Problem: Organizations have taken measures to reduce the length of stay by addressing logistical issues such as care coordination across the continuum. This becomes especially important for patients being discharged to skilled nursing facilities (SNF), where placement can be impeded due to complex care needs. In March 2021, the facility accounted for 20% of all extended length of stay (ELOS) patients across the organization. This quality improvement (QI) project evaluated the length of stay (LOS) of low-risk chest pain (CP) patients in a rural hospital. A convenience sample of emergency department (ED) physicians and non-physician providers ages 18-75 employed at Navicent Health Baldwin were eligible for recruitment. Pre- and post-surveys were used to evaluate to evaluate ED providers’ knowledge of low-risk chest pain metrics for statistical significance and a comparison of LOS for low-risk chest pain patients was compared for clinical and statistical significance seen at the facility. The trauma service in a large academic tertiary medical center admits a large proportion of patients with the secondary diagnosis of alcohol use disorder. Given the successful use of phenobarbital in the critical care unit for withdrawal prophylaxis and treatment of acute withdrawal, a quality improvement project was established to create and implement guidelines for the non ICU patient.

A root cause analysis demonstrated several issues to include inconsistent clinical decision documentation. As a result, several countermeasures were initiated to address the various issues.

However, evidence shows collaboration between all disciplines involved in patient care leads to improved patient outcomes. Interprofessional collaboration can reduce costs and improve patient care, yet is used inconsistently in health care systems. Studies on homelessness have shown that people who are homeless are admitted to hospital more frequently, for longer periods of time, and at a younger age than people who are housed.
