This study examined the relationship of adaptive and pathological (grandiose and vulnerable) expressions of narcissism to attachment style and the capacity for reflective functioning (RF). Narcissism serves a relevant personality construct in clinical theory, social psychology and psychiatry but remains inconsistently defined across these disciplines. Theoretical accounts support the notion that attachment difficulties and maladaptive patterns of mentally representing self and others serve as the substrates for narcissistic pathology but are less pronounced in adaptive narcissism. Objective: To examine the relationship of borderline personality disorder (BPD) to mood disorders by using data from the Collaborative Longitudinal Personality Disorders Study on the reciprocal interactions of BPD with both depressive and bipolar disorders over the course of 10 years.

Method: The study included 223 BPD patients with DSM-IV–defined co-occurring major depressive disorder (MDD) (n = 161), bipolar I disorder (n = 34), and bipolar II disorder (n = 28) who were reliably and prospectively assessed over a period of 10 years between 1997 and 2009. Despite being extensively discussed, the concepts of mental disorder in general and depression in particular remain unclear and no consensual definitions are yet established. Empirical research on how professionals and laypeople think about depression also points to tensions and lack of consensus. However, there still remains much work to be done in order to more effectively and clearly elucidate how depression is conceptualized.
