People with disabilities report physical barriers in doctors’ offices that affect the quality of care. The analysis seeks to describe overall primary care office physical accessibility and identify (1) in which areas offices meet access criteria, (2) which accessibility criteria are most often not met, and (3) whether there are urban/non-urban differences.

Mental health organizations can play a key role in enhancing youths' access to care by working together to bridge gaps in service delivery systems. This dissertation study examines partnerships among a network of children's behavioral health organizations. The specific aims are to (1) describe and understand the network of partnerships among members of the Children's Services Coalition, (2) assess the capacity of the system to provide coordinated service delivery, and (3) test how patterns of organizational characteristics influence conditions that facilitate and inhibit partnerships among the Children's Service Coalition organizations.

The President's Emergency Plan for AIDS Relief (PEPFAR) is the most recent international social program instituted by the U.S. Government to combat HIV/AIDS. Since its inception in 2003, this foreign policy initiative has dedicated $63 billion for HIV/AIDS prevention and treatment in foreign countries. Despite PEPFAR's many accomplishments, it continues to promote controversial prevention strategies. This paper analyzes these prevention strategies, utilizing social work values as described in the NASW Code of Ethics. Policy, practice, and research implications are discussed.

Breast Cancer Care In Canada And The United States: Ecological Comparisons Of Extremely Impoverished And Affluent Urban Neighborhoods, Kevin M. Gorey

This study examined the differential effect of extreme impoverishment on breast cancer care in urban Canada and the United States. Ontario and California registry-based samples diagnosed between 1998 and 2000 were followed until 2006. Extremely poor and affluent neighborhoods were compared. Poverty was associated with non-localized disease, surgical and radiation therapy (RT) waits, non-receipt of breast conserving surgery, RT and hormonal therapy, and shorter survival in California, but not in Ontario. Extremely poor Ontario women were consistently advantaged on care indices over their California counterparts. Effects of socioeconomic status on the long-term survival of 808 women with node-negative breast cancer in Canada and the United States were observed. Ontario and California samples diagnosed between 1988 and 1990 were followed until 2006. Socioeconomic data were taken from population censuses. Compared with their California counterparts, residents of low-income urban areas in Ontario experienced a significant 15-year survival advantage (RR = 1.66 [95% CI: 1.00, 2.76]). In these and other vulnerable, lower-middle- to working-class neighborhoods, significantly more Ontario residents gained access to adjuvant radiation therapy (RR = 1.75 [1.21, 2.53]) which seemed associated with better long-term survival (RR …

BACKGROUND: This study tested the hypothesis that physician supply thresholds are associated with breast cancer survival in Ontario.

METHODS: The 5-year survival of 17,820 female breast cancer patients diagnosed between 1995 and 1997 was surveilled until 2003 for all-cause mortality. Physician supply densities in 1991 and 2001 were computed for 49 Ontario regions.

RESULTS: There were independent threshold effects for general practitioners (GP; 7.25 per 10,000) and obstetrician/gynecologists (OB/GYN; 6 per 100,000) at or above which women with breast cancer were more likely to survive for 5 years. In this invited article, Cabrera-Nguyen provides guidelines for reporting scale development and validation results. Authors' attention to these guidelines will help ensure the research reported in JSSWR is rigorous and of high quality. This article provides guidance for those using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). In addition, the article provides helpful links to resources addressing structural equation modeling, multiple imputation for missing data, and a general resource for quantitative data analysis.

Indigenous And Mestizo Mexican Migrant Farmworkers: A Comparative Mental Health Analysis, William Donlan, Junghee Lee

Mexican-origin migrant farmworkers using: (a) lifetime prevalence of culture-bound syndromes, (b) self-rated emotional/mental health, (c) depression measured by PHQ-9, (d) stress level. Demographic and psychosocial variables were examined by ethnicity and gender using Chi square and independent t-tests. Logistic and linear regression models were constructed for mental health variables. Indigenous participants reported significantly higher stress compared to mestizos, and indigenous women reported significantly higher stress compared to all groups. Prevalence of culture-bound syndromes and mean PHQ-9 severity score was highest for indigenous females. Mean self-rated emotional/mental health was lowest among indigenous females. Each year approximately 12 million admissions and releases occur in U.S. jails (Solomon, A. L., Osborne, J. W. L, LoBuglio, S. F., Mellow, J., & Mukamal, D. A.). Those that cycle in and out of jail often times are unstable. These instabilities include: substance abuse, lack of housing and employment, mental illness, and health concerns. More than 80 percent of offenders are incarcerated for less than one month; some for a few hours or a few days (Solomon, et. al.). This study is a secondary data analysis that uses structural equation modeling (SEM) to assess the utility of the Behavioral Risk Factor Surveillance System (BRFSS)--the Centers for Disease Control's premier surveillance tool for monitoring behavioral risk factors--in predicting health related quality of life (HRQoL). Employing the statistical package SPSS/AMOS (version 7), the study utilizes New York State data extracted from the 2007 BRFSS national dataset to assess how well the observed pattern of variances and covariances of the state level data fit with a well-known HRQoL model developed by Wilson and Cleary (1995). The study used a stratified cluster design of public high schools in the DR with sample weights. The questionnaire included items from the 2009 Youth Risk Behavior Survey.
