The Communicative Capacities Of The Medical Discourse In Authoritarian Societies : The Case Of Aids In Iran, Elham Pourtaher

This study explores the role of medical discourse in the Iranian formal public sphere. It examines how an epidemiological wave of HIV/AIDS—known as "the third wave"—highlighted nontraditional sexual behaviors in public and enabled a shift in policy and discourse by the Islamic Republic State. Through analyzing published content on HIV/AIDS from five major Iranian newspapers between 2009 and 2013, this study identified four competing narratives of the third wave which coexist and have a dynamic relationship with one another. EXECUTIVE SUMMARYFor decades public administration and management scholars have conceptualized organizational networks as solutions to failures of markets and hierarchies (see, for example, Goldsmith and Eggers 2005; Osborne and Gaebler 1992). Relationships among organizations or actors within a network are framed positively as channels through which human, financial, and knowledge resources flow to address complex or “wicked” social problems (see, for example, Rittel and Weber, 1973). Obesity is a critical health and social concern. Almost 40% of American adults are obese, creating more than $300 billion dollars in annual medical costs. While obesity is a public concern that has been on the policy agenda for some time, obesity policies have proven difficult to formulate, adopt and implement in practice. The United States’ health care system has faced growing per capita use, and subsequent crowding, of the emergency department since the 1990’s, evidenced by longer wait times and the institution of ambulance diversions. In 2008, New York’s health commissioner asked LHDs and nonprofit hospitals to collaborate with community partners to conduct community health needs assessments, identify two or three health priorities to focus on, and implement evidence-based strategies to address them. The actions taken as part of the New York State Prevention Agenda mirrored the IRS rules and PHAB recommendations that followed. The dearth of published studies on public health-hospital collaborations and the momentum to understand how communities in New York have responded to the IRS rules and PHAB standards are the impetus for this research.

Those receiving medical care are at risk for developing serious hospital-acquired conditions (HACs), which include infections (HAIs) and other harm while receiving medical care. Pay for performance programs initiated by the Centers for Medicare and Medicaid Services (CMS), and private payers, linking reimbursements to quality of care, have created financial incentives for hospitals to address these HACs. The most common HAI in the United States, causing almost half a million infections, with 29,000 deaths within 30 days of diagnosis, results from Clostridium difficile. Infection with this bacterium leads to deadly diarrhea, and disproportionately impacts patients 65 and older, representing 80% …

Decentralization has become one of the most important public-sector reform programs globally since the 1990s with mixed results. In this four-paper dissertation, I explored how decentralization reforms affect public goods and services delivery, by analyzing how post-decentralization institutional changes shape local expenditure decisions and health service utilizations in Indonesia. The main goal of this dissertation was to improve understanding on a better approach to studying public goods and services delivery under a decentralized governance. Domestic violence has been shown to increase the risk of bullying behavior among youth. Substance use and excess weight have also been linked to bullying behaviors. The purpose of this study was to determine what anti-bullying interventions have been implemented throughout the United States, and to identify the role of three risk factors (domestic violence, excessive weight or substance use) on bullying behaviors. In the past 20 years, large-scale disasters have generated economic loss of $660 billion in the United States. Research has identified that low income individuals and economically vulnerable communities are at heightened risk and often experience disadvantages in access to public services during emergencies. Public administration literature has claimed that social equity should be included as a consideration together with economic and efficiency principles when formulating and implementing public policy. The New York State Nursing Home Transition and Diversion Medicaid waiver program (NHTD) assists individuals with long-term care needs with the goal of living safely in their communities. Although about 47 million low-income people participated in the Supplemental Nutritional Assistance Program (SNAP) program at the end of 2012, the effect of SNAP participation on some nutrition related outcomes is controversial or unknown. Background. Avoidable emergency room (ER) visits and hospital admissions among people with intellectual and developmental disabilities (IDD) contribute significantly to rising Medicaid costs. The Affordable Care Act (ACA) and New York State (NYS) Medicaid Redesign Team have identified a critical need to reduce ER and hospital utilization in an effort to contain healthcare costs and improve quality of care. Study Objective. This dissertation proposes a conceptual framework that integrates stakeholder and compliance theory and employs a multi-method approach to examine the influence of stakeholders, bureaucratic discretion and external environment on nursing home inspections and enforcement. Negative binomial regression analysis was conducted on a population of 655 nursing homes in New York to identify determinants of variation in compliance and enforcement (as measured by deficiency citations). As hypothesized, organizational characteristics (i.e., nursing home bed capacity, sponsorship, occupancy rates and Medicaid revenue) were significant predictors of regulatory compliance and quality of care. In the United States, drug-free substance abuse treatment programs generally operate under an abstinence-only, disease model based service orientation. Citing several reasons, critics suggest disease model approaches hinder client utilization and retention within such services. The framework of harm reduction offers an alternative approach to substance abuse treatment services and may have potential for improving utilization and retention outcomes. Yet little is known about how harm reduction is perceived by practitioners in drug-free substance abuse treatment settings, and the circumstances in which harm reduction may or may not be accepted.
