On the morning of 21st May 2007 at approximately 0630hrs [Demining group] BAC Team 2 re-deployed back to CBU 040 to start operations. At approximately 1140hrs, an uncontrolled explosion occurred at the building site adjacent to the rear of the BAC Team 2 CP, UTM 712082/3681844, involving a civilian worker, Mr. [the Victim], who was delivering building material to the construction site. Activity being performed when accident occurred: [the Victim] was performing excavation with a bayonet probe (prodder) from left to right (left handed) and hit a PMN mine. Activity being performed when accident occurred: According to [Demining group] Field Officer, Mr. [Name removed], both deminers were together on the base stick when the accident occurred because acting SL [Victim no.1] was verbally instructing Deminer [Victim no.2] how to perform excavation. Deminer [Victim no.2] was performing excavation with a bayonet probe (prodder) and hit a PMN mine. The land owner, [Name removed], immediately contacted the CLO area 1-5 on the 5th May 2007 to report the location and the events surrounding the incident. This site was visited by MACC SL Operations, QA and the CLO to conduct an investigation of the event surrounding the discovery of the sub-munitions.

After the inhabitants of the Milon village requested AMAC Gardez for clearance of this mined suspected road, the AMAC deployed a LIAT and subsequently a survey team to this area. The technical survey of this minefield completed on 14 June 2005 and as it was antitank mine, on 12 July 2005, [Demining group] # 15 was tasked to clear it. After clearance, it was handed over to local people. The involved tractor was carrying construction material to one of the inhabitant’s house but before arriving to the destination, it has been hit by a mine.

A. On 19 April 2007, while the [International demining NGO] loader (MDU # 9) was excavating the ground by using Gill bucket and then the soil inspecting for Antitank mines and UXO but an antipersonnel YM-1 mine has passed through the grids of the Gill bucket and missed from inspection. While the Loader has been levelling the loose dragged soil, in reverse position the front left was tyre driven on the mine and exploded it.

On 19th April-07 at 08:10 hrs while the Deminer was excavating the area by pickaxe suddenly an anti personal mine has hit by pickaxe and exploded caused the accident and Deminer got superficial injuries on his right arm and neck. During our observation from the accident point it was seemed that the Deminer was working with pickaxe not by bayonet/prodder, because the pickaxe handle was available in the accident point.

On the inspection of the incident site on the 17th April 2007 the following observation was established: The sub-munition was located on the surface, in a declared, sub-surface cleared area. The sub-munition was removed from a mound of soil that had been excavated from an area adjacent to a road, during excavation of a retaining wall and then the sub-munition was placed in this area. CBU 19 was completed on the 31stMarch 07, see annex A for the [Demining group] Completion Report. According to the Completion Report the task was a combination of surface and sub-surface clearance in accordance with the operational clearance plan. On 10 April 2007 at 1155 hrs while deminer [the Victim] with employ No. 4094 was working in his clearance lane after 3 m progress in the lane his trowel hit top of a PMN-02 anti personnel mine, as a result the mine exploded and the deminer sustained some injuries. Since the deminer was fully dressed with PPE and down visor, the deminer injuries are superficial. The victim right hand wrist and two fingers were injured.

On the morning of the 9th April 2007 the land owner had contracted a local farmer to commence cultivation of his land for follow-up planting, during the process of the cultivation the operator of the machine noticed on his return ploughing an M77 sub-munition, had been turned over and was now quite visible on the surface on the newly ploughed area. He immediately stopped work and report the incident to the land owner who in turn informed {Demining group] personnel in the area.

On 03 April 2007, one M-77 Submunition was located by a local farmer while harvesting his orange orchard and banana plantation, in the vicinity of Task CBU-203, which had been completed by [Demining group] using sub-surface Battle Area Clearance (BAC) procedures. On 24 May 2007, in this area while the SITA construction company was levelling the roadside (road shoulder), the top of a TC-2.4 (Italian Antitank) mine has been appeared. The company’s representative reported that case to the AMAC and the AMAC sent the QMA to the area for further investigation. The mine found 80 Cm from the paved road in had 90 Cm depth. According to local reports, after mine plantation in this area, about 100 Cm extra soil has gradually aggregated over the original surface of the ground.

The accident demolition was the first that day in a row of six demolitions. The target was a 120mm artillery shell. Beforehand we had digged a 1.1 m deep pit away from the closest village and had sandbagged the side towards the village. On 14 March 2007, a local farmer discovered an M-77 submunition while harvesting banana trees in his plantation, which had been cleared by [Demining group] using sub-surface BAC procedures. A mine has been discovered by the labourer of Mohammad Bin Rashid Housing Scheme in cleared task # 24/2413/036 of Salihan village, Panjwaee district, Kandahar province, while he was digging the ground for the foundation of building. The deceased deminer detonated the PMN mine with an unidentified object. He was in a kneeling position with his right side facing towards the mine. The blast struck him on the chest and right hand side, causing a fatal wound to the right under arm (armpit). He suffered extensive bleeding from this wound and also from the mouth and nose at the time. He was evacuated by ambulance but died en route to the hospital.

A mine blast incident occurred at approximately 07:27hrs on Monday 05/02/2007 in the lane No-2 in the minefield 71390013. The deminer detonated a No-10 Israeli anti personnel blast mine due to hacking. At the time of the incident the deminer was wearing the PPE (Body vest and Goggles). The deminer clearing the mine belt hit the mine on the top by heavy rake resulted in a mine blast. The deminer sustained THREE MINOR INJURIES IN UPPER LIP AND LEFT CHEEK/CHIN.

The 26th of January 2007, approx 12.25 (local time SL), the HQ in Jaffna was called up by the Team leader of Team 5, working in a heavy dense (P4s) minefield, LK-23B.The message he gave was that one of his deminers had set one mine of in his lane while conducting mine clearance.
