Purpose. The purpose of this pilot study was to determine if a difference existed between nosocomial pneumonia rates for intubated critical care unit (CCU) patients who received twice-daily oral hygiene care with 0.12% chlorhexidine gluconate and those who received the standard oral care. Methods. Over seven months (February to August), CCU patients were identified through screening and informed consent procedures, and randomized into 1 of 2 groups. Acute Lymphoblastic Leukemia (ALL) is the most common malignancy of childhood, constituting 31 percent of all childhood malignancies. Treatment strategies to target ALL include chemotherapeutic agents, irradiation of the neck and/or spine, and bone marrow transplantation. The aggressive nature of antineoplastic therapies often produces numerous craniofacial and dental sequelae as well as additional harmful effects to the entire body. Cranial irradiation may adversely affect the hypothalamic-pituitary axis decreasing growth hormone production. Consequently, children with ALL may experience a transient or permanent reduction in growth iatrogenically. Hand-wrist radiographs are valuable for evaluating maturational status in children with ALL. These radiographs allow …

Studies investigating the genetic and environmental characteristics of atrial fibrillation (AF) may provide new insights in the complex development of AF. We aimed to investigate the association between several environmental factors and loci of candidate genes, which might be related to the presence of AF. A nested case-control study within the PREVEND cohort was conducted. Standard 12 lead electrocardiograms were recorded and AF was defined according to Minnesota codes. For every case, an age and gender matched control was selected from the same population (n = 194). Mechanisms that cause Alzheimer's disease (AD), an invariably fatal neurodegenerative disease, are unknown. Important recent data indicate that neuronal heme deficiency may contribute to AD pathogenesis. If true, factors that contribute to the intracellular heme deficiency could potentially alter the course of AD. The porphyrias are metabolic disorders characterized by enzyme deficiencies in the heme biosynthetic pathway. We hypothesize that AD may differ significantly in individuals possessing the genetic trait for an acute hepatic porphyria. We elaborate on this hypothesis and briefly review the characteristics of the acute hepatic porphyrias that may be relevant to AD. We note the proximity …

Hypertension is a serious health problem that affects approximately 1 in 4 American adults. Most cases are diagnosed as essential hypertension, meaning that the exact cause is unknown. In most patients, however, excess weight is a major contributory factor to the development of essential hypertension. The role of obesity in promoting hypertension is now well documented and has become the foundation for an entire field of research known alternately as obesity-hypertension, obesity-induced hypertension, or obesity-associated hypertension. This Population Council Horizons report presents findings from an exploratory study by the Regional Psychosocial Support Initiative and Catholic Relief Services’ Support to Replicable, Innovative Village/Community-level Efforts Program of vulnerable youth living in and around Bulawayo, Zimbabwe. It describes their demographic characteristics, exposure to stress and trauma, and psychosocial well-being. The report also highlights the relationships between psychosocial well-being outcomes and exposure to stress and trauma, and the differences in psychosocial well-being between males and females, orphaned and nonorphaned youth, and younger and older adolescents. The report concludes with program and research implications.

This Horizons program report describes the status of pediatric HIV treatment in selected sites in South Africa, identifies gaps in service delivery, and proposes recommendations for strengthening services and expanding children’s access to treatment. The study provides much needed information on critical issues of pediatric HIV care, especially regarding health service and contextual issues surrounding the expansion of access to treatment for HIV-infected children, and key factors that facilitate sustainability of treatment by young children. Incorrect inhaled drug delivery technique is a common problem in paediatric asthma [1-4]. Previously, device technique training has been shown to significantly improve inhaler skills, using subjective methods, namely check-list analysis [5-8]. The distinctive pathological marker of Parkinson’s Disease (PD) is the progressive death of neurons that produce dopamine; however, there are also major alterations in the production of quantities of other neurotransmitters, such as norepinephrine and serotonin that contribute to the signs, symptoms, neuropsychological manifestations of the diseases. PD patients can be divided into classes based on the manifestation of motor symptoms, type A, classified as tremor dominant, and type B PD, classified as akinetic. History Faculty Research

Formative research for hygiene promotion was used to gather data relating to hygiene practices in rural Kyrgyzstan. Some of the hand-washing and faeces disposal practices observed were sub-optimal from a public health perspective. In combination with the poverty, limited medical services and poor water supply infrastructure characteristic of the study area, it is likely that these factors increase the risk of diarrhoeal infections among Kyrgyz children. An association was found between increased rates of hand-washing following latrine use and ownership of a washstand. This offers some empirical support for the idea that promotion of hand-washing technologies can form a useful …

Inquiry: The University of Arkansas Undergraduate Research Journal

Consumption of raw or lightly cooked alfalfa sprouts has been a concern of the U.S. Food and Drug Administration (FDA) in recent years due to connections between sprouts and foodborne illnesses. Researchers have identified, contaminated seeds as the primary source of alfalfa sprouts contamination. Contamination of alfalfa seeds can originate in the field, harvesting, storing, or sprouting. Two pathogens of particular concern on alfalfa seeds are Escherichia coli 0157:H7 (E. coli) and Salmonella. These pathogens are capable of producing biofilms that provide protection for individual cells and allow for survival in otherwise hostile environments, including some disinfectant washes. Our hospital led a multidisciplinary community team to improve the quality of care delivered to HIV-infected clients utilizing a disease management approach in a US metropolitan community of 150,000 people. Community needs assessment and client and community surveys were used to define the problems. Patient care flowcharting and the creation of an electronic patient database facilitated patient tracking across the entire community. Clinical guidelines and a consultation and referral immunology clinic standardized care practices. Measurable improvements in the quality of care were noted in multiple areas. Flowchart completion rates rose from 44% to 100%; medication adherence assessment rose from 82% …
