As cultural diversity continues to rise globally, there is an evident need for culturally competent care, especially within the client-centered field of occupational therapy. Previous research has shown that occupational therapy doctoral (OTD) students who participated in international service-learning demonstrated an immediate positive impact on their cultural competence. The current study revisits these same doctoral students to examine the long-term impact of international service-learning experiences at six-month, one-year, two-year, and three-year intervals following the initial experience. The purpose of this study was to evaluate the impact of an international fieldwork placement on occupational therapy doctoral (OTD) students’ cultural competence, community engagement, and ethical leadership. This study used a mixed methods approach. The Public Affairs Scale (PAS) was used to gather pre and post placement data on cultural competence, community engagement, and ethical leadership. Qualitative data was themed from a reflection paper along with researcher field notes from a fieldwork placement to Ibarra, Ecuador. Eleven OTD students participated in the study. Effective communication between occupational therapists (OTs) and their clients is key to quality, client-centered services. Most OTs can expect to work with clients with Limited English Proficiency (LEP). Language barriers may negatively impact client safety and client satisfaction. Working with language interpreters is a key means for OTs to best serve clients with LEP; however, few OT curricula provide adequate training in working with interpreters. As the population in the United States increases, it is essential that occupational therapy students and practitioners be aware of, respond to, and effectively interact with clients from diverse cultural backgrounds. Academic programs commonly face challenges in developing Level I fieldwork where students have ample opportunity to practice and understand occupation. In response to this challenge, our academic program developed a non-traditional Level I fieldwork. The purpose of this study was to understand the student perceptions of this learning experience. Focus group methodology was employed. Fifty-nine students participated in one of six focus groups. Six themes were identified through the data analysis process. As society trends towards increased cultural diversity, the need for cultural competence in the field of occupational therapy (OT) becomes more acute. Immersion in another culture within the curriculum offers the student an opportunity to put their knowledge into practice and recognize the need to competently interact with members of another culture.
