Suboptimal treatment response rates in interventions for youth internalizing disorders (anxiety, depression, OCD) highlight a critical need to enhance intervention outcomes. My dissertation project addresses this need by identifying therapeutic process variables that predict intervention outcomes, and examining how, for whom, and under what circumstances they contribute to outcomes. In a series of three manuscripts, I present findings on a systematic review and meta-analysis of alliance-outcome associations in youths receiving intervention for internalizing disorders (Chapter II) and examine variables that may explain or influence alliance-outcome associations in interventions for internalizing problems in university-based (Chapter III) and usual care …

Attention bias modification treatment (ABMT) is a computer-administered intervention informed by theoretical models implicating the role of attentional processes in the etiology and maintenance of anxiety. Attention bias modification treatment presents an innovative, cost-effective approach to meeting demand for treatment of anxiety disorders; however data are limited regarding long-term outcomes and related predictors of ABMT treatment outcomes in youth. Therefore, the present study examined long-term outcomes of ABMT as a standalone or adjunctive treatment in 74 youth referred to an anxiety disorders clinic. Rates of internalizing disorders in childhood are around 10% and higher among racial/ethnic minority youth and youth living in poverty. Targeting empirically derived processes associated with anxiety and depression may be an efficient and effective way to minimize risks for internalizing symptoms and impairment. Deficits in emotion regulation (e.g., increased use of emotional suppression, decreased use of cognitive reappraisal) are associated with anxiety and depression in youth and improve with treatment. The current study examined the acceptability and promise of an intervention targeting these emotion regulation strategies in the context of an after-school music program. Selective mutism (SM), an anxiety disorder most commonly presenting in childhood, is characterized by a failure to speak in certain social situations. Due to its unique presentation (e.g., lack of speech) and low prevalence, expertise in the treatment of SM is scarce, leaving many affected families without access to care. Intensive Group Behavioral Treatment (IGBT) allows families to travel to a specialty clinic to receive a course of treatment in a single week. Objective: Attention Bias Modification Training (ABMT) for anxiety aims to train attention away from threatening stimuli and toward neutral stimuli. Although ABMT shows promising anxiety reduction effects in children and adolescents, no study has examined its influence on neural indicators of attention measured using event-related potentials (ERPs) in children or adolescents (i.e., youths). Research documents considerable anxiety-related heterogeneity in youth with callous-unemotional traits (CU), a pattern of callousness and shallow emotionality (Frick & Ellis, 1999) associated with lasting impairment (Fontaine et al., 2011). This heterogeneity may relate to behavioral differences, with the presence of both CU and anxiety associated with increased questionnaire-based reports of aggression and/or historical documentations of past aggression (Kahn et al., 2013). The present study investigated the efficacies of Individual CBT (ICBT), Parent Relationship Skill Training (RLST, which targets increasing parental acceptance of youth and increasing autonomy granting) and Parent Reinforcement Skills Training (RLST, which targets increasing parental positive reinforcement and decreasing negative reinforcement). The specific aims were to examine treatment specificity and mediation effects of parenting variables. ICBT was used as a baseline comparison condition.
