Abstract

Background:

Readmission to the hospital within 30-days has a high cost and represents a gap in care for older adults. Older adults are at significant risk for depression, particularly given their medical comorbidities and social factors such as isolation due to SARS-CoV-2. Many patients who screen positive for depression may have no known history of depression. This investigation examines the relationship between a positive geriatric depression screen and chief complaint as a function of 30- and 90-day readmission risk.

Adult Day Services (ADS) have become increasingly available for community-dwelling older adults who are often experiencing multiple chronic conditions and/or dementia. ADS providers spend a significant amount of time with their clients and offer the opportunity for a wealth of clinical information that can be used by primary care providers and specialists for decision-making about patient care. There are also opportunities for hospitals to coordinate care transitions with ADS providers by involving them with discharge planning with appropriate patients who require post-hospital care. From a comparative analysis of 214 nursing homes in the Chicago area, it was found that the nursing home field is composed of institutions with great variations in treatment resources available to the residents (Kosberg and Tobin, 1972). While the determination of organizational correlates to the extent of treatment resources was the major objective of the study, an exploration of the attitudes of a sample of nursing home administrators was undertaken in an effort to learn of possible relationships between attitudes and the characteristics of facilities.
