Perinatal substance use (PSU) is a serious and growing public health concern. It is associated with a variety of adverse health outcomes for both mother and child and has been shown to negatively impact the parent-child relationship. Despite the growing prevalence of PSU, there are notable deficits in provider knowledge regarding, and comfortability with, PSU. Moreover, providers report feelings of judgment, resentment, fear, and hesitancy related to their work with women with PSU. Subsequently, women with PSU struggle to find appropriate, compassionate, and effective treatment for their substance misuse. United States military veterans are a special population of men and women that have willingly sacrificed their lives to serve their country. They are perceived to be patriotic, honorable, strong, and disciplined people. Unfortunately, veterans are not exempt from committing criminal acts that land them in the criminal justice system. This dissertation is a collection of four studies which collectively explore a hypothesized construct of ‘moral evaluation of patient behaviors’ (MEPB) as a driver of health professionals’ readiness to interact humanistically with their patients. In these studies, ‘humanistic interactions’ refer to the non-technical, intangible skills and factors of clinical competence; the factors specifically explored in these studies were compassion toward patients, self-efficacy for treating patients, and optimism toward patient treatment. For the purpose of specificity, all factors were examined as they pertained to patients with substance use disorders. Despite the trends showing a reduction in the use and abuse of drugs among American adolescents, the prevalence rates remain high. There is also comorbidity of mental illnesses among the adolescents using drugs. The aim of this study was to determine the presence and nature of the association between the use and abuse of marijuana and alcohol and mental illnesses among the American adolescent population. The focus of this study was to examine the experiences of two past participants of an athlete-specific drug and alcohol rehabilitation program. Researchers have discussed the vulnerability of an athlete population and prevalence of substance use among athletes ( e.g., Turrisi, Mastroleo, Mallett, Larimer, & Kilmer, 2007; Martens, Watson, Royland, & Beck, 2005). ADHD stimulant misuse is defined as taking a stimulant used to treat ADHD either without having a prescription for that stimulant or in a manner that deviates from the prescription’s instructions. This has been a growing trend among undergraduate students over recent years. Prior research has found that misusers are likely to have severe symptoms of ADHD, misuse for primarily academic reasons, and display problems associated with substance abuse. The Susan B. Anthony Recovery Center (SBARC) in Pembroke Pines, Florida is a residential center where women live with their children while receiving treatment for a variety of co-occurring substance abuse and mental health issues and while participating in mandatory parenting classes. Unlike most women's residential treatment centers, which address only the woman and her problems, SBARC treats the mother-infant/child dyad. I designed and created a database to examine the data previously available only in the paper client records of over 800 women who received treatment at SBARC from 1995 through 2010 in a previous project. Researchers have shown a relationship between mental health disorders and alcohol dependence. However, only 5-10% of individuals with substance use problems co-occurring with mental health problems are correctly identified. The purpose of this research was to identify predictors of relapse using three different instruments of varying complexity: the Patient Health Questionnaire-9 (PHQ-9), the Modified Mini Screen (MMS), and the Millon Clinical Multiaxial Inventory-III (MCMI-III). Researchers have found that using alcohol produces relief, similar to a pharmacological intervention, from troublesome mental health symptoms that individuals experience. Each and every evening many people do not have a home to return to. Solving the epidemic problem of homelessness is an ongoing pursuit. Analyzing issues related to homelessness will help solve the problem of homelessness for some individuals and families. Data was gathered, prioritized and analyzed to determine correlations and relationships between completion of a transformational shelter’s program and mental illness, substance abuse, and trauma. Demographic variables were assessed as well. A person is more likely to complete the program if they have a substance abuse issue. Females are more likely than males to complete the program. In 2011, the U.S. reported 3 million child maltreatment cases, an uncomfortably high but recurring figure each year. Research shows exposure to early life stress (ELS) increases an individual’s susceptibility to substance abuse, specifically of nicotine, alcohol, and cocaine. Increased susceptibility may result from dysregulation of the HPA axis sustaining activation into adulthood after ELS. Hyperactivation of the HPA axis significantly reduces hippocampal BDNF, a neurotrophin involved in neuronal growth and plasticity. Reduced hippocampal BDNF may be a factor in substance abuse vulnerability. Additionally, research shows exercise protects hippocampal BDNF from stress induced down-regulation. Research undertaken in the last three decades has consistently reported that the length of time spent in inpatient and outpatient alcohol and other drug (AOD) treatment programs predicts treatment success (De Leon, Melnick, Kressel, & Jainchill, 1994; Hubbard, Craddock, & Anderson, 2003; Simpson, Joe, Fletcher, Hubbard, & Anglin, 1999). However, treatment attrition rates are high and present a major problem for improving treatment outcomes. Various factors that have been reportedly associated with increased AOD treatment attrition rates include being female, younger clients, clients using methamphetamines, and clients with elevated psychopathology scores. The purpose of this study was to examine the role of children and mental illness in women's compliance in a treatment program for substance abuse. Information was retrieved from medical records of female clients ( N=221) who took part in a community-based day treatment program for substance abuse in a large city in southeastern Virginia during a 32-month period. Organizational commitment (OC) has been linked to different types of work-related behaviors including innovation implementation and professional behavior change but the influence of OC on technology transfer or research to practice processes is as yet unknown. Using a social exchange perspective, the study employed constructs derived from Klein & Sorra (1996) - perceived implementation climate and practitioner innovation-values fit - to understand the association between practitioner level OC and evidence-based practice implementation (EBP) in substance abuse treatment settings.
