The primary cause of this accident is listed as a ‘Field control inadequacy’ because the person in charge at the site had decided to move fuzed explosive hazards and did not enforce safety distances that would have reduced the casualty toll in the event of an accident.

Three workers of the [International demining organisation] have been killed and two injured in a mine explosion in Azerbaijan's separatist Nagorno-Karabakh region. All the victims were local residents.

Employees of the [International demining organisation] … were in a vehicle conducting surveys when the explosion occurred. Two individuals were also wounded and have been taken to hospital with both requiring surgery.

Two Cambodian demining experts were killed in Battambang province’s Samlot district on Saturday after detonating an anti-tank mine while carrying out clearance work in the former Khmer Rouge stronghold.

At between 2100 hours and 2200 hours on Wednesday, 29 February 2012 there was an undesirable explosion in a containerised ammunition storage site at the Daphniya Police Station Verbal evidence from the local population suggests that the explosion occurred as a result of an individual firing either an Assault Rifle (calibre unknown) into the padlock of the container, or an RPG-7 fired into the container.

GPM Support vehicle drove to the machine to handover the replacement fuze; the vehicle then reversed out back down the 'cleared lane' towards the 'safe area' and veered off this lane into an uncleared area (GPM skip area). The GPM Support vehicle had a mine-strike approximately 1m into the uncleared area. On Saturday 02 October 2010 at approximately 10:00am a [Civil Contractor] grader struck an AT mine approximately one meter from the road center-line whilst carrying-out maintenance on the Juba to Nimule Road. The grader operator suffered serious injuries as a result of the blast. He was evacuated to a hospital in Juba, stabilized and then as he was from Uganda he was evacuated the following day to Kampala for further treatment. The grader operator later died from his injuries 20 October in Kampala.

On 13th August 2010 an Al Lafa, Kassala farmer was ploughing his field by tractor at approximately 2300hrs, when he detonated an antitank mine. The left rear tyre was destroyed, luckily he survived with surprisingly minor injuries. After some initial confusion as to whether the mine was or was not in a cleared area (perhaps a data reading error) and a preliminary investigation, it was established that the site had been previously cleared. BOI 2010/03 was initiated to determine the facts and make recommendations to the mineaction community.

On 7th July 2010 at 08:30 KAB 174 Rippers FEL of [Demining group] MDU-05 started ground processing operations in MF 0351, supporting MDG-20 in their clearance operations. The burner prepared one MK5 stuck mine inside the stuck mine burning pit; he burned the mine in the right procedure and lift the burning area to a safe distance. 22 October, 1045 hrs, as the tractor moves back down the track after completing the first plough run an explosion occurs under the rear left wheel, this splits the tractor in two and kills the driver. After the inhabitants of the Milon village requested AMAC Gardez for clearance of this mined suspected road, the AMAC deployed a LIAT and subsequently a survey team to this area. The technical survey of this minefield completed on 14 June 2005 and as it was antitank mine, on 12 July 2005, [Demining group] # 15 was tasked to clear it. After clearance, it was handed over to local people. The involved tractor was carrying construction material to one of the inhabitant’s house but before arriving to the destination, it has been hit by a mine.

On the 6th of April 2004 at 1252 hours the WMF Wolf MPV, assisting in route clearance operations on the road from Chiede to Malungo wa Shikongo, set off an anti tank mine. At some time between 05:50 and 05:55 whilst travelling south, the rear right wheel of the 110 detonated an AT mine. The vehicle was thrown forward and through an arc of 180° coming to rest facing approximately north 5 metres from the point of impact.
