Infant formula is a human milk substitute generally based upon fortified cow milk components. In order to mimic the composition of breast milk, trace elements such as copper, iron and zinc are usually added in a single operation using a premix. The correct addition of premixes must be verified to ensure that the target levels in infant formulae are achieved. In this study, a laser-induced breakdown spectroscopy (LIBS) system was assessed as a fast validation tool for trace element premixes. LIBS is a promising emission spectroscopic technique for elemental analysis, which offers real-time analyses, little to no sample preparation and …

K+ channels containing Kv1.1 α subunits, which become prevalent at internodes in demyelinated axons, may underlie their dysfunctional conduction akin to muscle weakness in multiple sclerosis. Small inhibitors were sought with selectivity for the culpable hyper-polarizing K+ currents. Modeling of interactions with the extracellular pore in a Kv1.1-deduced structure identified diaryldi(2-pyrrolyl)methane as a suitable scaffold with optimized alkyl ammonium side chains. The resultant synthesized candidate [2,2′-((5,5′(di-p-topyldiaryldi(2-pyrrolyl)methane)bis(2,2′carbonyl)bis(azanediyl)) diethaneamine·2HCl] (8) selectively blocked Kv1.1 channels (IC50 ≈ 15 μM) recombinantly expressed in mammalian cells, induced a positive shift in the voltage dependency of K+ current activation, and slowed its kinetics. It …

Microbiota, Diet And Medication In Human Gut Disorders, Adam Clooney
Theses
The microbiota, a community of micro-organisms in a particular environment, have been intensely researched in the past decade and interest appears to continue to increase. The human body is richly populated with bacterial cells (3.8x10'^) and alterations in these communities are associated with disease. The work presented in this thesis explores aspects of microbiota research methodologies with regard to optimisation and concerns over a lack of reproducibility. Additionally, we investigated the effects of medication, in particular Proton Pump Inhibitors, on the human gut microbiota, along with comparisons between the microbiota of patients with active and inactive Inflammatory Bowel Disease.

The Effects Of The Chloride:Sodium Ratio On Acid-Base Statusand Mortality In Septic Patients, Hakan Korkut Atalan, Bülent Güçyetmez
Turkish Journal of Medical Sciences
Background/aim: Calculation of the chloride:sodium (Cl-:Na+) ratio is proposed to enable a quick evaluation of the effect of Cl- and Na+ on the acid-base balance in critically ill patients. In the present study, the relationship of the Cl-:Na+ ratio of septic patients with acidbase status and ICU mortality were investigated. Materials and methods: In our two-center study, 434 patients who were diagnosed with sepsis were included. The patients were divided into three groups: low (<0.75), normal (≥0.75, <0.80), and high (≥0.80) Cl-:Na+ ratio groups. Patients' demographic data, blood gas values, length of ICU stay, and ICU mortality were recorded. Results: In the low and high groups, ICU mortality was significantly higher than in the normal group (29.3%, 37.1%) (P = 0.005). There was a negative correlation between the Cl:Na+ ratio and each of HCO3 -, standard base excess, and PaCO2 (r2 = 0.21, r2 = 0.19, and r2 = 0.17) (P < 0.001 for each). In the multivariate analysis, the ICU mortality was increased 2.6-fold (1.2-5.8) by low Cl-:Na+ ratio (P = 0.019). Conclusion: The Cl-:Na+ ratio is a useful parameter for showing the relationship between Cl- and Na+ and their impact on acid-base status. Low Cl-:Na+ ratio at ICU admission can be used as a prognostic indicator for increased ICU mortality in septic patients.

Turkish Journal of Medical Sciences
Background/aim: Investigation of the association between obesity and the distinction of radiographic patterns in knee osteoarthritis. Materials and methods: Seven hundred and thirty-four women underwent weight-bearing antero-posterior knee radiography. Osteophytes and joint space narrowing (JSN) were graded according to the OARSI atlas. Each subject was assigned to one of the following groups with respect to the maximum score: osteophyte-dominant, indeterminate, JSN-dominant, and radiographically normal. Results: Obese patients had a significantly more frequent osteophyte-dominant pattern compared to nonobese subjects (74.5% and 38%, respectively, P < 0.001). Logistic regression analysis demonstrated that obesity had a stronger association with an osteophytedominant pattern compared to a JSN-dominant pattern (OR and 95% CI = 7.16 (3.15-16.26) and 1.63 (0.96-2.78), respectively). Age had a very weak effect on the distinction to an osteophyte-dominant pattern and no effect on JSN dominance (OR and 95% CI = 1.1 (1.06-1.15) and 1.02 (0.99-1.05), respectively). Conclusion: There might be an association between obesity and the radiographic phenotype in patients with knee osteoarthritis. The findings suggest that the association between obesity and the osteophyte formation is stronger than that of JSN.

Turkish Journal of Medical Sciences
Background/aim: Psoriasis is a chronic skin disease. To reduce side effects associated with current treatment modalities, new treatment methods are required. Some clinicians have begun to use microphototherapy to treat psoriatic patients with a limited number of lesions (lesions affecting <10% of the body surface area). Materials and methods: Microphototherapy is indicated in patients whose lesions persist despite systemic and topical treatment or when such treatment is contraindicated. Our study aimed to evaluate the efficacy of microphototherapy in psoriasis patients with localized lesions. Patients in this unblinded and prospective study were treated three times a week for 50 sessions using a MedLight CupCUBE Grimed microphototherapy device that emits an ultraviolet B (UVB) spectrum. Results: Forty-five lesions in 23 psoriatic patients were treated. Treatment outcome was evaluated based on the Psoriasis Severity Index (PSI). PSI scores decreased significantly during the treatment. Conclusion: The MedLight CupCUBE Grimed microphototherapy device was effective for the treatment of psoriasis patients with localized lesions. The device can administer UVB radiation to involved regions while protecting uninvolved regions and minimizing radiation dose. However, having to be administered by experienced technicians and the long time required for each treatment session are disadvantages of the technique.

Background/aim: Myocarditis in the acute phase usually presents with sinus tachycardia but many other arrhythmias might be seen as well. In this study we aimed to investigate repolarization abnormalities in baseline ECG of patients with myocarditis for the first time.Materials and methods: Thirty patients diagnosed with myocarditis and 25 healthy age-matched controls were included. Two different cardiologists measured corrected QT (QTc), QT dispersion (QTd), QT peak (QTp), T wave peak to T wave end (TpTe), TpTe/QT ratio, and TpTe/QTc ratio in 12-lead ECG.Results: When compared with the control group, QTp (P: 0.021), QT (P: 0.003), TpTe (P < 0.001), TpTe/QTc ratio (P < 0.001), and TpTe/QT ratio (P: 0.005) were significantly higher in patients with myocarditis. A comparison of receiver operating characteristic (ROC) curves was conducted using the Hanley and McNeil method. The area under the curve (AUC) of the electrocardiographic characteristics QT (AUC: 0.736; 95% CI [0.600-0.846]), QTP (AUC: 0.680; 95% CI [0.540-0.799]), and TpTe (AUC: 0.771; 95% CI [0.638-873]) and TpTe/QTc (AUC: 0.774; 95% CI [0.641-0.876]) and TpTe/QT (AUC: 0.726; 95% CI [0.589-0.838]) in myocarditis were not significantly different from each other but all of them were different from 0.5.Conclusion: Baseline ECGs of patients with myocarditis were associated with repolarization abnormalities. These novel findings may be one of the reasons underlying arrhythmic events in patients with myocarditis.
