[The Victim] was going away from the lane. At about 10 m distance from [Name removed], who approached the base line, he slipped with his left foot below the tape into a non-examined part of the minefield and activated a PMA-3. The working path was narrower there because of a larger rock. After the explosion, [the Victim] fell into the working path, but with his legs lying in the non-examined part. Deminer [Name removed] pulled him out into the cleared part. The tip of the left shoe was damaged. It was taken off.

Approximately 70cm past this 1.2m x 1.2m area was a visual PMR2A. [The Victim] had an indication from his metal detector and was investigating this reading. On the morning of the accident a pile of mines were discovered at the base of a large tree approximately 7 metres before the seat of the explosion, this pile consisted of 8 x PMR2A, 1 x PMA1 and 1 x PMA2. This minefield is PMA 3 Anti Personnel Blast minefield with the mines laid in a very dense pattern. All minefields in the Koshare area are cleared by prodding and excavation drills [detectors not used]. At 10:55 whilst Victim No.1 was conducting manual clearance, he stepped over his base stick and detonated a PMA3 blast mine.

The victim insisted that he help the International Supervisor with carrying the sticks into the mine rows and assist him with the marking. They had been in the lane for less than 10 minutes conducting marking, when the victim with a club hammer in his hand was bending down to pick up a marking stick. It was at this time that he stepped on a PMA-3 blast mine in what he thought to be the 10cm safety overlap.

At 11:35 Victim No.1 moved to edge of the cleared area to look at the mine he had seen and trod on a PMA-3 inside the area marked as cleared. "The victim walked/ran by himself to a safe area" where other deminers made him lie down and calmed him. The medics arrived and Victim No.1 was carried to the ambulance. He left for the Argentine KFOR hospital at 11:45 and arrived as 12:10. He was conscious at all times. The hospital released him "Later in the afternoon".

Anxious to speed up progress, the victim decided to carry out a "reconnaissance". When they moved to the second site, they sent for a local villager to show them where the mines were. The villager led them along a trail where they found parts of tripwire and PMA-3 but could not locate the "minefield pattern". The victim and his partner went off the trail and found evidence of a PMA-2 detonation, then an intact PMR-2A and two PMA-3 mines.

The victim, was clearing a lane past the remains of a dead cow and was already a metre past the cow when the accident occurred. While the victim was sweeping with the mine detector he received a signal on the right 10-cm overlap of the lane he was busy clearing. The ground at the accident site sloped slightly to the right of the lane. The victim started to prod in the area of the signal by using the prodder. When he was prodding on the right of his base stick, he activated an explosive device. This occurred at approximately 13:00.

The victim's partner found two PMA-3 mines and the Team Leader disarmed them and put them in a "mine storage area". The victim took over demining and after 4 minutes detonated a mine. He was squatting or kneeling and prodding at the time. He remained conscious but he had suffered damage to his eyes "due mostly to dirt coming up under his visor". He was evacuated to hospital "immediately", arriving after 18 minutes.

The team were finishing their shift for the day and the victim was asked to mark the edge of the area that had been surveyed that day. As he walked to that point he trod on an undetected PMA-3. The victim was later told that the mine had been laid too deep for the detectors to locate. The victim was wearing military boots, leggings, a frag-jacket, and a helmet & visor.

The document stated that the victim had been recently trained by the group he was working with. He was working in an area known to contain PMA-1 and PMA-3 mines. He was not wearing a helmet and visor and was prodding for mines. He was prodding with "a vertical stabbing motion" when he detonated a PMA-3.
