Advances in Clinical Medical Research and Healthcare Delivery

In 2022, 4722 osteopathic medical students chose to take the United States Medical Licensing Examination Step 1 (USMLE Step 1) in addition to the Comprehensive Osteopathic Medical Licensing Examination Level 1 (COMLEX Level 1). Both of these examinations are currently pass/fail assessments to demonstrate clinical knowledge competency. Osteopathic medical students are only required to take the COMLEX series of examinations for graduation from an osteopathic medical school while allopathic medical students are only required to take the USMLE Step series of examinations instead. The Future Of Medicine Is Behind Us: The Case For A 1912 Case-Based Curricula, Aaron Mcguffin

Patient outcomes continue to suffer despite exponential advances in access to medical knowledge. This requires an evaluation of the pedagogy of our medical education delivery. Case-based learning has long been shown to be an effective teaching method to improve student knowledge, student and faculty engagement, and improve patient outcomes. In order to improve patient outcomes specifically in accurate and timely diagnosis, we need to strongly consider converting our existing curricular models entirely to a case-based format.

Objective: Applicant demographics during the 2019-2020 residency cycle were evaluated to determine if strict utilization of United States Medical Licensing Examination (USMLE) scores in applicant selection could lead to a restriction in diversity.

Study design: Cross-sectional study.

Setting: Otolaryngology residency applicants to a single institution.

Methods: A total of 381 applicants were analyzed by age, gender, applicant type, race/ethnicity, USMLE scores, permanent zip code, and graduating medical school.

Results: Among applicants, 37% were women; 9% were ≥30 years of age; 12% were underrepresented minorities (URMs); 71% to 81% had above-average socioeconomic surrogate markers; 22% were from a top 25 US …

In February of 2020 the sponsors of the USMLE examinations, which includes the NBME and the Federation of State Medical Boards (FSMB) decided to change the scoring system of the USMLE Step 1 from a three-digit numeric score to a Pass/Fail outcome1. The basis of this change was made to “address concerns about Step 1 scores impacting student well-being and medical education”2. While previous studies have analyzed the relationship of pre-admission factors on USMLE Step 1 scores few studies have looked at the effects obtaining a graduate degree prior to medical school matriculation. Therefore, this study sought to determine if there was an advantage between obtaining a graduate degree before medical school when it came to step 1 board scores. This retrospective study analyzed step 1 scores and medical school application data from 718 students who matriculated at a public midwestern medical school between 2014 and 2016. Research indicates that medical students experience unusually high levels of stress, anxiety, depression, and loss of empathy while in medical school. As the number of medical students rises and residency positions remain stagnant, the USMLE receives greater emphasis as a barometer to determine residency placement and future career paths thereby increasing levels of stress among students. Stress is associated with diminished self-care, potentially leading to a negative impact on well-being. This ethnographic, qualitative study sought to examine the extent to which the anticipation of, preparation for, and implications of the USMLE contribute to medical student stress, wellness, and self-care. Purpose: The aim of the study is to demonstrate whether resident performance on the United States Medical Licensing Exam (USMLE) Step 1 and the Ophthalmology Knowledge Assessment Program (OKAP) correlates to American Board of Ophthalmology Written Qualifying Examination (ABO-WQE).

Design: This is a retrospective cohort study.

Participants: Forty-one residents who completed their postgraduate training between 1999 and 2011 from our institution were included in the study.
