Public Health Surveillance Using Electronic Health Records: Rising Potential To Advance Public Health, Guthrie S. Birkhead, Michael Klompas, Md Mph, Nirav R. Shah

Frontiers in Public Health Services and Systems Research

Background: Public health surveillance has traditionally relied on manual processes including paper-based reporting by clinicians. The introduction of electronic laboratory reporting increased the efficiency and completeness of infectious disease surveillance but clinical and risk factor data are often still collected manually. The use of electronic health records (EHR) has significant promise to enrich surveillance by collecting these data automatically and by expanding surveillance to chronic diseases (e.g., diabetes, hypertension, obesity). However, the extent of the use of EHRs for public surveillance is not well studied.

Evidence Acquisition: The peer-reviewed medical literature was searched for descriptions of the use of EHRs …

Public Health At Risk: The Conscious Choice To Refuse Vaccinations Threatens Public Health Jun 2015

Public Health At Risk: The Conscious Choice To Refuse Vaccinations Threatens Public Health

DePaul Magazine

Public health officials declared that measles had been eliminated from the United States after only 15 measles cases were reported between 1998 and 2001 and 90 percent of schoolchildren had been inoculated against the disease. Yet, in 2014, there were more than 660 documented measles cases in the country, the most in 20 years. This uptick started in the late 1990s, when a growing number of parents began refusing to inoculate their children with some or all of the recommended vaccines. Various factors fed their decision: an increase in the number of vaccines given in the first year of life, …

The Paleoepidemiology Of Malaria In The Ancient Near East, Nicole Elizabeth Smith May 2015

The Paleoepidemiology Of Malaria In The Ancient Near East, Nicole Elizabeth Smith

Graduate Theses and Dissertations

The end of the Late Bronze Age in the Near East (1300 - 1200 BCE) saw the widespread collapse of several large cultural centers, the reasons for which are a subject of continued debate. Evidence from events leading up to this cultural collapse suggest epidemic disease may have factored into the eventual downfall of these early civilizations. Recent DNA analysis from Egyptian mummies who lived during the period leading up to the Late Bronze Age collapse identified malaria in several elite individuals, suggesting the widespread prevalence of this infectious disease in Egypt. However, the exact prevalence, antiquity, and dynamics of …

Fighting Microbial Drug Resistance: A Primer On The Role Of Evolutionary Biology In Public Health, Gabriel Perron, R. Inglis, Pleuni Pennings, Sarah Cobey

Biology Department Faculty Works

Although microbes have been evolving resistance to antimicrobials for millennia, the spread of resistance in pathogen populations calls for the development of new drugs and treatment strategies. We propose that successful, long-term resistance management requires a better understanding of how resistance evolves in the first place. This is an opportunity for evolutionary biologists to engage in public health, a collaboration that has substantial precedent. Resistance evolution has been an important tool for developing and testing evolutionary theory, especially theory related to the genetic basis of new traits and constraints on adaptation. The present era is no exception. The articles in …

Validating Estimates Of Prevalence Of Non-Communicable Diseases Based On Household Surveys: The Symptomatic Diagnosis Study, Spencer L. James, Minerva Romero, Dolores Ramírez-Villalobos, Sara Gómez

Dartmouth Scholarship

Easy-to-collect epidemiological information is critical for the more accurate estimation of the prevalence and burden of different non-communicable diseases around the world. Current measurement is restricted by limitations in existing measurement systems in the developing world and the lack of biometry tests for non-communicable diseases. Diagnosis based on self-reported signs and symptoms (“Symptomatic Diagnosis,” or SD) analyzed with computer-based algorithms may be a promising method for collecting timely and reliable information on non-communicable disease prevalence. The objective of this study was to develop and assess the performance of a symptom-based questionnaire to estimate prevalence of non-communicable diseases in low-resource areas.

Association Between Respiratory Syncytial Virus Activity And Pneumococcal Disease In Infants: A Time Series Analysis Of Us Hospitalization Data., Daniel M. Weinberger, Keith P. Klugman, Claudia A. Steiner, Lone Simonsen, Cécile Viboud

Epidemiology Faculty Publications

BACKGROUND:

The importance of bacterial infections following respiratory syncytial virus (RSV) remains unclear. We evaluated whether variations in RSV epidemic timing and magnitude are associated with variations in pneumococcal disease epidemics and whether changes in pneumococcal disease following the introduction of seven-valent pneumococcal conjugate vaccine (PCV7) were associated with changes in the rate of hospitalizations coded as RSV.

METHODS AND FINDINGS:

We used data from the State Inpatient Databases (Agency for Healthcare Research and Quality), including >700,000 RSV hospitalizations and >16,000 pneumococcal pneumonia hospitalizations in 36 states (1992/1993-2008/2009). Harmonic regression was used to estimate the timing of the average seasonal …

Maternal syphilis results in an estimated 500,000 stillbirths and neonatal deaths annually in Sub-Saharan Africa. Despite the existence of national guidelines for antenatal syphilis screening, syphilis testing is often limited by inadequate laboratory and staff services. Recent availability of inexpensive rapid point-of-care syphilis tests (RST) can improve access to antenatal syphilis screening. A 2010 pilot in Zambia explored the feasibility of integrating RST within prevention of mother-to-child-transmission of HIV services. Following successful demonstration, the Zambian Ministry of Health adopted RSTs into national policy in 2011. Cost data from the pilot and 2012 preliminary national rollout were extracted from project records, …

The Role Of Religious Vaccination Exemptions In The Ocean County, New Jersey Pertussis Outbreak, Siobhan Bridget Pappas Jan 2015

The Role Of Religious Vaccination Exemptions In The Ocean County, New Jersey Pertussis Outbreak, Siobhan Bridget Pappas

Walden Dissertations and Doctoral Studies

Pertussis, also known as whooping cough, is a vaccine-preventable disease that is on the rise in the United States, a trend which has been attributed to vaccination exemption. Indeed, the pertussis outbreak that occurred in February, 2012 in Ocean County, New Jersey was associated with vaccine exemption. Considering that pertussis is deadly to young children, it is important to understand why this disease rate is on the rise. The research questions were focused on whether a relationship existed between pertussis status (no, yes) and exemption status (no, yes), sex (male, female), and county type (Middlesex, Ocean, or Other), using a …

Herpes Virus Infections, Inflammatory Markers And Risk Of Developing T2dm And Cvd: An Analysis Of Nhanes With Adults, Aged 20-49, 1999-2010, Margarita Irizarry-De La Cruz

Walden Dissertations and Doctoral Studies

Herpes simplex viruses (HSVs), are among the most virulent and widespread pathogens; they affect 60-90% of the population worldwide. Substantial evidence indicates a possible association between pathogens and chronic disease. HSVs, among other viruses, have been associated with increased risk for inflammatory diseases. However, prior findings have been inconsistent on the role of infection in triggering autoimmune response and chronic disease. This study builds on the premise that pathogens can induce an inflammatory response and increase the risk for disease development. A representative U.S. sample from NHANES, a national population-based cross-sectional survey, was used to examine the relationship between HSVs …

Within hosts, RNA viruses form populations that are genetically and phenotypically complex. Heterogeneity in RNA virus genomes arises due to error-prone replication and is reduced by stochastic and selective mechanisms that are incompletely understood. Defining how natural selection shapes RNA virus populations is critical because it can inform treatment paradigms and enhance control efforts. We allowed West Nile virus (WNV) to replicate in wild-caught American crows, house sparrows and American robins to assess how natural selection shapes RNA virus populations in ecologically relevant hosts that differ in susceptibility to virus-induced mortality. After five sequential passages in each bird species, we …

The Effects Of Landscape On Malaria Vectors Of The Neotropics, William David Lainhart

Legacy Theses & Dissertations (2009 - 2024)

Worldwide malaria control efforts have had varying degrees of success, leaving many countries either eliminating malaria or controlling malaria. In order to maintain forward momentum toward malaria elimination, malaria-eliminating and –controlling countries require different information regarding risk factors, spatial distributions of cases and vectors, vector biology metrics, and vector population genetics. The goal of this dissertation work was to provide information that addresses the knowledge gaps preventing the creation of effective vector and malaria control programs in one malaria-eliminating country (Panama) and two malaria-controlling countries (Peru and Brazil). Case data from Panama were analyzed to assess differences in risk factors …
