Background: The United States Army is implementing the Holistic Health and Fitness program (H2F), intending to transform the Army's culture of health and fitness. It aims to optimize soldier readiness by improving physical and non-physical performance, decreasing injury rates, and improving rehabilitation after injury. A part of that program includes replacing the current Army Physical Fitness Test (APFT) with the new Army Combat Fitness Test (ACFT). The ACFT consists of six functional events that gauge combat readiness. There is already established literature regarding injury epidemiology as it relates to the previous APFT, but little published work on the ACFT …

Several Supreme Court rulings, starting with Buckley v. Valeo and culminating with Citizens United v. Federal Election Commission, have created two unequal campaign finance systems: One where party and candidate committees must follow strict fundraising regulations from the Federal Election Commission, and another populated with Super PACs and similar organizations which are allowed to take in unlimited donations. This paper sets out to see if Super PACs, which are freer to operate under the law, have challenged the party and candidate committees’ agenda-setting ability in an election.

Introduction: The Affordable Care Act (ACA) increased access to health insurance throughout the United States. To date, an estimated 20 million previously uninsured individuals have gained access to coverage since the expansion. As a result, the number of fraudulent schemes reported has been on the rise. Among the many fraudulent activities in the healthcare sector, abuse of pharmacy benefits has been the most prevalent. The misuse and abuse of opioids, and opioid related overdoses has created a widespread epidemic throughout the country, thus extending opportunities for potential fraud within the pharmaceutical industry.

Strategic placement of ambulances is important to the efficient functioning of emergency services. As part of an ongoing collaboration with Wayne County 911, we developed a strategy to optimize the placement of ambulances throughout Wayne County based on de-identified call and response data from 2016 and 2017. The primary goals of the optimization were minimizing annual operating cost and mean response time, as well as providing a constructive solution that could naturally evolve from the existing plan. Medicine 2.0 has emerged within healthcare information technology to enable more defined relationships among providers and patients. Physicians, hospitals, and patients are using Medicine 2.0 through social networking to maintain their foothold in the evolution of medical technologies. The authors’ purpose was to determine potential improvements that Medicine 2.0 has on communication and collaboration of healthcare information. Research has shown that Medicine 2.0 has integrated into the healthcare industry and is enabling an increase in communication in healthcare matters. The provider-patient relationship is improving through the use of Medicine 2.0 and has positively impacted society so far.

INTRODUCTION: The United States has spent approximately $2.6 trillion on healthcare in recent years. This accounts for 17.9% of the national gross domestic product. Fraud has been identified as one of the leading causes of the nation's increasing health expense. Medicare fraud has taken many forms including overutilization, upcoding, billing for services not provided and filing false cost reports. Fraud has been found throughout the healthcare industry and has been difficult to detect.

When it was introduced in the Affordable Care Act of 2010 as the new 2012 payment model for Medicare, an accountable care organization (ACO) was a new and untested concept in healthcare delivery and payment. The authors estimated the likelihood of engagement in ACOs by small group and solo healthcare practitioners. An evaluation of five case studies showed that significant organizational, financial, and technological challenges had to be met in order to launch an ACO. Health care disparities in rural areas remain significant in the U.S. health care industry. Uncompensated care makes health care disparities in rural areas worse, and rural hospitals are unfavorably positioned to compete with urban hospitals in the economic-downturn marketplace. How uncompensated care affects quality care among rural hospitals has been lightly investigated. In 2008, health care spending in the United States reached $2.4 trillion dollars, and is projected to reach $3.1 trillion in 2012.During the past decades, there has been a steady increase in the utilization of expensive inpatient imaging studies, with an overall increase in health care costs. Significant gaps exist in health care regarding gender in the United States. Health status, social roles, culturally patterned behavior and access to health care can be influenced by gender. Women have been the primary users of health care and minority women usually have received poorer quality care than Non-Hispanic White (NHW) females. The objectives of this study were to identify gender, racial and ethnic disparities in access to substance abuse treatment in a Texas hospital. Secondary data collected on 1,309 subjects who underwent detoxification were studied. Gender, race/ethnicity, drug of abuse, relapse and financial classification were included in the analysis. …

Cultural aesthetics are the latent effects of human relations informing cognitive schemas as cultural variations of social forms in specific time-space contexts. To understand what conditions produce intra-national conflict during wartime, engagement reactivity between social control mechanisms and antiwar protesters was measured. Hypothesis-1 showed high numbers of arrests were influenced by the type and duration of protest and military presence at protest events during Vietnam, whereas place and size of protest were influential during Iraq. Once the heart of a town’s commercial district, Main Street in the post-World War II era became a ghost town as suburbanization lured consumers and merchants to new shopping complexes in rural areas. From the 1950s to the 1970s, with the aid of federal dollars cities embarked on various urban renewal projects. These revitalization attempts did not help the slumping commercial districts. This thesis examines President Ronald Reagan’s poverty discourse during his first term. Using the work of Walter Fisher, this study examines Reagan’s narrative rationality in constructing images of welfare recipients and the welfare system in the justification of his policies. By identifying links between Reagan’s rhetoric and the rhetoric of debates leading to the passage of the 1996 Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA), this thesis supports Asen’s (2002) claim that Reagan created a "discursive context" that made passage of PRWORA possible. Medicaid-funded long-term care services are traditionally delivered in nursing homes. States may apply for waivers which allow them to provide home and community based services with Medicaid funds. Because these services are by definition an alternative to nursing home care, waiver services are generally based upon a medical model which emphasizes medical deficits and tends to restrict consumers’ movements to inside the home.
