Three Essays In Health Economics, Mir Nahid Mahmud

This dissertation comprises three empirical essays. The first essay estimates the long-term health and socioeconomic impact of early life nutritional deficit using the Bangladesh Famine from 1974-1975 as a natural experiment. The second essay investigates if pubic health insurance coverage of “ immigrant children¬” changed as a result of the lift of the “five-year-ban” in the Children Health Insurance Reauthorization Act-2009. The final chapter estimates the relationship between child health conditions and maternal employment using conventional econometric models. To expand the analysis and contribute to the literature, I estimate the relationship by implementing a Multiple Indicator Multiple Causes Model (MIMIC).

My dissertation includes three main studies: the first study examines the seasonality of the birth weight and the underlying mechanisms for the seasonality; the second study documents the impact of weather conditions on the gestation period by using the duration analysis and on the birth weight by using the decomposition method and quantile regression; the third study analyzes the effects of density of immigrant population around place of residence on the residents’ health outcomes.In the first study, we evaluate a range of plausible mechanisms that may underlie the seasonality of the birth weight by using U.S. birth certificate records from …

Benzodiazepines (BZDs), a class of sedative-hypnotic drugs, are at the center of an emerging prescription drug crisis. From approximately 1995-2015, overdose deaths involving BZDs quadrupled and average dose equivalents more than tripled. Specific concern has centered on elevated rates of BZD use among individuals with chronic pain, given that BZDs are generally not indicated for pain management. Consistent with negative reinforcement and motivational models of substance use, desire for pain alleviation may be a salient motivator of BZD use, particularly as individuals commonly report using BZDs for negative affect alleviation. The modulation of neuronal cell firing is mediated by the release of the neurotransmitter GABA (γ-aminobuytric acid), which binds to two major families of receptors. The ionotropic GABAA receptors (GABAARs) are composed of five distinct subunits that vary in expression by brain region and cell type. The action of GABA on GABAARs is modulated by a variety of clinically and pharmacologically important drugs such as benzodiazepines and alcohol. Exposure to and abuse of these substances disrupts homeostasis and induces plasticity in GABAergic neurotransmission, often via the regulation of receptor expression. Numerous concerns have emerged regarding the dangers of extended benzodiazepine use and abuse, as well as continued prescribing by medical professionals despite related contraindications. Primary care physicians (PCPs) may find decisions around benzodiazepine prescription and related patient encounters to be especially challenging. The Nebraska Department of Health and Human Services (DHHS) partnered with Support and Training for the Evaluation of Programs (STEPs) at the University of Nebraska at Omaha, Grace Abbott School of Social Work, to complete a statewide needs assessment between October 2018 and April 2019. The purpose of this needs assessment was to gauge the capacity of statewide systems in Nebraska to respond to surges or clusters of intentional, unintentional, and unknown drug overdoses, especially in high-burden areas and with a focus on opioids. The misuse of designer benzodiazepines, as an alternative to prescription benzodiazepines and for drug-facilitated sexual assaults, has emerged as a growing threat, due in part to the ease of purchasing these drugs on the internet at low prices. Causing concern for safety, is the lack of dosage information resulting in users self-medicating, often leading to unintended overdoses, coma, or death at higher doses. The concurrent use of opioids and benzodiazepines (BZDs poses a formidable challenge for clinicians who manage chronic pain. While the escalating use of opioid analgesics for the treatment of chronic pain and the concomitant rise in opioid-related abuse and misuse are widely recognized trends, the contribution of combination use of BZDs, alcohol, and/or other sedative agents to opioid-related morbidity and mortality is underappreciated, even when these agents are used appropriately. Patients with chronic pain who use opioid analgesics along with BZDs have a defined increase in rates of adverse events, overdose, and death, warranting close monitoring. While previous studies find mixed evidence of an association between opioid use and intimate partner violence perpetration among community samples, initial evidence has detected increased rates of partner violence among individuals receiving pharmacological intervention for opioid dependence. The current study evaluated the role of current comorbid substance use diagnoses, a robust risk factor for violent behavior, on the likelihood of perpetrating partner violence among a high risk sample of offenders receiving pharmacological intervention for opioid dependence. This translational research piece involved collaborating with a local community mental health agency to examine knowledge, skills, attitudes, practices, and outcomes for panic disorder treatments. The project included designing and administering an online survey to client care personnel including psychologists, counselors, social workers, nurses, and psychiatrists. Additionally, a database review was utilized to obtain information about treatment modalities, duration, and outcomes. Survey results were analyzed using goodness of fit statistics to show differences between attitudes of participants by discipline regarding the safety and effectiveness of panic disorder treatments. Benzodiazepines are a group of commonly prescribed drugs utilized to manage anxiety, sleep problems, symptoms of alcohol withdrawal, or as a muscle relaxant. Older adults are a common population to be prescribed these drugs, and the prescriptions often come in inappropriate amounts. Long term use of Benzodiazepines increases the risk of negative side effects, such as addiction, misuse, memory loss, and falls.
