Our segregated medical and mental health care system misses opportunities for early detection of physical disease. Missed opportunities lead to increased costs. Colorado’s Medicaid health plan estimates that an integrated health program will reduce costs for patients with psychiatric disorders by 10%. The cost of doing nothing may exceed $300 billion per year in the United States (NCCBH, 2010). The purpose of this Capstone project is to critically examine existing models of Integrated Primary and Mental Health care. These models will then be evaluated to identify those most compatible with rural communities in Southern Minnesota.

Suicide and suicide attempts are a frequently encountered clinical crisis and the assessment, management and treatment of suicidal patients is one of the most stressful tasks for clinicians (Fowler, 2012). The purpose of this project was to examine whether current assessment and treatment protocols followed in the ED, Mankato, adhere to best-‐practices as identified through research, as well as to make recommendations to MCHS-‐Mankato for future considerations and potential modifications to existing protocols.

This research investigated the working alliance between therapist and client at RiverView Clinic, an outpatient mental health center. Utilizing data captured by the Working Alliance Inventory-Short Revised (WAI-SR), the research posited the following:
- median total scores from therapist’s would be higher than median scores of the working alliance from clients
- clients who presented with mild to moderate concerns would have higher working alliance scores than clients who reported moderate to severe concerns

Treatment Foster Care is a unique model of care that provides the best components of traditional foster care and residential care (Foster Family-‐Based Treatment Association, 2004). There is an increase interest in exploring the provision of Treatment Foster Care for adjudicated youth. Exposure to traumatic events during childhood can affect brain development, how someone reacts to stress, and what they may find threatening or unsafe. Trauma can be a precursor to the development of a range of psychopathology including post-traumatic stress disorder, depression, anxiety, substance abuse, personality distortions and psychosis (APA, 2000, Rose, Freeman & Proudlock, 2012). Achieving timely legal permanency for children placed in out-of-home care due to abuse or neglect is one of the primary goals set forth by the state of Minnesota and the Department of Human Services. In recent years some counties have failed to achieve the percentage of finalized permanency cases at the targeted time intervals required by the state. A comprehensive review of one county was conducted of the 49 cases reaching permanency during 2011 and 2012 to determine if there is a correlation between certain demographic factors and/or key court dates on the length of time to achieve permanency.

An evaluation was conducted of the Re-Education philosophy, current practices of individual and group therapy, skills training, and psychoeducation for Level IV special education students diagnosed with an emotional behavioral disorder and/or mental health diagnosis. An assessment of current evidenced based practices for school-based mental health services was integrated. This provided the foundation for recommendations that are congruent with the Re-Education philosophy and for an evaluation tool to measure student success in learning and comprehending the skills curriculums.

The Juvenile Protection Provisions of the Juvenile Court Act, describes the requirement of county social service agencies to provide reasonable efforts to prevent placement prior to the removal of any child from the home. In St. Peter's Independent School District #508, 50% of all elementary school students are enrolled in the free and reduced lunch, which is available only to low‐income households (Greater Mankato Area United Way, 2008). Furthermore, the overall food insecurity rate for Nicollet County is 9.6%, which translates to 3,090 people (Feeding America, 2013). Of those 3,090 people faced with food insecurity in Nicollet County, 1,000 are youth (Feeding America, 2013). This translates to a rate of 14.4% of youth facing food insecurity in Nicollet County (Feeding America, 2013). Even more distressing is that 39% of these youth are ineligible for …
