี mixed effect linear regression model ผลการศึกษา: ผู้เข้าร่วมการศึกษาทั้งสิ้น 14 คน เป็นเพศชายทั้งหมด อายุเฉลี่ย 71.3 (SD 8.7) ปี ค่า FEV1 เฉลี่ย ร้อยละ 60.7 (SD 13.4) ระยะเวลาการออกกำลังขณะใช้ออกซิเจนอัตราไหลสูง 768.4 (SD 276.0) และอัตราไหลต่ำผ่านทางสายจมูก 737.3 (SD 280.2) วินาที เปรียบเทียบกับกลุ่มที่ไม่ได้ใช้ออกซิเจนซึ่งออกกำลังกายได้นาน 665.6 (SD 313.4) วินาที (p=0.060 และ p=0.200) กลุ่มที่ได้รับออกซิเจนอัตราไหลสูงไม่พบว่ามีภาวะออกซิเจนต่ำขณะออกกำลัง ในขณะที่กลุ่มที่ได้ออกซิเจนอัตราไหลต่ำพบผู้เข้าร่วมการศึกษามีภาวะออกซิเจนต่ำขณะออกกำลังกาย ร้อยละ 14 ใน และกลุ่มที่ไม่ได้ออกซิเจน ร้อยละ 62 นอกจากนี้พบว่ากลุ่มที่ได้ออกซิเจนอัตราไหลสูงและอัตราไหลต่ำ มีคะแนน Borg-D ที่ isotime น้อยกว่ากลุ่มที่ไม่ได้รับออกซิเจน 2 (SD 0.6) และ 1.7 (SD 0.6) คะแนน ตามลำดับ …

ที่มา : ความสัมพันธ์ระหว่างการกลืนและการหายใจมีผลต่อการเกิดการสำลัก โดยโอกาสสำลักจะมากขึ้นเมื่อจุดที่ เริ่มกลืนนั้นอยู่ในช่วงการหายใจเข้า จากการศึกษาก่อนหน้านี้พบว่าการให้ออกซิเจนบำบัดอาจส่งผลต่อช่วงเวลา ที่เริ่มกลืน ซึ่งทำให้กลไกการป้องกันการสำลักนั้นเปลี่ยนแปลงไป วัตถุประสงค์ของการศึกษา : เพื่อศึกษารูปแบบความสัมพันธ์ระหว่างการกลืนและช่วงการหายใจ ขณะที่ทำ การกลืนน้ำอย่างต่อเนื่องในผู้ป่วยหลังกาารถอดท่อช่วยหายใจ โดยจะเปรียบเทียบระหว่างที่ผู้ป่วยได้รับออกซิเจน อัตราไหลสูง ( High flow nasal cannula:HFNC ) และออกซิเจนอัตราไหลต่ำ ( Low flow nasal cannula :LFNC ) วิธีศึกษา : การศึกษานี้เป็นการทดลองไขว้กลุ่มแบบสุ่ม โดยศึกษาในผู้ป่วยที่เพิ่งได้รับการถอดท่อช่วยหายใจมาไม่ เกิน 48 ชั่วโมง ทำการแบ่งผู้ป่วยเป็น 2 กลุ่ม โดยการสุ่ม โดยกลุ่มแรกจะได้รับออกซิเจนอัตราไหลสูง ที่ 50 ลิตร ต่อนาทีก่อน กลุ่มที่สองจะได้รับออกซิเจนอัตราไหลต่ำ 5 ลิตรต่อนาทีก่อน เป็นเวลา 5 นาที จากนั้นจะทำการตรวจ วัดคลื่นไฟฟ้าของกล้ามเนื้อการกลืน และช่วงการหายใจระหว่างที่ผู้ป่วยทำการกลืนน้ำปริมาณ 10 มิลลิลิตร อย่าง ต่อเนื่องในเวลา 1 นาที โดยจะทั้งหมด 3 ครั้ง จากนั้นจะทำการสลับออกซิเจนที่ให้ และทำการกลืนน้ำซ้ำอีก 3 ครั้งเช่นเดิม การตรวจวัดคลื่นไฟฟ้าของกล้ามเนื้อการกลืนจะใช้อุปกรณ์วัดคลื่นไฟฟ้ากล้ามเนื้อติดบริเวณใต้คาง ส่วนช่วงการหายใจจะตรวจวัดจาก ECG-derived respiratory signal และวัดคลื่นไฟฟ้าของกล้ามเนื้อที่ใช้ในการ หายใจประกอบกัน ความสัมพันธ์ระหว่างการกลืนและช่วงการหายใจแบ่งเป็น 4 รูปแบบ ได้แก่ I, E, I-E และ E-I ( I; inspiration, E; Expiration ) การวิเคราะห์ช้อมูลทางสถิติใช้การทดสอบ Wilcoxon Signed Rank test และใช้ค่าความน่าจะเป็น ( p value ) < 0.05 จึงจะถือว่ามีนัยสำคัญทางสถิติ ผลการศึกษา : มีผู้ป่วยทั้งสิ้น 22 คน เข้าร่วมในการศึกษา อายุเฉลี่ยอยู๋ที่ 55.8 ปี สาเหตุในการใช้เครื่องช่วยใจ ที่พบมากที่สุด คือ ปอดอักเสบ โดยมีระยะเวลาการใส่ท่อช่วยหายใจโดยเฉลี่ยอยู่ที่ 4.95 วัน ความถี่ของการกลืน อยู่ที่ 6.17 ครั้งต่อนาทีในกลุ่ม HFNC และ 7 ครั้งต่อนาทีในกลุ่ม LFNC รูปแบบความสัมพันธ์ระหว่างการกลืน และการหายใจที่พบบ่อยที่สุดคือ การกลืนตอนหายใจออก ( E swallow ) ในขณะที่ใช้ HFNC พบว่าผู้ป่วยมีการ กลืนแบบ E swallow มากกว่าอย่างมีนัยสำคัญ ( 74.3% ขณะใช้ HFNO เปรียบเทียบกับ 67.6% ขณะใช้ LFNC ; p 0.048) และมีผลรวมของอัตราการกลืนแบบ I swallow และ E-I swallow ต่ำกว่า ( 22.5% ขณะใช้ HFNC เปรียบเทียบกับ 31.15% ขณะใช้ LFNC ; p 0.04) สำหรับรูปแบบการกลืนอื่นๆนั้นไม่แตกต่างกัน สรุป : HFNC ช่วยเพิ่มการกลืนแบบ E swallow และลดการกลืนแบบ I และ E-I swallow ได้ในผู้ป่วยหลังการ ถอดท่อช่วยหายใจได้อย่างมีนัยสำคัญ เมื่อเทียบกับ LFNC ผลการศึกษาที่พบนี้อาจบ่งชี้ว่าการใช้ HFNC ในผู้ป่วย กลุ่มนี้อาจสามารถช่วยลดความเสี่ยงในการสำลักได้

ที่มา: ผู้สูงอายุที่เป็นมะเร็งเป็นกลุ่มที่มีความเสี่ยงต่อได้รับยาเคมีบำบัดไม่ครบตามกำหนด ปัจจุบันมีเครื่องมือคัดกรอง Geriatric 8 (G8) ที่ช่วยประเมินถึงความพร้อมต่อการรักษาโรคมะเร็งรวมถึงเคมีบำบัด โดยผู้สูงอายุที่มีความเสี่ยงจากการใช้เครื่องมือคัดกรอง G8 ควรได้รับประเมินผู้สูงอายุแบบองค์รวม(comprehensive geriatric assessment, CGA) เพื่อที่จะแสดงให้เห็นถึงปัญหาและรักษาปัญหานั้น อย่างไรก็ตามผลของการประเมินผู้สูงอายุแบบองค์รวมต่ออัตราการได้รับยาเคมีบำบัดครบตามกำหนดนั้น ยังมีการศึกษาไม่มากนัก วัตถุประสงค์: เพื่อศึกษาถึงประโยชน์ของการประเมินผู้สูงอายุแบบองค์รวมและให้การรักษาตามปัญหาที่พบต่อการเพิ่มอัตราการได้รับยาเคมีบำบัดครบตามกำหนด วิธีการศึกษา: การศึกษาแบบเปิดและสุ่ม ในผู้สูงอายุมากกว่าหรือเท่ากับ 65ปีที่ดีรับการวินิจฉัยเป็นมะเร็งก้อนทูม ต้องคัดกรองด้วยเครื่องมือG8แล้วเข้าเกณฑ์กลุ่มเสี่ยง(น้อยกว่าหรือเท่ากับ 14คะแนน) และต้องมีแผนการรักษาด้วยยาเคมีบำบัดเท่านั้น สุ่มประชากรเป็น 2กลุ่มคือกลุ่มที่ได้รับการประเมินผู้สูงอายุแบบองค์รวมและกลุ่มที่ได้รับการดูแลตามมาตรฐานปกติ วัตถุประสงค์หลักของการศึกษาคืออัตราการได้รับขนาดยาเคมีบำบัดครบตามกำหนด นอกจากนี้ยังศึกษาถึงปัจจัยที่มีผลต่อการได้รับยาเคมีบำบัดครบตามกำหนดด้วย ผลการศึกษา: ตั้งแต่มิถุนายน พ.ศ. 2562 ถึง ธันวาคม พ.ศ. 2562 มีผู้สูงอายุมะเร็งทั้งหมด 52 คนโดยสุ่มไปในแต่ละกลุ่ม กลุ่มละ 26คน อายุเฉลี่ยอยู่ที่ 72ปี ส่วนใหญ่เป็นเพศหญิงคิดเป็นร้อยละ 59.6 วินิจฉัยเป็นมะเร็งเต้านมมากที่สุดคิดเป็นร้อยละ 40.4 ข้อมูลพื้นฐานในทั้งสองกลุ่มสมดุลกัน พบว่าจากการวิเคราะห์แบบper protocol อัตราการได้รับยาเคมีบำบัดครบตามกำหนดในกลุ่มที่ได้รับการประเมินผู้สูงอายุแบบองค์รวมเทียบกับกลุ่มที่ได้รับการดูแลตามมาตรฐานปกติไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ(ร้อยละ 57.1 เทียบกับร้อยละ 50, OR 1.33; 95%Cl 0.42-4.24; p=0.62) และพบว่าปัจจัยที่ส่งผลเพิ่มอัตราการได้รับยาเคมีบำบัดครบตามกำหนดคือ อายุน้อยกว่า 75ปี ค่าดัชนีมวลกายที่มากกว่า 20 กิโลกรัมต่อตารางเมตร ประเมินคะแนนด้วยเครื่องมือคัดกรองG8อยู่ในเกณฑ์เสี่ยงปานกลาง และค่าแอลบูมินในเลือดที่มากกว่า 4 กรัมต่อเดซิลิตร สรุปผลการศึกษา: ผลการศึกษานี้พบว่าการประเมินผู้สูงอายุแบบองค์รวมและให้การรักษาตามปัญหาที่พบไม่สามารถเพิ่มอัตราการได้รับยาเคมีบำบัดครบตามกำหนดอย่างมีนัยสำคัญทางสถิติ

Cancer-associated cachexia is a condition defined by a sustained net-negative energy imbalance. Although the different types of adipose tissue – white, beige, and brown – have been implicated in contributing to cancer-associated cachexia, the mechanisms of these maladaptive changes and their impact on whole-body energy expenditure have not been fully elucidated. Using breast cancer as our model, we demonstrate white adipose tissue browning in murine and human breast cancer; furthermore, we demonstrate that this effect is extremely localized and takes place early in tumor progression. Pregnancy loss in cattle is greatest in the first 30 days of gestation. Near d15 of gestation, bovine conceptuses elongate and express LGALS1, an immunosuppressive glycan-binding protein believed to be essential for implantation. However, other members of the bovine galectin family, including LGALS3, LGALS4, LGALS7, LGALS8, LGALS9 and a galectin-13-like gene (LGALS13L), also may be expressed by the elongating conceptus. Therefore, galectin expression in elongating, d16 bovine conceptuses and intercaruncular endometrium as well as elongating, d14 ovine conceptuses and intercaruncular endometrium was assessed by RT-qPCR. Materials and methods: We conducted a cohort study involving children aged 7?17 years old with asthma. Background/aim: We aimed to investigate Internet addiction in pediatric patients with migraine- and tension-type headache in this study. Materials and methods: Among our 200 subjects, 103 had migraine-type headache and 97 had tension-type headache. Results: Headache triggered by computer use was more common in the migraine-type headache group. There was no difference between the Internet addiction scale score of the two groups. The Internet addiction scale scores of the patients were found to differ depending on the aim and duration of computer use. Internet addiction was found in six (6%) patients. We evaluated the association between serum fetuin-A levels and vascular calcification, intima-media thickness, and nutritional and inflammatory markers in different stages of chronic kidney disease (CKD). Materials and methods: CKD patients were sampled for calcium-phosphate parameters and nutritional and inflammatory markers [highly sensitive C-reactive protein (hs-CRP)], and serum fetuin-A levels.
