Atrial Fibrillation, Julianna G. Perritt, Jennifer Goldsberry

The Corinthian

Atrial fibrillation, also known as A-Fib or AF, is a cardiovascular abnormality that is characterized by an abnormal heart rhythm or muscle contractions in the atria. These abnormalities are a result of disorganized electrical impulses. AF has various causes including, but not limited to, hypertension (HTN), heart surgery, thyroid disorders, heart defects, and acute alcohol ingestion. Persons with AF are often asymptomatic; however, some patients report heart palpitations, chest tightness, shortness of breath, and lightheadedness. A review of literature was conducted in order to determine the availability of risk factors, patient education, and treatment of AF.

Journal of Community Hospital Internal Medicine Perspectives

Carnitine deficiency is a rare metabolic condition that can result in fasting hypoglycemia. Carnitine deficiency could be primary or secondary to other conditions. Among secondary causes, antiepileptics such as valproic acid have been incriminated. Valproic acid is known to deplete carnitine stores and inhibit the process of β-oxidation. Herein we report the case of a 44-year-old female with epilepsy that presented with breakthrough seizures associated with hypoglycemia despite being on appropriate antiepileptic therapy. The patient was later found to have carnitine deficiency. Discontinuation of valproic acid and supplementation with L-carnitine resolved the patient’s hypoglycemia and breakthrough seizures. With this case …

The Case For Using Composition Tomography To Evaluate Perirectal Necrotizing Fasciitis: Is It Really Necessary?, Tyler Bayliss, David Denning

Marshall Journal of Medicine

Clinical reviews of Fournier’s Gangrene state that diagnostic protocol includes interpreting CT, MRI, or Ultrasound imaging along with clinical symptoms and lab findings. We think that the use of imaging techniques as a diagnostic tool is no longer needed as Fournier’s Gangrene is specific enough of a disease that clinicians can diagnose using labs and physical examination alone.

Cases of perirectal necrotizing soft tissue infection recorded at St. Marys Medical Center were reviewed. Results of physical exams and imaging were compared along with measurements of severity upon admittance and length of stay. Due to the COVID-19 pandemic, we investigated if …

The COVID-19 pandemic saw rapid adoption of telehealth, including remote patient monitoring (RPM). There is limited evidence about how patients and staff experience such services in New Zealand. This study aimed to understand the acceptability of the RPM experience, particularly for Maori and Pacific peoples, and identify strengths, gaps, and limitations to inform future delivery of services. A mixed methods study was undertaken between 4 July and 11 September 2022 in Auckland. We conducted telephone surveys with patients and semi-structured interviews with patients and staff. Survey, and clinical and administrative data were analysed descriptively using SPSS. Interviews were analysed using …

A Responsible Parrhesia? A Review Of The Price Of Secrecy, Sara Tafakori Apr 2024

A Responsible Parrhesia? A Review Of The Price Of Secrecy, Sara Tafakori

RadioDoc Review

The Price of Secrecy immerses the listener in stories of individual trauma, of child abuse and rape, yet also draws lessons from them of wider social significance. It includes moments of narrative catharsis, interspersed with repeated reminders that the stories are unfinished and open-ended—that the solutions lie out there, in social action, rather than in the stories themselves. The series also gestures towards structural critique, especially of ‘the legal constraints’ it identifies, yet it places greater importance on changing the wider culture through challenging the culture of secrecy and shame around victims’ stories of rape and abuse. This centrally means …

A Case Of Siadh After Bowel Preparation For Routine Colonoscopy, Joseph C. Rath, Allison M. Hemmer, Kennedy A. Forest, Kahlen R. Morris, Mary E. Watson

Aesculapius Journal (Health Sciences & Medicine)

Electrolyte abnormalities, including hyponatremia, are a known outcome of bowel preparations used for colonoscopy. Severe and symptomatic hyponatremia after bowel preparation is uncommon and occurs due to excessive release of ADH among other contributing factors. Risk factors for developing hyponatremia after bowel preparation include increased age, female gender, kidney disease, heart failure, and use of various medications. We present the case of a 78-year-old female who presented with altered mental status and possible seizures after completing bowel preparation with polyethylene glycol for a colonoscopy. Serum and urine studies were consistent with SIADH. She required fluid restriction, hypertonic saline, and sodium …

Evaluation Of Nursing Approach To Assessment Of Post-Operative Respiratory Depression Using A Simulation Model, Natalia Tarasova, Usha Asirvatham, Robin D. Goetz, Mariela Riveria, Juraj Sprung, Toby N. Weingarten Feb 2024

Evaluation Of Nursing Approach To Assessment Of Post-Operative Respiratory Depression Using A Simulation Model, Natalia Tarasova, Usha Asirvatham, Robin D. Goetz, Mariela Riveria, Juraj Sprung, Toby N. Weingarten

Journal of Perioperative Nursing

Introduction: Assessments of post-operative patients that have been carried out by health care providers before critical opioid-induced respiratory events often do not detect respiratory depression. We hypothesise that opioidinduced respiratory patterns present during sleep may not be properly recognised as providers typically awaken patients for vital sign checks, and awake state assessment is recorded. We used a simulation manikin model to test this hypothesis.

Methods: Nurses who work on a standard post-operative hospital ward volunteered to participate in a study designed to record vital signs on an adult male manikin. None of the nurses had formal critical care or postoperative …

Hypothermia Versus Normothermia As Intervention To Reduce Mortality Rates Following Out Of Hospital Cardiac Arrest, Cora Blunden

Soaring: A Journal of Undergraduate Research

Abstract

Background: Cardiac arrest contributes to 300,000 to 450,000 deaths in the United States every year. Individuals who survive cardiac arrest often still die after resuscitation. Induced hypothermia is often an intervention used for individuals after being resuscitated after a cardiac arrest and has become standard practice for several institutions. There is significant motivation to see if induced hypothermic states promote improved mortality rates for patients who experience cardiac arrests.

Objective: The purpose of this paper is to determine the effectiveness of induced hypothermia versus normothermia on out-of-hospital cardiac arrest mortality rates. Methods: A review of recent literature in the …
