Societal expectations raise the issue of the nature of a profession and a profession's relationship with society. Influential policy leaders want reform of the oral health workforce and delivery system in such a manner as to ensure that improvements are made for accessing care, particularly for vulnerable and disadvantaged populations, especially children. This essay is based on a presentation to the House of Delegates of the California Dental Association on Nov.13, 2009.

The vast majority of periodontal pathogens are classified as gram negative bacteria. These bacteria have an outermost membrane consisting of lipopolysaccharide (LPS), a molecule that can act as an endotoxin and elicit an immune response. The lipopolysaccharide structure consists of three major regions: oligosaccharide, core, and lipid A. Oligosaccharide is exposed on the cell surface, and lipid A anchors LPS in the outer membrane. LPS has been known to hinder certain defensive mechanisms of the immune system by secreting products that interfere with signals that promote chemotaxis of leukocytes enabling the pathogens to go unrestricted. Those with physical and mental disabilities are at an increased risk of oral complications such as gingivitis and periodontal disease. In comparison with this risk, this population struggles to access appropriate dental care that accommodates those with disabilities. This thesis reviews the demographics of those with special needs, dental education barriers to working with this population and financial barriers that hinder access to care. An overview of the oral health problems of common disabilities is presented as well as behavioral management techniques that could be easily implemented in the dental office. The maxillary lateral incisor is the second most frequently missing tooth in the dental arch with clinical management requiring a complex and multidisciplinary treatment approach. It has been suspected that teeth could be smaller in the maxillary or mandibular arches in patients experiencing agenesis of the maxillary lateral incisor, making ideal occlusal relationships and space creation for restoration of the lateral incisor difficult to obtain. The purpose of this study is to determine if a tooth size discrepancy exists in orthodontic patients with agenesis of the maxillary lateral incisor(s).

OBJECTIVE: To assess the discrepancies in marginal integrity and contact points of Porcelain Fused to Metal (PFM) crowns and its association with caries in adjacent teeth.

STUDY DESIGN: Cross-sectional study.

PLACE AND DURATION OF STUDY: Dental Section of the Aga Khan University Hospital, Karachi, from January to August 2009.

METHODOLOGY: Ninety six (96) patients with PFM crowns on maxillary and mandibular premolars and molars were included in this study.
