Background and Purpose: Whole-body vibration (WBV) is a relatively new form of exercise training that may influence muscle performance. This study investigated the acute effects of high (26 Hz) and low (2 Hz) frequency WBV on isometric muscle torque of the quadriceps and hamstrings in persons with multiple sclerosis (MS).

Recent research has demonstrated body mass (M) bias in military physical fi tness tests favoring lighter, not just leaner, service members. Mathematical modeling predicts that a distance run carrying a backpack of 30 lbs would eliminate M-bias. The purpose of this study was to empirically test this prediction for the U.S. Army push-ups and 2-mile run tests. Two tests were performed for both events for each of 56 university Reserve Offi cer Training Corps male cadets: with (loaded) and without backpack (unloaded). Results indicated signifi cant M-bias in the unloaded and no M-bias in the loaded condition for both events. …

Thoracolumbar braces are commonly used to treat Adolescent Idiopathic Scoliosis (AIS). Braces serve to reduce and prevent progression of the spinal curve by applying corrective forces. The magnitude and direction of these corrective forces applied by the brace to the spine remain unknown. Additionally the brace fitting process involves making alterations to the brace that affect its corrective force capacity. The objective was to design and validate an analog model of a mid-thoracic single curve scoliotic deformity for quantifying structural properties of the brace and the force response of the brace on the spine. Early language delay has often been associated with atypical language/literacy development. Neuroimaging studies further indicate functional disruptions during language and print processing in school-age children with a retrospective report of early language delay. Behavioral data of 114 5-year-olds with a retrospective report of early language delay in infancy (N = 34) and those without (N = 80) and with a familial risk for dyslexia and those without are presented. Behaviorally, children with a retrospective report of early language delay exhibited reduced performance in language/reading-related measures. A voxel-based morphometry analysis in a subset (N = 46) demonstrated an association between reduced …

Objective: The purpose of this thesis was to investigate the inter-rater reliability of the McKenzie System of Mechanical Diagnosis and Therapy (MDT) when classifying patients with musculoskeletal knee pain using clinical vignettes. Methods: This study was divided into two phases. First, ten clinicians experienced in the use of MDT were randomly recruited to write a total of 60 clinical vignettes based upon the initial assessment of past patients with knee pain. Second, six different MDT raters were recruited to rate 53 selected vignettes and reliability was determined using Fleiss Kappa. Results: There was “substantial agreement” among six MDT raters …

The Simple View of reading emphasizes the critical role of two factors in normal reading skills: word recognition and reading comprehension. The current study aims to identify the anatomical support for aspects of reading performance that fall within these two components. Fractional anisotropy (FA) values were obtained from Diffusion Tensor images in twenty-one typical adolescents and young adults using the Tract Based Spatial Statistics (TBSS) method. We focused on the Arcuate Fasciculus (AF) and Inferior Longitudinal Fasciculus (ILF) as fiber tracts that connect regions already implicated in the distributed cortical network for reading. Abstract
Objective: Present a clinic case demonstrating the effectiveness of the Mulligan Concept (MC) in treating an apparent meniscal injury. The utilization of the MC in the evaluation and treatment of a 20-year-old soccer player with an apparent acute meniscal injury is presented. Background: Meniscal injuries are common knee injuries. The MC is a therapeutic intervention strategy applied as both a treatment-based evaluation and therapeutic intervention. Treatment: The patient was successfully treated in four treatment sessions using the MC. Managing the physical aspects of lymphedema requires an individual to be committed to daily treatment of this chronic condition. Performing manual lymph drainage, using compression bandaging or compression garments, exercising, and caring for the skin all take time and a high level of dedication. This commitment to self-care can be emotionally and psychologically exhausting. When coupled with other emotional stressors in a patient’s life, successful treatment of lymphedema is challenging. We present a case demonstrating how the physical and psychological aspects of care interplay, and, when well-managed, can positively affect the outcome.

The Reading Acceleration Program, a computerized reading-training program, increases activation in neural circuits related to reading.We examined the effect of the training on the functional connectivity between independent components related to visual processing, executive functions, attention, memory, and language during rest after the training. Children 8–12 years old with reading difficulties and typical readers participated in the study. Behavioral testing and functional magnetic resonance imaging were performed before and after the training. Imaging data were analyzed using an independent component analysis approach. After training, both reading groups showed increased single-word contextual reading and reading comprehension scores. Greater positive correlations between …

Objective evaluation of language function is critical for children with intractable epilepsy under consideration for epilepsy surgery. The purpose of this preliminary study was to evaluate word recognition in children with intractable epilepsy by using magnetoencephalography (MEG). Ten children with intractable epilepsy (M/F 6/4, mean ± SD 13.4 ± 2.2 years) were matched on age and sex to healthy controls. Common nouns were presented simultaneously from visual and auditory sensory inputs in “match” and “mismatch” conditions. Balance is among the most challenging tasks for patients with movement disorders. Study and treatment of these disorders could greatly benefit from combined software tools that offer better insights into neuromuscular biomechanics, and predictive capabilities for optimal surgical and rehabilitation treatment planning. A platform was created to combine musculoskeletal modeling, closed-loop forward dynamic simulation, optimization techniques, and neuromuscular control system design. Spinal (stretch-reflex) and supraspinal (operational space task-based) controllers were developed to test simulation-based hypotheses related to balance recovery and movement control. The objective of this study was to investigate the changes that occur in the gait of a set of subjects after they ride a horse. Eleven riders with a developmental disability were selected and included in an experimental group and nineteen “typical” subjects or those without a developmental disability were included as a second comparison group. The length of time the subjects were able to remain standing in a tandem stance and in a one-legged stance without becoming unbalanced was measured and recorded. Background and purpose: The purpose of this case report is to describe vestibular, oculomotor, and cervical interventions that were theorized to improve the function and decrease symptoms typically seen in the post-concussion population.
Case Description: The patient was a 16 year old female who was status post concussion due to head trauma during gymnastics routine. Symptoms included short term memory loss, fatigue, drowsiness, dizziness, difficulty concentrating, headaches, visual disturbances, and neck pain.

Primary spinal hydatid cysts are uncommon and extradural involvement is rare. We report an unusual case of Paraplegia due to hydatid disease of primary site of infection extra spinal muscles in thoracic region from T5 to T8 level extending into a daughter cyst at T9 level, is main cause of acute Paraplegia. Magnetic resonance imaging (MRI) showed an extradural round cystic lesion in the spinal canal filling the posterior and left postero-lateral expect at T9 level, markedly compressing the cord. Though increasing age and body weight (BW) have been widely known to be associated with slower distance run times, the common convention in 5K road races is to categorize competitors by age and, sometimes, BW. This has the disadvantage of assigning only small numbers of competitors to age categories and giving advantage to runners close to the minimum age or BW values allowable. Recent research has empirically documented a consistent penalty against heavier service members for events identical or similar to those in the physical fitness tests of the Army, Air Force, and Navy. These penalties, not related to body fatness, are based on biological scaling models and have a physiologic basis. Using hypothetical cases, we quantified the penalties for males, 60 vs. 90 kg body weight, and females, 45 vs. 75 kg, to be 15-20% for the fitness tests of these three services. Such penalties alone can adversely impact awards and promotions for heavier service members. To deal equitably with these penalties …

Recently, a fitness competition called the Pump and Run (PR) has been popularized. Composed of 2 events, a 5-km road race time (RT) in seconds and a maximal-repetition bench press (BPR) with resistance based on a percentage of body mass (M), the final score (RTadj) equals RT - 30(BPR). From published findings, the authors hypothesized that the PR would impose a bias against heavier competitors. Furthermore, the potential for age bias in this event has not been evaluated. Recent research findings combined with the theoretical laws of biological similarity make the compelling case that all physical fitness test items for the Army, Air Force, and Navy impose a 15-20% physiologic bias against heavier, not fatter, men and women. Using the published findings that actual scores of muscle and aerobic endurance scale by body mass raised to the 1/3 power, correction factor tables were developed. This correction factor can be multiplied by one’s actual score (e.g., push-ups, sit-ups, abdominal crunches, or curl-ups repetitions or distance run time) to yield adjusted scores that are free of body mass bias. The adverse effect of increasing age and/or body weight on distance run performance has been well documented. Accordingly, nearly all five kilometer (5K) road races employ age categories and, sometimes, a heavier body weight classification. Problems with such conventions include small numbers of runners within older age categories and the advantage given to the lightest runners within each weight category. We developed a 5K Handicap (5KH), a model that calculates an adjusted run time based on the inputs of actual 5K run time, age, and body weight for men and women.
