The Massachusetts Patient Centered Medical Home Initiative (MA PCMHI) is a statewide, multi-payer demonstration that seeks to transform 46 primary care practices into PCMHs. Clinical care management of high-risk patients is a key element of the PCMH and a new service for most primary care practices. Training for practices in the MA PCMHI includes the clinical care manager (CCM) role, identification/tracking of highest-risk patients, care plan development/implementation, care coordination, and communications. Content is delivered through learning collaborative sessions, monthly webinars and practice-based transformation facilitation. The 2011 SOGC clinical practice guideline “Prenatal Screening for Fetal Aneuploidy in Singleton Pregnancies” recommends that clinicians offer prenatal screening to all pregnant women and provide counselling in a non-directive manner . Non-directive counselling is intended to facilitate autonomous decision-making and remove the clinician’s views regarding a particular course of action . However, recent research in genetic counselling raises concerns that nondirective counselling is neither possible nor desirable, and that it may not be the best way to facilitate informed choice . This article presents a brief overview on the importance of adopting healthy behavior during and after the completion of primary cancer treatment. Increasing evidences are advocating physical activity engagement in cancer survivors due to its convincing beneficial outcomes. Today, outcomes from numerous trials confirmed the need to examine beyond physical activity engagement, into physical inactivity as an independent factor for cancer recurrences. range from patient-related, through institutional-related to staff-related factors. This paper highlights the low response rate and the recruitment barriers faced in our Qigong exercises trial. Materials and Method: The Qigong trial is a three-arm trial with a priori power size of 114 patients for 80% power. The University Malaya Medical Centre database showed a total of 1,933 patients from 2006-2010 and 751 patients met our inclusion criteria. These patients were approached via telephone interview. 131 out of 197 patients attended the trial and the final response rate was 48% (n=95/197). Objective: This feature article on knowledge transfer presents principles and strategies to support the development of short communiqués to end-users. Participants: Formal and informal knowledge brokers are the targeted users of the strategies. Methods: Research studies and conceptual literature in knowledge transfer informed the development of brief-Knowledge Transfer (b-KT) principles. Principles are explained and a sample of how they informed the development of KIT-Tip Sheets is offered to promote ways to use principles in knowledge dissemination. Current literature demonstrates a significant investigation of chronic pain. The actual mechanisms that promote adaptation, accommodation, and change, however, are not explicated in the current literature on chronic pain. Our goal was to conduct a mini realist synthesis approach in current literature to identify mechanisms critical to promote return to work (RTW) for persons with chronic pain. Specifically, we sought to identify if the mechanisms of these programs are consistent with an occupation-based approach used in Occupational Therapy (OT).

Why does child malnutrition persist in India? Amongst the fastest growing economies over the last two decades, India has struggled to make progress in the health of its children. In this article the author argues that the reason malnutrition persists is not limited to poverty or inadequate access to food; but that a lack of nutritional knowledge amongst families plays a very important role.
