A significant number of Acute Myocardial Infarction (AMI) patients were readmitted to an urban San Francisco Medical Center within 5-7 days post discharge this year. Two of the main identified causes were symptom management issues and medication instructions, both of which are part of the discharge instructions. It’s not surprising that 80% of all discharge teaching is forgotten by patients by the time they hit the parking lot. With the recommended timeframe for post discharge follow up appointments at 48 to 72 hours post discharge and as those appointments are not typically available within the recommended timeframe, patients are more …

Background Weight gain during the childbearing years and failure to lose pregnancy weight after birth contribute to the development of obesity in postpartum Latinas. Methods Madres para la Salud [Mothers for Health] was a 12-month, randomized controlled trial exploring a social support intervention with moderate-intensity physical activity (PA) seeking to effect changes in body fat, fat tissue inflammation, and depression symptoms in sedentary postpartum Latinas. This report describes the efficacy of the Madres intervention. Results The results show that while social support increased during the active intervention delivery, it declined to pre-intervention levels by the end of the intervention. There …

Struggling To Maintain Occupation While Dealing With Risk: The Experiences Of Older Adults With Low Vision, Debbie Rudman, Suzanne Huot, Lisa Klinger, Beverly Leipert, Marlee Spafford

The primary aim of this descriptive phenomenological study was to describe the core aspects of living with low vision in later life among older adults (aged 70 years and older) who had not accessed rehabilitation services for low vision. Thirty-four older adults from urban and rural areas participated in a semi-structured qualitative interview and a telephone follow-up. Drawing on an occupational science perspective and using Giorgi and Giorgi’s (2003) method of analysis, the essence of the experience of living with low vision was identified as struggling to maintain valued and necessary occupations while dealing with risk. BACKGROUND
- Older adults are at increased risk for falls and fall related injuries, leading to loss of independence, disability, and increased mortality.
- Preventing injury and falls is a national Healthy People 2010 objective. However, physicians and other health professionals frequently fail to incorporate known evidence into usual care. Screening for falls is not consistently performed as an essential assessment in older adults by primary care providers.
- Multidisciplinary assessment can be an important component in preventing falls in community dwelling adults.

A 1999 evaluation of case studies performed by staff from the Institute of Medicine found that between 40,000 and 98,000 patients died from preventable errors, while 43,598 individuals died in car accidents that year. A 2011 report increased that estimate nearly 10 times. Widespread preventable patient harm still occurs despite an increase in healthcare regulations. High-reliability organization theory has contributed to improved safety and may potentially reverse this trend. This explorative single case study explored how the perceptions and experiences of nursing and respiratory staff affected the successful transition of a healthcare organization into a reliability-seeking organization. Hospital length of stays are decreasing throughout the U.S. in an effort to contain healthcare costs, but these measures can be detrimental to patients. Those who get knee replacement surgery are finding their length of stay reduced from an average of 3 to 4 days to 24 to 48 hours, which can affect pain control and interfere with rehabilitation and recovery. This affects many people, as more than 632,000 knee replacements occur each year (American Academy of Orthopedic Surgeons, 2008). Effective goal setting involves collaboration between the client and therapist and is an important component of occupational therapy practice. However, encouraging involvement and collaboration does not necessarily guarantee that client goals are incorporated into the treatment plan. The purpose of this innovative treatment program was to determine if providing a client with a venue for goal identification, documentation, and maintenance might impact participation and satisfaction in a day rehabilitation setting. The purpose of this quality improvement project was to increase physical activity among postoperative adult liver transplant patients, improve documentation of daily activity, and ultimately influence quality of life (QOL). Quality of life has been shown to improve dramatically after liver transplant, as patients enter transplant severely debilitated with limitations on their physical activity which carry over following transplant. The literature supports that liver transplant patients should engage in physical activity which may improve QOL. Thirteen liver transplant patients were recruited within the seven days of their post-operative hospitalization. Twelve patients consented, were educated about the benefits of walking, given …
