Social Cognition Across Eating Disorders: A Systematic Review, Jina C. Kim

Social cognition refers to the cognitive processes involved in social interactions. Deficits in social cognition may play a role in the onset and maintenance of eating disorders (ED). The goal of this review was to examine the current literature on social cognition across EDs, specifically, anorexia nervosa (AN), bulimia nervosa (BN), and binge eating disorder (BED).

The search revealed 79 studies which were organized according to six domains of social cognition: alexithymia, theory of mind, empathy, social processing, emotion recognition, and emotion processing. Anorexia nervosa (AN) is an eating disorder characterized by a restriction of energy intake, an intense fear of gaining weight, and often distorted body image. AN has the second highest mortality rate of all psychiatric disorders, due to high suicide rates and medical complications associated with malnutrition. An estimated 10% of those who have AN die because of the disorder (Insel, 2012). Interacting factors—genetic, biological, environmental, and psychosocial—contribute to the etiology and maintenance of AN. This research investigates the relationship between narcissism and eating disorders (EDs). Two forms of narcissism are studied, as each are present in the individual (O’Brien, 1987). The first form, core narcissism, is major, and refers to the way the individual views themselves while the second form, narcissistic defenses, are minor, serving only to protect the sense of self (Waller et al., 2006; O’Brien, 1987). Core narcissism is exhibited as grandiose or vulnerable narcissism where grandiose includes feelings of entitlement and high self-esteem while vulnerable includes low self-esteem and self-criticality (Maples et al., 2011). Hispanic people are the largest and fastest-growing minority group in the U.S. with a total of 18.5% of the U.S population being of Hispanic descent (United States Census Bureau, 2020). Although eating disorders (ED), particularly those involving binge eating, comprise a significant health concern, EDs are often overlooked in the Hispanic culture (Higgins et al., 2016; Opara & Santos, 2019). Hispanic individuals have comparable rates of anorexia nervosa, bulimia nervosa, and an even higher rate of binge eating disorders compared to non- Hispanic (NH) White individuals (Higgins et al., 2016). Background: Several unsuccessful attempts have been made to reach a cross-disciplinary consensus on issues fundamental to the field of eating disorders in the United States (U.S.). In January 2020, 25 prominent clinicians, academicians, researchers, persons with lived experience, and thought leaders in the U.S. eating disorders community gathered at the Legacy of Hope Summit to try again. This paper articulates the points on which they reached a consensus. It also: (1) outlines strategies for implementing those recommendations; (2) identifies likely obstacles to their implementation; and (3) charts a course for successfully navigating and overcoming those challenges.
