Assessing 4-H And Its Circle Of Courage In A Psychiatric Residential Treatment Facility: A Case Study, Elizabeth Steering

This 4-H case study takes place in a psychiatric residential treatment facility (PRTF) in Kentucky. The PRTF provides clinical services to youth that are not able to be safely maintained in their homes due to having demonstrated unsafe or harmful behaviors. Youth admitted to the PRTF stay for an average of three to six months while they receive intensive therapeutic and psychiatric care as well as medical treatment and public schooling. Racism, discrimination, and oppression further exist at the root of compounding social problems, including limits in minority access to mental health services. The COVID-19 pandemic has also exacerbated mental health problems for young children, especially those who are already underserved and at risk. Approximately two-thirds of U.S. children and adolescents report having experienced a traumatic event. A posttraumatic stress disorder (PTSD) diagnosis requires that the youth has directly experienced a traumatic event, witnessed a traumatic event occur to others, or learned that a traumatic event has occurred to someone close. The clinician is in a diagnostic quandary, however, when the youth presents with the PTSD hallmarks of hyperarousal, avoidance, and emotional dysregulation, in absence of such life experiences. The answer to this quandary lies in an understanding of developmental trauma disorder (DTD). Clinical interventions for DTD are the focus of this capstone.

Despite calls for increased Adoption Communication Openness (ACO) within the adoptive family, research indicates that families still struggle to accomplish the recommended elements and levels of openness. What could be holding families back from this key process? Three focus groups comprised of 17 adoptive parents of early adolescents (aged 10-14) who were age 0-2 at the time of placement were thematically coded. This inductive analysis revealed the complexity rooted in being —sometimes successfully and sometimes not—communicatively open. This study shined light on the gap in literature about the impact of communication and involvement of stepparents has on emerging adult’s romantic attachment styles. A sample of 289 college students between the ages of 18 and 22 at the University of Kentucky was recruited to complete an online survey about their perceived experience with their stepparent and parental figures and the effects on their romantic attachment styles. An alarming portion of youth experience traumatic events during childhood, and there is a robust body of literature documenting the adverse consequences of trauma exposure on the developing child. Fortunately, numerous empirically-supported phase-based interventions have been developed for youth that target the deleterious effects of trauma. While several of these interventions have demonstrated symptom reduction from the baseline to completion of treatment, much less is known regarding the trajectory of posttraumatic stress symptoms (PTSS) during the course of treatment. Of the two-thirds of children who live with their father, only a percentage of them have fathers who report regular play time with their children. Children in the child welfare system comprise a group characterized by their exposure to trauma via experiences of maltreatment, under circumstances presenting multiple risk factors for traumatic stress. High rates of posttraumatic stress have been observed in this population. However, there is currently no standard for the universal screening of children in child welfare for trauma exposure and traumatic stress. The purpose of this study was to analyze the trauma experiences of a sample of maltreated children and examine whether child welfare workers are effective screeners of traumatic stress symptoms with children from their caseloads. Despite the federal government’s $1.5 billion investment between 1993 and 2010 to fund 164 separate community-based systems of care, there has been an extremely limited attempt to measure the impact of system of care. The impetus for this research is the struggle for how the value based concept of system of care is communicated within a community. While child mental health services researchers have published a number of randomized control trials to explore individual level supports for youth served in a system of care community, researchers have struggled to devise a way to measure system of care philosophy diffusion.

During the spring semester of 2010, I have been working on the design of a psychiatric residential treatment facility in Wheelwright, Kentucky as part of the requirements for ID 659, Interior Design Graduate Studio. I am a senior, but was allowed permission to enroll in this class because of my interest in research-based design. The facility will offer therapy services in a home environment for seven adolescents between the ages of ten and seventeen.
