Successful Percutaneous Thoracic Duct Embolization For Chylothorax After Total Arch Replacement, Fumihiro Kitashima, Naohiro Shimada, Yojiro Machii, Masashi Tanaka
The Texas Heart Institute Journal
Chylothorax after cardiac surgery is a rare complication associated with severe morbidity and mortality. This report documents successful treatment with percutaneous thoracic duct embolization for chylothorax after total arch replacement. A 69-year-old man underwent replacement of the aortic arch to treat a ruptured aortic aneurysm. After surgery, the left thoracic drain discharged 2,000 to 3,000 mL serosanguineous fluid per day, even though the patient took nothing orally and was administered subcutaneous octreotide therapy. On postoperative day 9, percutaneous thoracic duct embolization was performed, and the drain could be removed. The chylothorax did not recur, and the patient was discharged on …

The rapid advancement of the COVID-19 pandemic has prompted an accelerated pursuit to identify effective therapeutics. Stages of the disease course have been defined by viral burden, lung pathology, and progression through phases of the immune response. Immunological factors including inflammatory cell infiltration and cytokine storm have been associated with severe disease and death. Many immunomodulatory therapies for COVID-19 are currently being investigated, and preliminary results support the premise of targeting the immune response. However, because suppressing immune mechanisms could also impact the clearance of the virus in the early stages of infection, therapeutic success is likely to depend on …

BACKGROUND: Stroke remains a leading cause of death and disability worldwide despite recent treatment breakthroughs. A primary event in stroke pathogenesis is the development of a potent and deleterious local and peripheral inflammatory response regulated by the pro-inflammatory cytokine interleukin-1 (IL-1). While the role of IL-1β (main released isoform) has been well studied in stroke, the role of the IL-1α isoform remains largely unknown. With increasing utilization of intravenous tissue plasminogen activator (t-PA) or thrombectomy to pharmacologically or mechanically remove ischemic stroke causing blood clots, respectively, there is interest in pairing successful cerebrovascular recanalization with neurotherapeutic pharmacological interventions (Fraser et …

The promise of idiotype-based therapeutics has been disappointing forcing a new look at the concept and its potential to generate an effective approach for immunotherapy. Here, the idiotype network theory is revisited with regard to the development of efficacious anti-idiotype vaccines. The experience of polyclonal anti-Idiotype reagents in animal models as well as an understanding of the immune response in humans lends to the proposition that polyclonal anti-Idiotype vaccines will be more effective compared to monoclonal-based anti-Idiotype vaccines. This novel strategy can be adapted in Biotech-standard production of therapeutic antibodies.

The objective of this scholarly project is to determine the impact of patient-centered communication education in an emergency department (ED) on the perception of workplace safety. The ED is a vulnerable setting and susceptible to workplace violence (WPV) due in part to increasing numbers of patients presenting to EDs with primary psychiatric complaints. High-quality patient-staff interactions correlate positively with better treatment outcomes. Patient-centered communication skills can be taught, and patients have similar expectations of what patient-centered communication means no matter their diagnoses. The purpose of this quality improvement project was to conduct patient-centered communication education with nurses, nursing assistants, and …

Effects Of Therapeutic Music On Pain In Spinal Surgery Recovery, Michael Poulsen
Effects Of Therapeutic Music On Pain In Spinal Surgery Recovery, Michael Poulsen
Evidence-Based Practice Project Reports
Pain is one of the most common experienced symptoms reported by more than 80% of postoperative patients. Approximately 77-98% of postoperative patients report pain following their procedure with 40- 80% having moderate to severe pain. Pain is shown to elevate stress levels manifesting in increased heart rates, blood pressures, and oxygen demand. Inadequate pain control can develop into surgical complications causing surgical failure, blood clots, pneumonia, and chronic pain. Complimentary and alternative medicine such as music can be used in combination with opioid medication help improve pain control leading to successful surgical outcomes. The purpose of this evidence-based practice project …

Illawarra Health and Medical Research Institute
This work presents an in-depth analysis into the dependencies of radiosensitisation on X-ray beam energy, particle morphology and particle concentration for Ta2O5 nanostructured particles. A maximum sensitisation enhancement ratio of 1.46 was attained with irradiation of a 10 MV x-ray photon beam on 9L cells exposed to the less aggregated form of NSPs at 500 μg/mL-1. A significant increase in sensitisation of 30% was noted at 150 kVp for irradiation of the less aggregated form of tantalum pentoxide NSPs compared to its more agglomerated counterpart. Interestingly, no differences in sensitisation were observed between 50 and …
Unraveling And Understanding The Therapeutic Self: A Heuristic Inquiry On Countertransference, Donna Decotiis
Unraveling And Understanding The Therapeutic Self: A Heuristic Inquiry On Countertransference, Donna Decotiis
Creative Arts Therapies Theses
The purpose of this heuristic inquiry was to discover how my countertransference informed my clinical internship experience. My research questions were: How does my countertransference inform my therapeutic relationships while working with survivors of domestic violence? How does my countertransference impact my clinical choices and my development as a dance/movement therapist? Data was collected through semi-structured journal entries that included both writing and art-making; this occurred three times per week during the last five weeks of my internship. I analyzed data sequentially through creative synthesis with a research consultant once per week, which began two weeks after data collection. Findings …

A Proposed Therapeutic Algorithm Based On Multiple Case Analysis Regarding The Repair Options Of Iatrogenic Biliary Lesions Following Open And Laparoscopic Surgery, Cosmin Alec Moldovan, Dan Florin Ungureanu, Vladimir Beliș
Journal of Mind and Medical Sciences
Introduction: the problem of the iatrogenic lesions of the main biliary pathways is far from being completely clarified and still represents a serious surgical situation, during both open and laparoscopic surgeries. The outcome of these situations is closely linked with the actual moment of discovery of the lesion and the surgical methods for repairing such defects and implies a great number of options, such as reconstructive, derivative and substitutive techniques. Objectives: the goal of this paper is to present an algorithm of choices and their consequences for clinical conduit in the case of iatrogenic biliary lesions discovered in …

Learning To Relate: A Heuristic Inquiry Exploring The Development Of Therapeutic Relationships, Megan King
Creative Arts Therapies Theses
This heuristic inquiry explored the creation of therapeutic relationships. The intention of this research was to learn how to build therapeutic relationships with clients that I felt uncomfortable around, with whom I did not have much experience and whom I did not want to relate to due my personal prejudices around severe illnesses and extreme developmental and cognitive delays. This study was important not only to expand my knowledge and experiences working with these populations but to also inform my future clinical experiences with new clients as I embark on my professional career. I collected data through reflective journaling and …
