Efforts to establish a reliable and valid case definition for myalgic encephalomyelitis and chronic fatigue syndrome (ME and CFS) have been complicated by an over-reliance on clinical consensus, and inconsistent application of established case definitions by researchers across study sites. This has resulted in the absence of an empirically-based case definition for ME and CFS, as well as failed replication studies on potential diagnostic tests and biomarkers. One step toward an empirically-driven case definition is determining which symptoms best discriminate between patients with ME and CFS versus controls. The Fukuda et al. criteria is the most widely used clinical case definition for diagnosing patients with chronic fatigue syndrome (CFS). Despite the frequency with which the Fukuda criteria are applied, the list of symptoms outlined in this case definition were not well enough specified to be easily applied to research settings. In 2005, Reeves et al. laid out a set of standards for operationalizing the Fukuda definition, specifying scales and cutoff scores for measuring the symptom criteria. Fibromyalgia (FM) is a musculoskeletal and neurological disorder that causes constant pain, cognitive problems with short term memory, and sleep disorders. There are more females diagnosed with FM than males, and research has therefore primarily been conducted with females. Because little is known about African American males with FM, biopsychosocial theory was used in this phenomenological study to examine how African American males with FM are affected biologically, psychologically, and socially. Eight African American males diagnosed with FM were recruited from Houston, Texas and were interviewed about their lived experiences with FM, including experiences with the healthcare system, and how …
