Therapists experience thoughts, feelings, and behaviors in response to their clients, which are sometimes referred to as countertransference. Such responses may be influenced by the therapist’s personal history, including the quality of their attachment experiences. Research has demonstrated that adult attachment organizations influence a person’s cognitive, behavioral, and affective responses toward close others, thus providing a useful framework for understanding some countertransference experiences of therapists. This quantitative study sought to add to the existing literature by examining the relationship between therapist self-reported attachment organization and countertransference responses to clients. The purpose of this dissertation is to explore how a mainstream theory of psychological practice might inadvertently conceal and ignore contemporary values and ideologies and their pathological consequences. Through a hermeneutic approach, I interpreted Leslie Greenberg’s Emotion-focused therapy: Coaching clients to work through their feelings (2nd ed), a popular and widely used theory in psychotherapy. As a practitioner with humanistic foundations, this was also an opportunity for the author to understand his own unexamined values as a therapist. This study completed a critical review of psychologists’ mental health by developing a conceptual analysis based on the current empirical literature of the mental health needs of clinical psychologists. Distress among psychologists was explored by examining the following domains: (a) examining the prevalence of mental illness and psychological distress that exist among them, (b) examining the barriers they encounter to seeking treatment when experiencing this distress, and (c) reviewing current interventions and integrating remedies for access to mental health care that best meets psychologists’ needs. There are too few mental health providers to meet the needs of residents of rural communities. Rural inhabitants often present for treatment with severe symptoms, high risk for suicide, comorbid chronic health conditions, and socioeconomic stressors. It is difficult to recruit psychologists to rural communities due to limited training in rural psychology, financial barriers to sustaining a practice, frequent ethical dilemmas posed by small towns, and limited cultural amenities. This dissertation addresses the subject of problems of professional competency within clinical psychology. A review of the current literature on the subject begins with topics including recent changes in terminology, conceptualization, and prevalence of competency problems. Contributing factors, differences between practicing psychologists and trainees, and ethical issues are addressed. The review concludes with current perspectives for assessing and intervening for problems of professional competence. This dissertation examines how the class background of social workers and doctoral level psychologists influences negative countertransference towards working with the poor in an outreach setting. A literature review explores countertransference from a psychoanalytic stance and showcases the development of the two disciplines, psychology and social work, and how class has directly or implicitly been a factor. Finally, the review discusses outreach therapy, its advantages and limitations, and how doing this work can impact clinicians.
