The Section Leader was kneeling, or squatting, over the mine. When it exploded, his body took the full impact of the blast from a very close proximity. This is substantiated by the extensive blast damage to his remains and extensive burning of the remains from the blast. The team arrived in the local, approximately 10:40, and tried to park the vehicles in the usual parking area. The right rear of the truck set off an AT mine. The truck was projected to approximately 7 metres high. In the same time five occupants at the back of the truck were projected violently due to the blast effect. [Nine named victims were recorded in the Demining group’s accident spreadsheet, from which I infer that four were in the cab.] Six victims were listed as having suffered no injury.

The Managers turned off the track and drove up to the fence. In doing so they drove straight through the minefield followed by the victims in their vehicle. The lead vehicle drove through three or four rows of VS1.6 anti-tank mines. The following vehicle, with the office staff and the only vehicle radio, activated a VS1.6 anti-tank mine as it followed.

The demining group were clearing an 80km stretch of road using mine-dog teams and manual demining methods. They began work in November 1999. Work was suspended during the period of National floods, then restarted. They had cleared “approximately 43km” at the time of the accident. During the clearance task the demining team had found “1 UXO, 377 SAA of AKM” and 1724 pieces of metal.

The mine accident happened at 10:00 hrs on the 3rd July 2001 while the [Demining group] were conducting manual clearance operations at minefield GY002. The deminer had a signal when using his mine detector which he investigated with a prodder and then excavated using a trowel. After exposing the item that had given the signal the deminer removed it from the ground using his right hand. The item exploded.

At approximately 11:00hrs on 20 February, a left hand drive Land Rover with add-on armour for mine protection, crewed by a [Demining group] Survey Team struck a landmine. The vehicle’s front left wheel area and part of the engine were extensively damaged. There were no serious injuries.

[Victim No.2] and [Victim No.1] marked the place with a red-top picket 1.2m high (to be found later in the vicinity of the accident spot) and asked [the Local witness] to help them identify the mine. [The Local witness] refused and went to [RS MAC Ops Officer No.2], who handled VALLON, trying to locate where other mines are. 2-3 minutes after he left the site of located mine, there was an explosion at the crossing of trenches.

The site of the accident was indicated by two dogs and the two victims moved forward to investigate. Victim No.1 began to excavate at the site with his pick. After some time he changed with Victim No.2. Without waiting for Victim No.1 to withdraw, Victim No.2 set to work with the pick and “immediately” initiated an M19 mine.

The report recorded that the victim "accidentally detonated an anti-tank mine while preparing for the testing of the newly purchased Hydrema mine clearance machines". The victim was "deploying and arming" mines in a marked test ground and wore a "ballistic jacket and visor". He worked alone arming the mines at a distance of 300 metres from the nearest person (a medic). It is not recorded whether this mine was the first to be armed. As the officials were pulling the destroyed vehicle away over another area of road (cleared by a mine-dog group in 1997) one of its wheels detonated another AT device at the side of the road. Three people were injured and the vehicle was "further damaged".

The victims were in a vehicle that detonated an AT mine with a front wheel. The ground was a dirt road in grazing land. A photograph showed a flat earth area with water in puddles. "The front wheel of the truck and the cabin were destroyed". [A photograph showed the cab separated from the truck and severely damaged.]

[Victim No.2] started the clearance and was alone inside the tank at first. As it entered the area it set off a tank mine and the roller system was thrown backwards with far more force than anticipated, hitting the tank. The detonation made the driver’s view ports of armoured glass muddy, so [Victim No.2] called a deminer to clean it. The tank progressed and initiated another three AT mines.

An experienced ex-pat deminer was working on manual clearance of PTMIBA-III AT mines. The mines were surface laid and in a regular pattern. He had disarmed several that morning and had changed with his No2. He returned after a rest break and approached the next mine in the row. He was witnessed approaching the mine, bending down and it is believed he had a tool in his hand. He reached the mine and his partner reported that he was brushing the sand from the top of the mine when it detonated.
