Primary Care Physician Engagement In Health System Transformation: A Case Study, Atharv Joshi
Primary Care Physician Engagement In Health System Transformation: A Case Study, Atharv Joshi
Electronic Thesis and Dissertation Repository
Primary care physician engagement is essential in enhancing organizational performance, system efficacy, and provider collaboration; yet limited literature exists within health systems research. This thesis explored physician engagement throughout the development of an organization known as the London Middlesex Primary Care Alliance (LMPCA). This thesis used a qualitative case study approach guided by a constructivist paradigm. The findings revealed six themes which contributed to the development of the LMPCA and provided insight into how physicians were engaged within health systems work. Unique to this study were two facilitators for engagement: the role of a transformation lead and the use of …
Developing Artificial Intelligence And Machine Learning To Support Primary Care Research And Practice, Jacqueline K. Kueper
Developing Artificial Intelligence And Machine Learning To Support Primary Care Research And Practice, Jacqueline K. Kueper
Electronic Thesis and Dissertation Repository
This thesis was motivated by the potential to use "everyday data", especially that collected in electronic health records (EHRs) as part of healthcare delivery, to improve primary care for clients facing complex clinical and/or social situations. Artificial intelligence (AI) techniques can identify patterns or make predictions with these data, producing information to learn about and inform care delivery. Our first objective was to understand and critique the body of literature on AI and primary care. This was achieved through a scoping review wherein we found the field was at an early stage of maturity, primarily focused on clinical decision support …
Development Of A Multi-Factorial Data Quality Score For Primary Care Electronic Medical Records, Kathryn Stirling
Development Of A Multi-Factorial Data Quality Score For Primary Care Electronic Medical Records, Kathryn Stirling
Electronic Thesis and Dissertation Repository
As healthcare providers transitioned from paper-based records to electronic medical records (EMRs), researchers gained access to more patient and care data, with greater detail. Current research regarding Canadian primary care EMR data suggests the quality of data is variable. For researchers who wish to use EMR data, it is important to have a method of evaluating data quality that is applicable to multiple EMR datasets so research quality can be assured. There is currently no unified scoring system for primary care EMR data. This thesis built on previous EMR data quality research by developing and testing a composite measure of …

BACKGROUND: Access to primary care outside of regular working hours is limited in many countries. This study investigates the relation between the after-hours premium, an incentive for primary care physicians to provide services after hours, and less-urgent visits to the emergency department in Ontario, Canada.
METHODS: We analyzed a retrospective cohort of a random sample of Ontario residents from April 2002 to March 2006, and a subcohort of patients followed from April 2005 to March 2016. We linked patient and primary care physician data with emergency department visit data. We used fixed-effects regression models to analyze the association between the …

Psychotherapy In Family Medicine, Saadia N. Hameed
Electronic Thesis and Dissertation Repository
Abstract
Aim: To explore the perspectives and practice of psychotherapy of family physicians, and analyze Ontario’s mental health policy documents within the context of psychotherapy.
Methods: In the first study descriptive qualitative methodology was used. Eighteen family physicians were interviewed and the results were presented thematically. In the second study, a policy analysis of Ontario’s mental health policy documents was conducted. The qualitative policy analytic methodology of Peter et al. was used.
Findings: Psychotherapy is an effective treatment of mental illness and is practiced by family physicians. Lack of training and time are barriers, while patient needs and …
The Symptom Of Fatigue In Primary Care: A Comparative Study Of Health Care Utilization Patterns Using Electronic Medical Records, Kathryn A. Nicholson
The Symptom Of Fatigue In Primary Care: A Comparative Study Of Health Care Utilization Patterns Using Electronic Medical Records, Kathryn A. Nicholson
Electronic Thesis and Dissertation Repository
The experience of fatigue has been a common problem among community populations for decades. The symptom of fatigue is also a frequent health complaint in primary care practices. Fatigue patients have been shown to be significantly high users of health care resources, particularly when compared to non-fatigued patient populations. To date, an understanding of the presence and the impact of the symptom of fatigue within the context of the Canadian health care system has been understudied.
The objective of this research was to determine the number of patients who were suffering from the symptom of fatigue within a primary care …
