10-Minute Ebd: Preferred Analgesia For Orthodontic Tooth Movement: Acetaminophen Or Nsaids?, Ryan N. Mcgovern

The Journal of the Michigan Dental Association

This 10-Minute EBD column explores the choice between acetaminophen and NSAIDs for managing pain during orthodontic tooth movement. The author presents a scenario involving a patient's discomfort after archwire adjustment and delves into the impact of tooth movement on the periodontium. The PICO question focuses on the effectiveness of NSAIDs and acetaminophen in controlling discomfort without impeding tooth movement. The literature search reveals varying recommendations on analgesics, with some studies favoring acetaminophen. The evidence suggests that acetaminophen and short-term celecoxib are suitable choices for orthodontic analgesia. However, caution is advised in extrapolating findings from animal studies to human conditions.

10-Minute Ebd: Digital Scans Or Conventional Impressions: Which Results In Better Crowns?, Laura Tsu Dds

The Journal of the Michigan Dental Association

This 10-Minute EBD column critically evaluates the outcomes of digital scans versus conventional impressions in fabricating single-unit crowns, focusing on factors such as marginal and internal fit. Employing a PICO question and a literature search pathway via PubMed, the analysis includes systematic reviews and meta-analyses. Findings suggest that crowns from digital scans generally exhibit clinically acceptable marginal and internal fit, often outperforming conventional impressions. However, heterogeneity in study samples and methodologies contributes to varied results. While digital scans meet the standard of care, ongoing research is essential for a comprehensive understanding of the evolving landscape in digital prosthodontics.

Genetic Duties, Jessica L. Roberts, Alexandra L. Foulkes Oct 2020

Genetic Duties, Jessica L. Roberts, Alexandra L. Foulkes

William & Mary Law Review

Most of our genetic information does not change, yet the results of our genetic tests might. Labs reclassify genetic variants in response to advances in genetic science. As a result, a person who took a test in 2010 could take the same test with the same lab in 2020 and get a different result. However, no legal duty requires labs or physicians to inform patients when a lab reclassifies a variant, even if the reclassification communicates clinically actionable information. This Article considers the need for such duties and their potential challenges. In so doing, it offers much-needed guidance to physicians …

We will argue that truly informed decision making can help to reduce excessive end-of-life care by any measure. Most importantly for dying patients, better informed decisions can help reduce unnecessary suffering and result in care that aligns with their well-considered values and preferences.

In the first part of this paper, we will explain that, by any of these measures, many dying patients are receiving too much therapy and …
