On the 22nd of June 2010 an uncontrolled detonation occurred at [Name of demining group removed] task site DA-SR-3948 in Central Equatoria within the confines of the control point. The detonation fatally injured the team medic and minor injuries were sustained to one of the team deminers’ who was in close proximity to the seat of explosion. While the deminer try to investigate a signal indicated by the metal detector in an expected site of an AP M14 mine, the deminer didn’t approach the signal in the proper procedure and hit the mine with the heavy Rake on the pressure plate which activated the mine and caused the blast 200 away from the deminer

The primary cause of this accident is listed as “Inadequate survey” because there seems to have been confusion about the area that required search. The secondary cause is listed as a “Management Control Inadequacy” because the failure to search the areas that were very obviously hazardous appears to have been the result of poor communication, a lack of adequate task documentation, a failure to keep MDD working records, poor understanding of Technical Survey, and other “shortcuts” that commercial demining groups may be expected to make when inadequately tasked.

Deminers reported that the Victim was in a particularly happy mood that day. The Victim went to the grenade without telling anyone and began to dismantle it. His actions can be reliably inferred because the black cap of the grenade was undamaged at the site (and featured in the first site photographs after the accident). On 04 May 2010 at 08:45 am while [the Victim] the de-miner was excavating a detected signal in his clearance lane, his bayonet hit top of an anti-personnel PMN mine and caused it to go off. The team leader [the Victim] was doing a QA check on one of the Foot Track already cleared by the deminer. Out of five mines laid only one M14 AP mine has been recovered from this FT. The team leader was trying to recheck the cleared area again with the metal detector trying to find the missing mine, during his QA check he stepped on one deep buried M14 AP mine in the area which has been already checked by the deminer and Team Leader.

On the 16th March 2010 at [International demining organisation] task CBU 478, an uncontrolled detonation of a US M series M77 sub-munition occurred while [International demining organisation] searcher [the Victim] was conducting sub-surface instrument search.

[The Victim] sustained fragmentation injuries to his right leg in the thigh and lower leg. When he stood on an unstable rock in his clearance lane, this caused a sub-munition underneath the rock to detonate.

According to the investigation report the de-miner was using the scraper direct on the top of the detected signal causing it to go off. It was then found that the exploded item was a MUV fuse. As a result of the explosion, [MUV fuze] sustained injuries on his eyes, right side ear, arm and head; the injuries indicate that the de-miner was not using his visor properly.

The Deminer was trying to check signal using the heavy rake, the M14 mine was laid in a grassy solid ground full of grass root and stones, the deminer tried to drew the grass by the heavy rake, the roots of the grass push the pressure plate of the mine, that cause the mine to blast.

The Deminer today was officiated as a team leader due to the absence of the team leader. He took one deminer tools and started to work himself in one of the remaining cluster in his section. While he was using the grass cutter to cut the grass in front of the base stick he hit the barely covered M14 mine by the grass cutter which caused the mine to initiate.
